Release form and composition
The dosage form of Hepazolone is rectal suppositories: torpedo-shaped, white or white with a creamy or yellowish tint (5 pcs. In blisters, 2 packs in a cardboard box).
Active ingredients in 1 suppository:
- sodium heparin - 120 ME (international units);
- lidocaine hydrochloride - 10 mg;
- prednisolone - 1.7 mg.
Auxiliary component: witepsol N 15 - in sufficient quantity to obtain a suppository weighing 1000 mg.
Pharmacological properties
Pharmacodynamics
The pharmacological action of Hepazolone determines the properties of its active substances.
Heparin helps prevent the formation of blood clots, blood coagulation in hemorrhoids, as well as the regeneration of connective tissue, accelerating healing and reducing the number of relapses of the disease. It has a decongestant and anti-inflammatory effect, affecting the vascular endothelium.
When used in this dosage in combination with antithrombin III of blood plasma, heparin inhibits thrombosis (due to deactivation of factor Xa), and also inhibits the conversion of prothrombin to thrombin.
Prednisolone belongs to the group of synthetic glucocorticosteroids of medium duration. It has anti-inflammatory, anti-allergic and anti-exudative effects, reduces the inflammatory process and associated subjective sensations in the anorectal region, manifested in the form of itching, burning, pain, feeling of moisture associated with eczematous changes.
The main effects of prednisone:
- reduction in the formation, release and activity of inflammatory mediators - lysosomal enzymes, kinin, histamine, prostaglandin;
- suppression of cell migration to the site of inflammation;
- decrease in vasodilation and increased vascular permeability in the focus of inflammation;
- preventing access of sensitized T-lymphocytes and macrophages to target cells;
- reduction of exudation (due to vasoconstrictor action);
- suppression of the action of macrophages, cytokines, target cells that are involved in the occurrence of allergic reactions.
Lidocaine has a local anesthetic effect due to the blockade of voltage-dependent sodium channels. Due to this, the generation of impulses in the endings of the sensory nerves and the conduction of pain impulses along the nerve fibers are disrupted.
The therapeutic effect of Hepazolone is due to the complex effects of three components.
Pharmacokinetics
Prednisolone and heparin are effectively absorbed after rectal administration. After penetration into the intestinal wall, after a few hours, they are gradually transported to the liver through portal circulation. In the liver, biotransformation by oxidizing enzymes into metabolites occurs. Excreted through the kidneys. T 1/2 (half-life) of heparin and prednisolone is 1-1.5 and 18-36 hours, respectively.
About 35% of lidocaine administered rectally undergoes systemic absorption. The time to reach the maximum concentration of the substance in the blood is 2 hours. It is rapidly metabolized in the liver. The excretion of lidocaine occurs through the kidneys, part of the lidocaine after enterohepatic circulation is excreted in the feces. T 1/2 is in the range of 1-3 hours.
Indications for use
- internal and external hemorrhoids, including acute;
- thrombophlebitis of hemorrhoidal veins of the perianal region;
- eczema;
- fistulas;
- anal fissures;
- itching in the anus associated with inflammation;
- thrombosed and operated hemorrhoids (simultaneously with other drugs);
- the period of preparation for surgery in the anorectal zone.
Contraindications
- tuberculous, fungal, viral and other infectious processes of the anorectal region, disorders of the coagulation system in the presence of a predisposition to bleeding;
- age up to 12 years;
- pregnancy and the period of breastfeeding;
- individual intolerance to the components of Hepazolone.
Instructions for the use of Hepazolone: method and dosage
Suppositories are used rectally.
For internal hemorrhoids, Hepazolone suppositories are used 1 or 2 times a day.
For external hemorrhoids, the suppository should be inserted so that its base can be felt from the outside. For 2-3 minutes (until the suppository completely melts) it must be held with a gauze napkin so that it does not "fall" into the rectum without exerting a therapeutic effect.
The duration of the course is 7 days. After consulting a doctor, it is possible to extend the use of the drug for the same period.
Side effects
Usually, Hepazolone suppositories are well tolerated. Subject to the recommended dosing regimen, the occurrence of adverse reactions was not noted.
In cases of a long course (longer than 5 weeks), the development of systemic side effects associated with the action of prednisolone may occur:
- metabolism: hypocalcemia, increased excretion of calcium ions, increased sweating;
- nervous system: dizziness, insomnia;
- cardiovascular system: arrhythmias, bradycardia (if there is a predisposition), increased blood pressure, changes in the ECG, which are characteristic of hypokalemia;
- endocrine system: decreased glucose tolerance, steroid diabetes mellitus, inhibition of adrenal function, Itsenko-Cushing's syndrome.
It is also possible to develop allergic reactions (itching and irritation of the anus), which is associated with the action of lidocaine and heparin.
