Pharmacological properties
Pharmacodynamics
Bismuth tripotassium dicitrate is the active substance of Vitridinol, an antiulcer agent with bactericidal activity against Helicobacter pylori. It also has anti-inflammatory and astringent effects. When bismuth oxide enters the acidic environment of the stomach, insoluble citrate and bismuth oxychloride are precipitated, chelated compounds with a protein substrate are formed in the form of a protective film on the surface of erosions and ulcers.
By increasing the biosynthesis of prostaglandin E, the formation of mucus and the secretion of bicarbonate, bismuth oxide stimulates the activity of cytoprotective mechanisms, helps to increase the resistance of the mucous membrane of the gastrointestinal tract to the effects of salts and enzymes of bile acids, hydrochloric acid, pepsin. Also, the substance reduces the activity of pepsinogen and pepsin, leading to the accumulation of epidermal growth factor in the area of the defect.
Pharmacokinetics
Bismuth tripotassium dicitrate is practically not absorbed from the gastrointestinal tract. Excretion is carried out mainly with feces. Bismuth, which entered the plasma in a small amount, is excreted from the body by the kidneys.
Indications for use
- gastroduodenitis, chronic gastritis, gastric ulcer and duodenal ulcer in the acute phase, including those associated with Helicobacter pylori;
- functional dyspepsia, which is not associated with organic diseases of the gastrointestinal tract;
- irritable bowel syndrome, accompanied mainly by signs of diarrhea.
Contraindications
- impaired renal function;
- pregnancy and lactation;
- age up to 4 years;
- individual intolerance to the components of the drug.
Vitridinol, instructions for use: method and dosage
Vitridinol tablets are taken orally 30 minutes before meals or 2 hours after the last meal and washed down with a little water.
Recommended dosage regimen:
- children 4–8 years old: daily dose - 8 mg / kg in 1–2 doses;
- children 8-12 years old: 2 times a day, 1 tablet (120 mg);
- children from 12 years old and adults: 4 times a day, 1 tablet (120 mg) or 2 times a day, 2 tablets (240 mg).
The duration of the course is from 28 to 56 days. At the end of treatment, preparations containing bismuth should not be taken for the next 8 weeks.
For the eradication of Helicobacter pylori, it is advisable to use Vitridinol in combination with other antibiotics that have anti-Helicobacter activity.
Side effects
- digestive system: nausea, vomiting, more frequent stools, constipation (these undesirable phenomena are temporary and not dangerous for health);
- allergic reactions: itching and rash;
- others: with a long course using high doses - encephalopathy (due to the accumulation of bismuth in the central nervous system).
Overdose
The main symptom of an overdose associated with prolonged use of Vitridinol in doses exceeding recommended is renal dysfunction (completely reversible after discontinuation of therapy).
Therapy: gastric lavage, intake of saline laxatives and activated charcoal. In the future, symptomatic treatment is carried out. In case of impaired renal function, which is accompanied by a high plasma level of bismuth in the blood, complexing agents can be administered - dimercaptosuccinic and dimercaptopropanesulfonic acids. Patients with severe renal impairment are prescribed hemodialysis.
special instructions
Vitridinol should not be taken for longer than 8 weeks.
During the period of therapy, it is impossible to use other drugs containing bismuth, since an increase in the concentration of bismuth in the blood can lead to the development of systemic adverse reactions.
During treatment, feces may become dark in color, which is associated with the formation of bismuth sulfide. Some patients experience a slight darkening of the tongue.
Application during pregnancy and lactation
Vitridinol is not prescribed during pregnancy / lactation.
Pediatric use
It is contraindicated to carry out therapy in patients under 4 years of age.
With impaired renal function
Vitridinol tablets for violations of renal function are contraindicated.
