Ibuprofen-Akrikhin is a drug with antipyretic, anti-inflammatory and analgesic effects, intended for use in children.

Release form and composition

Ibuprofen-Akrikhin dosage form:

  • suspension for oral administration (orange): orange in color, with an orange smell, during storage, separation into a sediment and a liquid layer is permissible, after mixing they form a homogeneous suspension (in a cardboard box 1 glass or plastic bottle of 100 g each complete with a dispenser - a spoon or syringe);
  • rectal suppositories (for children): almost white or white, torpedo-shaped (in a cardboard box, 1 or 2 blisters containing 5 suppositories).

Each pack also contains instructions for the use of Ibuprofen-Akrihin.

Composition of 100 ml suspension:

  • active substance: ibuprofen - 2 g;

Pharmacological properties

Pharmacodynamics

Ibuprofen-Akrikhin is an NSAID (non-steroidal anti-inflammatory drug) that has antipyretic, anti-inflammatory and analgesic effects.

The antipyretic effect of ibuprofen is associated with blocking COX-1 and -2 (cyclooxygenase-1 and -2) in the arachidonic acid cascade of the central nervous system. This helps to reduce the biosynthesis of prostaglandins and their concentration in the cerebrospinal fluid, as well as to reduce the excitation of the center of thermoregulation. In case of fever, the effect of lowering body temperature begins to appear 30 minutes after taking Ibuprofen-Akrikhin, its maximum effect develops within three hours.

The leading mechanism of the analgesic effect is a decrease in the production of biogenic amines, prostaglandins of classes E, F and I. This prevents the development of hyperalgesia at the level of changes in the sensitivity of nociceptors. The analgesic effect is most pronounced in patients with inflammatory pains.

The development of anti-inflammatory action is associated with inhibition of COX activity and a decrease in the biosynthesis of prostaglandins in the foci of inflammation, which leads to a decrease in the secretion of inflammatory mediators, a decrease in the activity of the exudative and proliferative phases of the inflammatory process.

Ibuprofen, like other NSAIDs, exhibits antiplatelet activity.

Pharmacokinetics

After oral administration, more than 80% of ibuprofen is absorbed from the gastrointestinal tract. C max (maximum concentration) in blood plasma is achieved in 45 minutes or 1.5-2.5 hours when taken on an empty stomach or after meals, respectively. The concentration of the substance in the synovial fluid is higher than the plasma concentration, the time to reach C max is from 2 to 3 hours. Due to the absorption of about 60% of the pharmacologically inactive R-form of the substance, it is gradually transformed into the active S-form.

When administered rectally, ibuprofen is well absorbed from the lumen of the rectum, after which it quickly enters the systemic circulation.

Ibuprofen binds to plasma proteins (mainly albumin) at a level of 90%.

It does not accumulate in the body.

Biotransformation of ibuprofen occurs mainly in the liver. The substance undergoes first-pass and post-system metabolism.

Ibuprofen has a biphasic elimination kinetics with a T 1/2 (half-life) of 2 to 2.5 hours. Most of the substance (60–90%) is excreted by the kidneys as metabolites and products of their conjugation with glucuronic acid, to a lesser extent, no more than 1%, is excreted unchanged and with bile. After taking a single dose, ibuprofen is excreted in full within 24 hours.

Indications for use

As an antipyretic agent, Ibuprofen-Akrikhin is prescribed at elevated body temperature of various etiologies, including:

  • acute respiratory viral infections, including influenza;
  • colds;
  • angina (tonsillitis);
  • post-vaccination reactions;
  • childhood infections.

As an anesthetic drug, Ibuprofen-Akrikhin is indicated for low / moderate intensity pain syndrome of various etiologies:

  • toothache, painful teething;
  • headache, migraine;
  • ear pain with inflammation of the middle ear;
  • muscle / joint pain associated with injuries of the musculoskeletal system;
  • neuralgia.

Ibuprofen-Akrikhin does not affect the progression of the disease, it is intended for symptomatic treatment, reducing the severity of pain and inflammation during the period of use.

Rectal suppositories are recommended for children from 3 months to 2 years old if it is impossible to use oral forms of ibuprofen or if the absorption of the substance is impaired (in particular, during vomiting).

Contraindications

Absolute:

  • full / partial combination of bronchial asthma with recurrent polyposis of the nose and paranasal sinuses, intolerance to acetylsalicylic acid or other NSAIDs (currently or in history);
  • erosive and ulcerative diseases of the gastrointestinal tract (including exacerbation of gastric ulcer and duodenal ulcer, ulcerative colitis, peptic ulcer, Crohn's disease);
  • severe renal failure (in patients with creatinine clearance <30 ml / min), progressive renal disease;
  • severe liver failure, active liver disease;
  • confirmed hyperkalemia;
  • bowel disease of inflammatory etiology;
  • blood clotting disorders (including hemophilia, prolonged bleeding time, hemorrhagic diathesis, bleeding tendency);
  • individual intolerance to the components of Ibuprofen-Akrikhin or other NSAIDs (including acetylsalicylic acid).

Absolute contraindications additionally for suspension:

  • deficiency of sucrase / isomaltase, fructose intolerance, glucose-galactose malabsorption;
  • lactation period;
  • children's age up to 3 months.

Absolute contraindications additionally for suppositories:

  • burdened history of bleeding / perforation of gastrointestinal ulcers provoked by the use of NSAIDs;
  • decompensated heart failure, the period after coronary artery bypass grafting;
  • intracranial hemorrhage;
  • proctitis;
  • III trimester of pregnancy;
  • body weight up to 6 kg.

Relative (Ibuprofen-Akrikhin is prescribed under medical supervision):

  • hepatic / renal failure;
  • cirrhosis of the liver with portal hypertension;
  • arterial hypertension;
  • chronic heart failure;
  • hyperbilirubinemia;
  • nephrotic syndrome;

  • presence of Helicobacter pylori infection;
  • diseases of the gastrointestinal tract (gastritis, colitis, enteritis);

  • severe somatic diseases;

Ibuprofen-Akrikhin, instructions for use: method and dosage

Suspension for oral administration

Ibuprofen-Akrikhin suspension is taken orally, after meals. For accurate dosing of the suspension, it is recommended to use the dispenser supplied with the vial (spoon or syringe). The bottle must be shaken before taking until a homogeneous suspension is obtained.

The dose is determined by age and weight. A single dose is from 5 to 10 mg / kg, the frequency of administration is 3-4 times a day. The maximum dose is 20-30 mg / kg per day.

Recommended dosing regimen of Ibuprofen-Akrikhin:

  • 6-12 months (7.7-9 kg) - 50 mg 3 times a day;
  • 1-3 years (10-15 kg) - 50 mg 3-4 times a day;
  • 4-6 years (16-20 kg) - 100 mg 3 times a day;
  • 7-9 years (21-29 kg) - 100 mg 4 times a day;
  • 10-12 years (30-41kg) - 200 mg 3 times a day.

The interval between doses of the drug should be 6-8 hours.

For infants of 3-6 months with a body weight of 5-7.6 kg with post-vaccination reactions, Ibuprofen-Akrikhin is prescribed 2.5 ml of suspension, after 6 hours the same single dose can be repeated. Maximum 5 ml per day.

Ibuprofen-Akrikhin should be used for no longer than 3 or 5 days as an antipyretic or pain reliever, respectively. If symptoms persist, you should consult your doctor.

Rectal suppositories

The maximum daily dose when used as an anesthetic and antipyretic agent should not exceed 30 mg / kg weight, the intervals between the use of single doses - 6-8 hours.

A single dose is 1 suppository. Multiplicity of application:

  • from 3 to 9 months (6–8 kg): up to 3 times a day (maximum 180 mg per day);
  • from 9 months to 2 years (8–12 kg): up to 4 times a day (maximum - 240 mg per day).

The duration of therapy is no longer than 3 days.

If symptoms persist / worsen in children aged 3-5 months for 24 hours or in children from 6 months for three days, you should consult your doctor.

With post-immunization fever, children under 1 year of age are prescribed 1 suppository, according to indications, after 6 hours, Ibuprofen-Akrikhin may be reapplied (maximum 120 mg per day).

Side effects

  • digestive system: hepatitis, NSAID-gastropathy (manifested in the form of nausea, vomiting, loss of appetite, heartburn, abdominal pain, diarrhea, flatulence, pain / discomfort in the epigastric region), ulceration of the mucous membrane of the gums and the mucous membrane of the gastrointestinal tract (possibly complication of bleeding and perforation), constipation, xerostomia, pancreatitis, aphthous stomatitis;
  • respiratory system: bronchospasm, shortness of breath;
  • nervous system: anxiety, confusion, sleep disturbance, drowsiness, depression, irritability, psychomotor agitation, headache, dizziness, hallucinations; rarely - aseptic meningitis (in most cases against the background of autoimmune diseases);
  • sensory organs: amblyopia, scotoma, hearing loss, tinnitus / ringing in the ears, reversible toxic optic neuritis, eye irritation / dryness, eyelid and conjunctival edema (allergic etiology), diplopia / blurred vision, color vision disturbance;
  • hematopoietic organs: anemia (including aplastic, hemolytic), thrombocytopenic purpura, thrombocytopenia, leukopenia, agranulocytosis;
  • heart and blood vessels: heart failure (development or aggravation), tachycardia, increased blood pressure;
  • urinary system: nephrotic syndrome (edema), cystitis, polyuria, allergic nephritis, acute renal failure;
  • allergic reactions: fever, pruritus, skin rash (urticaria, erythematous), anaphylactoid reactions, angioedema, toxic epidermal necrolysis, allergic rhinitis, eosinophilia, anaphylactic shock, bronchospasm, exudative erythema multiforme;
  • laboratory parameters: an increase in hematocrit, hemoglobin, bleeding time, plasma creatinine concentration, hepatic transaminase activity; decrease in creatinine clearance, plasma glucose concentration;
  • others: increased sweating, edema (including peripheral).

The frequency and severity of adverse reactions may vary depending on the dosage form of Ibuprofen-Akrikhin.

The likelihood of these disorders can be minimized by conducting short-term therapy using the lowest effective dose.

Adverse reactions are predominantly dose-dependent.

In the treatment of chronic conditions and in the case of prolonged treatment, other adverse reactions may occur.

Overdose

Serious complications associated with the toxic effects of Ibuprofen-Akrikhin usually appear after taking a dose above 200 or 400 mg / kg (for rectal and oral administration, respectively). In case of overdose, you should immediately consult a doctor.

The main symptoms: abdominal pain, metabolic acidosis, nausea, vomiting, headache, lethargy, tinnitus, depression, drowsiness, coma, hemorrhagic diathesis, decreased blood pressure, convulsions, respiratory arrest, acute renal failure, hepatic dysfunction, bradycardia, tachycardia, atrial fibrillation.

Children under 5 years of age are particularly prone to sleep apnea, seizures and coma.

For therapy with oral administration of high doses, gastric lavage is prescribed (only for 60 minutes after ingestion), alkaline drink, activated carbon.

In case of an overdose of Ibuprofen-Akrikhin in any dosage form, symptomatic treatment is indicated, including correction of the acid-base state, blood pressure, airway patency, monitoring of the electrocardiogram and vital signs. Prolonged / frequent seizures are treated with intravenous diazepam or lorazepam. When the course of bronchial asthma worsens, bronchodilators are prescribed.

special instructions

Ibuprofen in patients with bronchial asthma or other diseases that occur with bronchospasm may increase the risk of bronchospasm. When prescribing Ibuprofen-Akrikhin, such patients need to be very careful, in case of difficulty breathing, you should immediately consult a doctor.

During long-term therapy, the peripheral blood picture and the functional state of the kidneys and liver should be monitored.

In order to prevent the appearance of NSAID gastropathy, it is recommended to combine Ibuprofen-Akrikhin with prostaglandin E preparations (misoprostol). If symptoms of the disease occur, careful monitoring of the condition is shown, including carrying out esophagogastroduodenoscopy, analysis of feces for occult blood, blood test with determination of hematocrit, hemoglobin.

If it is necessary to determine the 17-ketosteroids, Ibuprofen-Akrikhin must be canceled 48 hours before the study.

During the period of therapy, the use of alcoholic beverages and the intake of drugs containing ethanol are not recommended.

1 ml of Ibuprofen-Akrikhin suspension contains about 0.34 g of sucrose, which corresponds to about 0.03 XE (bread units).

Influence on the ability to drive vehicles and complex mechanisms

If necessary, drive vehicles during the period of therapy should take into account the possibility of developing significant disorders of the nervous system and sensory organs, such as lethargy, drowsiness or visual impairment.

Application during pregnancy and lactation

Suspension:

  • pregnancy: the use of Ibuprofen-Akrikhin is possible only in cases where the expected benefit is higher than the possible harm;
  • lactation period: therapy is contraindicated.

Suppositories:

  • III trimester of pregnancy: therapy is contraindicated;
  • I – II trimester of pregnancy, lactation period: Ibuprofen-Akrikhin can be used only with caution in cases where the expected benefit is higher than the possible harm.

Pediatric use

Suspension:

  • up to 3 months: therapy is contraindicated;
  • 3–6 months: the use of Ibuprofen-Akrikhin is possible only after a medical consultation.

Ibuprofen-Akrikhin suppositories are intended for use in children aged 3 months to 2 years.

With impaired renal function

  • severe renal failure, progressive kidney disease: therapy is contraindicated;
  • renal failure: Ibuprofen-Akrikhin should be used under medical supervision.

For violations of liver function

  • severe liver failure, active liver disease: therapy is contraindicated;
  • hepatic impairment: Ibuprofen-Akrikhin should be used under medical supervision.

Use in the elderly

Ibuprofen-Akrikhin in the form of suppositories in elderly patients should be used with caution.

Drug interactions

  • vasodilators (including angiotensin-converting enzyme inhibitors): their hypotensive activity decreases;
  • diuretics: the combination is not recommended, a weakening of the diuretic effect is possible;
  • oral hypoglycemic agents (especially sulfonylurea derivatives), insulin: their effect is likely to increase;
  • cholestyramine, antacids: absorption of ibuprofen is reduced;
  • other NSAIDs: the combination is not recommended; in particular, when combined with acetylsalicylic acid, the anti-inflammatory effect decreases, side effects increase;
  • inducers of microsomal oxidation (phenylbutazone, rifampicin, phenytoin, barbiturates, ethanol, tricyclic antidepressants): the production of hydroxylated active metabolites increases, the risk of severe hepatotoxic reactions increases;
  • drugs with myelotoxic effect: manifestations of ibuprofen hematotoxicity increase;
  • valproic acid, cefamandol, cefotetan, cefoperazone, plikamycin: the incidence of hypoprothrombinemia increases;
  • cyclosporine: the plasma concentration of ibuprofen increases, while the likelihood of its hepatotoxic effects increases;
  • gold preparations, cyclosporins: the effect of ibuprofen on the biosynthesis of prostaglandins in the kidneys increases, which is manifested by an increase in nephrotoxicity;
  • indirect anticoagulants, antiplatelet agents, fibrinolytics: their effect increases, while the likelihood of developing hemorrhagic complications increases;
  • drugs that block tubular secretion: the plasma concentration of ibuprofen increases, excretion decreases;
  • caffeine: the analgesic effect of ibuprofen is enhanced;
  • digoxin, methotrexate, lithium preparations: their concentration in the blood increases.
