Ipigrix is a cholinesterase inhibitor.

Release form and composition

  • tablets: flat-cylindrical, round, with a chamfer, almost white or white (25 pcs. in blisters, in a cardboard box, 2 packages);
  • solution for intramuscular (i / m) and subcutaneous (s / c) administration: colorless transparent liquid (1 ml each in colorless glass ampoules, with a line or break point and two marking rings: at a dosage of 5 mg / ml - yellow and red, at a dosage of 15 mg / ml - green and red; 5 ampoules in cell packs / pallets, in a pack of cardboard 2 packs / pallets).

Composition for 1 tablet:

  • active substance: ipidacrine hydrochloride monohydrate 21.6 mg (equivalent to 20 mg ipidacrine hydrochloride);
  • auxiliary ingredients: potato starch, dry potato starch, lactose monohydrate, calcium stearate.

Composition for 1 ml of solution:

  • active substance: ipidacrine hydrochloride monohydrate 5.4 or 16.2 mg (equivalent to 5 or 15 mg ipidacrine hydrochloride);
  • auxiliary ingredients: 1 M hydrochloric acid solution to pH 2.8-4.0, water for injection.

Pharmacological properties

Pharmacodynamics

Ipidacrine reversibly inhibits cholinesterase by directly stimulating the conduction of impulses along nerve fibers, interneuronal and neuromuscular synapses of the central and peripheral nervous system.

The action of ipidacrine is based on the blockade of potassium channels in the membrane of neurons and muscle cells, which hinders the release of potassium, and reversible inhibition of cholinesterase in synapses. It increases the effect on smooth muscle of acetylcholine, as well as adrenaline, serotonin, oxytocin and histamine.

Pharmacodynamic effects of ipidacrine:

  • regeneration and stimulation of neuromuscular transmission;
  • restoration of nerve impulse conduction after blockade in the peripheral nervous system caused by trauma, inflammation, the action of local antibiotics, anesthetics, toxins, potassium chloride;
  • improved memory;
  • increased tone of smooth muscles of internal organs;
  • stimulation of the central nervous system (CNS) is specific, moderate, in combination with individual manifestations of sedation.

In the course of the preclinical study, no embryotoxic, teratogenic, mutagenic, immunotoxic, carcinogenic effects of Ipygrix and its effect on the endocrine system were revealed.

Pharmacokinetics

  • absorption: after oral administration, ipidacrine is rapidly absorbed from the gastrointestinal tract, the maximum concentration in blood plasma reaches after 1 hour; after s / c or i / m administration, the maximum plasma concentration is reached within 25–30 minutes, 15–20 minutes after the injection, the therapeutic effect appears. The duration of action of the active substance is 3-5 hours, ~ 40-55% of ipidacrine binds to blood plasma proteins;
  • distribution: ipidacrine quickly enters the tissues, penetrates the blood-brain barrier, the equilibrium plasma contains ~ 2% of the substance;
  • metabolism and excretion: ipidacrine is metabolized in the liver, excreted through the kidneys (renally) unchanged in the urine and through the gastrointestinal tract; the half-life is ~ 40 minutes.

Indications for use

  • diseases of the central nervous system: paresis and bulbar paralysis;
  • motor / cognitive impairment due to organic lesions of the central nervous system in the recovery period;
  • diseases of the peripheral nervous system: polyradiculopathy, mono- and polyneuropathy, myasthenia gravis, myasthenic syndrome of various etiologies;
  • intestinal atony (for tablets).

Contraindications

Absolute:

  • extrapyramidal disorders with hyperkinesis;
  • bronchial asthma;
  • epilepsy;
  • angina pectoris;
  • severe bradycardia;
  • vestibular disorders;
  • exacerbation of stomach and / or duodenal ulcers;
  • ileus (mechanical intestinal obstruction);
  • obstruction of the urinary tract;
  • the period of pregnancy and lactation (breastfeeding);
  • age up to 18 years;
  • increased individual sensitivity to ipidacrine and any of the auxiliary components of the drug.

Relative (use with caution):

  • gastric ulcer and 12 duodenal ulcer;
  • thyrotoxicosis;
  • diseases of the cardiovascular system;
  • acute respiratory diseases;
  • a history of obstructive respiratory diseases;
  • additionally for tablets - lactase deficiency, lactose intolerance, lactose / isomaltose malabsorption syndrome (the tablets contain lactose).

Instructions for the use of Ipigrix: method and dosage

The dose and duration of the course of therapy is determined by the attending physician individually, depending on the severity of the disease.

Elderly patients and patients with impaired liver / kidney function do not require dose adjustment.

Pills

Ipigrix tablets are taken orally.

Recommended dosage:

  • diseases of the peripheral nervous system: 1 pc. (20 mg) 1-3 times a day. The duration of the course is 30-60 days, if necessary, the treatment can be repeated several times with breaks between courses of 30-60 days;
  • myasthenic crises, severe disorders of neuromuscular conduction (to prevent): after short-term parenteral administration of 1–2 ml (15–30 mg) of a 15 mg / ml solution, continue therapy with Ipigrix tablets at a dose of 1–2 pcs. (20-40 mg) 5 times a day;
  • diseases of the central nervous system: 1 pc. (20 mg) 2-3 times a day. The duration of the course is 60-180 days, if necessary, the treatment can be repeated;
  • intestinal atony: 1 pc. (20 mg) 2-3 times a day. The duration of the course is 7-14 days.

The maximum daily dose is 10 tablets (200 mg).

If the next dose is missed, the drug should be taken immediately, but a double dose should not be taken to replace the missed one. Further treatment is carried out as directed.

Solution

In this dosage form, the drug is intended for intramuscular or subcutaneous administration.

Recommended dosage:

  • diseases of the peripheral nervous system: 5-15 mg s / c and i / m 1-2 times a day. Duration of the course - 10-15 days (in severe cases - up to 30 days); in the future they switch to the oral form of the drug;
  • myasthenia gravis, myasthenic syndrome: 5–30 mg s / c and i / m 1–3 times a day, then switch to the oral form of the drug. The duration of the general course is 30-60 days, if necessary, the treatment can be repeated several times with breaks between courses of 30-60 days;
  • diseases of the central nervous system (paresis and bulbar paralysis): 5-15 mg s / c and i / m 1-2 times a day. Duration of the course - 10-15 days; later they switch to the oral form of the drug;
  • organic lesions of the central nervous system (rehabilitation): 10–15 mg / m 1–2 times a day. Course duration up to 15 days; later they switch to the oral form of the drug.

Side effects

Ipidacrine is usually well tolerated, side effects are mainly associated with stimulation of m-cholinergic receptors and are observed in ≤ 10% of patients.

Classification of adverse reactions by groups of systems and organs (frequency scale: often - 0.01-0.1; infrequently - 0.001-0.01; rarely - 0.0001-0.001):

  • Gastrointestinal tract (gastrointestinal tract): often - nausea, hypersalivation; infrequently - vomiting; rarely - epigastric pain, diarrhea; frequency unknown - dyspepsia;
  • nervous system: infrequently - headache, dizziness, drowsiness, weakness, muscle spasm;
  • cardiac function: often - bradycardia, palpitations;
  • organ of vision: frequency unknown - miosis;
  • respiratory system, chest and mediastinal organs: infrequently - bronchospasm, increased bronchial secretion;
  • musculoskeletal system and connective tissue: frequency unknown - convulsions, tremors, increased uterine tone;
  • skin and subcutaneous tissues: often - hyperhidrosis; infrequently - allergic reactions (rash, itching), usually observed in the case of high doses of the drug;
  • general disorders, reactions at the injection site (for solution): the frequency is unknown - chest pains, hypothermia, jaundice.

The appearance of the listed adverse reactions, as well as other side effects, requires a visit to a doctor.

Overdose

An overdose symptom (in severe cases) is a cholinergic crisis, manifested by decreased appetite, bronchospasm, lacrimation, increased sweating, pupillary constriction, nystagmus, increased gastrointestinal motility, spontaneous defecation and urination, vomiting, intravenous jaundice, irregular heartbeat, arterial hypotension, anxiety, anxiety, agitation, fear, ataxia, convulsions, coma, speech impairment, drowsiness, general weakness.

It requires symptomatic therapy, the use of m-anticholinergic substances (including atropine, metacin, cyclodol).

special instructions

During therapy, it is necessary to exclude the use of alcohol, since ethanol enhances the side effects of ipidacrine, which, in turn, can increase the negative effects of alcohol.

Ipidacrine can exacerbate the course of epilepsy, and in patients with depression, aggravate the manifestation of its symptoms.

Since taking the drug increases the risk of bradycardia, cardiac activity should be monitored.

Influence on the ability to drive vehicles and complex mechanisms

When using ipidacrine, a sedative effect is possible. Patients with a tendency to such reactions should be careful during therapy, driving vehicles and performing other types of work that require an increased concentration of attention and quick reactions.

Application during pregnancy and lactation

According to the instructions, Ipigrix is contraindicated for pregnant women, since ipidacrine increases the tone of the uterus, which can be the cause of premature labor.

The use of ipidacrine is also contraindicated during breastfeeding.

Pediatric use

Ipidacrine is contraindicated for use in children and adolescents under the age of 18 years, since there is not enough systematic reliable data on the safety and effectiveness of its use in this age group.

Drug interactions

  • local anesthetics, aminoglycosides, potassium chloride: ipidacrine weakens their inhibitory effect on neuromuscular transmission, improving the conduction of excitation along peripheral nerve fibers;
  • drugs that depress the central nervous system, including ethanol, as well as other inhibitors of cholinesterase and m-cholinomimetic drugs: their sedative effect under the influence of ipidacrine increases;
  • other cholinergic drugs: in patients with myasthenia gravis, the risk of a cholinergic crisis is increased;
  • β-adrenolytics: increase the severity of bradycardia that ipidacrine can cause;
  • Cerebrolysin: potentiates the effectiveness of ipidacrine;
  • ethanol: enhances the negative side reactions of ipidacrine.

Ipigrix solution can be used in combination with nootropic drugs.
