Sefpotec is an antibacterial drug, a third generation cephalosporin.

Release form and composition

Sefpotec is produced in the form of film-coated tablets: white, oblong, with a dividing line on one side; the core of the tablet is cream-colored (in blisters: 5 pcs., in a cardboard box 2 or 4 blisters; 7 pcs., in a cardboard box 2 blisters).

1 tablet contains:

  • active substance: cefpodoxime proxetil - in a dose equivalent to 200 mg of cefpodoxime;
  • auxiliary components: Avicel RC 591 (sodium carmellose, microcrystalline cellulose), colloidal silicon dioxide, hyprolose, magnesium stearate, sodium lauryl sulfate;
  • the composition of the film shell: Sepifilm LP761 Blanc - titanium dioxide (E171), microcrystalline cellulose (E460), hypromellose (E464), stearic acid (E570).

Pharmacological properties

Pharmacodynamics

Sefpotec is an antibiotic that has a bactericidal effect. The active substance of the drug - cefpodoxime, belongs to the group of III generation cephalosporins. The mechanism of action of the drug is due to the ability of cefpodoxime to disrupt the synthesis of the bacterial cell wall. The drug is resistant to beta-lactamases.

The following microorganisms are sensitive to Sefpotek:

  • gram-positive bacteria: Streptococcus pneumoniae (except for penicillin-resistant strains), Streptococcus pyogenes, Staphylococcus aureus (including strains producing penicillinase);
  • gram-negative bacteria: Haemophilus parainfluenzae (positive and negative strains of beta-lactamases), Escherichia coli, Haemophilus influenzae, Salmonella speciales (spp.), Klebsiella spp. (including Klebsiella oxytoca, Klebsiella pneumoniae), Moraxella (Branhamella) catarrhalis, Neisseria gonorrhoeae, Providencia, an indole-positive strain Proteus, other Proteus species (including Proteus vulgaris), Enterobacter (including Enterobacter cloacogenes, Enterobacterella aerppae.

In vitro, cefpodoxime is active against most strains of bacteria such as Aeromonas hydrophila, Yersinia enterocolitica, Brucella, Providencia rettgeri, Providencia stuartii, Neisseria, Morganella, Serratia, Citrobacter.

In relation to anaerobes, cefpodoxime exhibits weak activity, including against most species of Campylobacter, Bacteroides, Yersinia. The following microorganisms are resistant to the drug: Streptococcus spp. (group D), Corynebacterium spp. (groups J, K), methicillin-resistant strains of Staphylococcus spp., Pseudomonas spp., Pseudomonas aeruginosa, Acinetobacter baumannii, Listeria monocytogenes, Bacteroides spp., Clostridium difficile. Cephalosporinases of chromosomal origin Enterobacter, Citrobacter, Bacteroides destroy it.

Pharmacokinetics

After taking Sefpoteka inside cefpodoxime, proxetil is absorbed in the small intestine and turns into an active metabolite of cefpodoxime. The absolute bioavailability of the drug is 50%.

Single and repeated administration at a dose of 100–400 mg allows in 1.9–3.1 hours to reach a therapeutic concentration of the drug in the amount of 1–4.5 mg / l.

Binding to blood proteins - 20-30% of the dose.

Through the kidneys, within 12 hours, 30–35% of the dose is excreted unchanged.

T 1/2 (half-life) is 2.1-2.8 hours.

In case of impaired renal function, T 1/2 is lengthened: with creatinine clearance (CC) 50-80 ml / min, it is 3.5 hours, CC 30-49 ml / min - 5.9 hours, CC 5-29 ml / min - 9.8 hours.

In old age, including patients with bronchopulmonary infection, a slight lengthening of T 1/2 and an increase in the level of drug concentration in the blood does not require correction of the dosage regimen.

Indications for use

The use of Sefpoteka is indicated for the treatment of the following infectious and inflammatory diseases, which are caused by microorganisms sensitive to cefpodoxime:

  • oral cavity infections;
  • tonsillitis, acute sinusitis, pharyngitis, acute bronchitis, pneumonia;
  • exacerbation of chronic bronchitis (in smokers, alcohol abusers, in patients over the age of 65);
  • infections of the gastrointestinal tract;
  • cystitis and other uncomplicated infections of the urinary system;
  • infections of soft tissues and skin;
  • uncomplicated infections of the anorectal region in women caused by Neisseria gonorrhoeae;
  • uncomplicated form of gonorrhea.

Contraindications

  • age up to 12 years;
  • breast-feeding;
  • hypersensitivity to cephalosporins;
  • individual intolerance to the components of the drug.

According to the instructions, Sefpotec should be used with caution in chronic colitis, renal failure, hypersensitivity to penicillins, concomitant therapy with loop diuretics and other nephrotoxic drugs, during pregnancy.

Instructions for use of Sefpoteka: method and dosage

Sefpotec tablets are taken orally, with meals, at the same time interval between doses.

Recommended dosage:

  • infections of the upper and lower respiratory tract: 100 mg (1/2 tablet) 2 times a day (morning and evening), the duration of the course of treatment is 5-10 days;
  • acute community-acquired pneumonia: 200 mg 2 times a day for 14 days;
  • exacerbation of chronic bronchitis, acute maxillary sinusitis: 200 mg 2 times a day, course of treatment - 10 days;
  • infections of soft tissues and skin: 400 mg 2 times a day for 7-14 days;
  • cystitis: 100 mg 2 times a day, course duration - 7 days;
  • uncomplicated gonorrhea, uncomplicated infections of the anorectal region in women caused by Neisseria gonorrhoeae: 200 mg once.

In case of impaired renal function with CC less than 30 ml / min, a single dose for all indications should be reduced by 2 times.

Side effects

  • on the part of the immune system: the development of allergic reactions in the form of itching, skin rash, urticaria, eosinophilia, fever, angioedema, anaphylactic shock;
  • on the part of the cardiovascular system: lowering blood pressure;
  • from the nervous system: irritability, headache, fatigue, dizziness, nightmares in dreams, insomnia;
  • from the hematopoietic organs: leukopenia, leukocytosis, lymphocytosis, lymphocytopenia, epistaxis, neutropenia, thrombocytosis, thrombocytopenia, basophilia, agranulocytosis, monocytosis;
  • from the digestive system: decreased appetite, abdominal pain, nausea, vomiting, salivation, persistent diarrhea, gastritis, dysbiosis (development of Clostridium difficile), stool retention, flatulence, pseudomembranous colitis;
  • from the genitourinary system: violation of the menstrual cycle;
  • from the respiratory system: cough;
  • laboratory parameters: hyperbilirubinemia, increased activity of alkaline phosphatase and hepatic transaminases, increased levels of lactate dehydrogenase (LDH), gamma-glutamyl transferase (GGT), creatinine, urea, prothrombin time, decreased hemoglobin (Hb), hypoproteinemia or hyperglycemia, hypoalukemia, hypoalukemia Coombs reaction;
  • others: weakness, increased sweating, chest pain.

Overdose

Symptoms: epigastric discomfort, nausea, vomiting, diarrhea.

Treatment: the appointment of hemodialysis or peritoneal dialysis, especially for patients with impaired renal function.

Application during pregnancy and lactation

The use of an antibiotic during breastfeeding is contraindicated.

Pediatric use

With impaired renal function

Drug interactions

With the simultaneous use of Sefpoteka:

  • bactericidal antibiotics: cause synergy;
  • tetracyclines, macrolides, chloramphenicol (bacteriostatic antibiotics): enhance the nephrotoxicity of aminoglycosides, decrease the therapeutic effect of cefpodoxime;
  • non-steroidal anti-inflammatory drugs: help to slow down the excretion of cephalosporins by the kidneys, increase the risk of bleeding;
  • diuretics, polymyxin B, aminoglycosides, ethacrynic acid: cause an increase in the concentration of cephalosporins in the blood serum, block their secretion, lengthen T 1/2, increase nephrotoxicity;
  • indirect anticoagulants: their effect increases as a result of suppression of intestinal microflora by cefpodoxime and a decrease in the prothrombin index;
  • blockers of H 2 -histamine receptors: by changing the acidity in the stomach, they reduce the bioavailability of the drug;
  • aluminum hydroxide, sodium bicarbonate: high doses of these and other antacids reduce the absorption of the drug.
