Aktiferrin is a drug for replenishing iron deficiency.

Release form and composition

The drug is available in the form of capsules, syrup and drops.

The active ingredients in the capsules are ferrous sulfate monohydrate and serine. The auxiliary components are: yellow beeswax, lecithin, soybean oil, rapeseed oil.

The active substances of Aktiferrin in the form of a syrup are iron sulfate heptahydrate and serine, and the auxiliary ones are ascorbic acid, ethanol, invert sugar syrup, raspberry and cream flavors, purified water.

In addition to these auxiliary components, Aktiferrin drops also include potassium sorbate.

Pharmacological properties

Pharmacodynamics

Iron is a trace element that is a coenzyme of cytochrome oxidase, peroxidase, catalase and a component of hemoglobin, myoglobin, cytochromes and is involved in various metabolic processes in the body.

The daily requirement for iron for adults is 1–2 mg, for pregnant women - 2–5 mg, for children under the age of 7 years - 0.5–1.5 mg. The level of absorption of iron is on average 10%, therefore, when taken orally, to replenish the need for iron, it is necessary to prescribe doses that exceed the daily requirement by 10 times.

The total iron content in the body of adults is 3000–5000 mg, in newborns - 200–

300 mg. Approximately 60% of this amount is involved in the transport of oxygen in the composition of hemoglobin in erythrocytes. Approximately 1000 mg in the form of ferritin or hemosiderin (mobilizable form) is contained in the cells of macrophages of the spleen, liver, and bone marrow. Plasma transport of iron - about 4 mg. In the bone marrow, 25 mg of iron is transferred daily, which is formed as a result of hematopoiesis (mostly due to the destruction of red blood cells).

Pharmacokinetics

As a result of oral administration, 10-15% of ferrous iron is absorbed in the duodenum and jejunum. Also, iron absorption is possible through the mechanism of passive diffusion (this process is largely activated with increased erythropoiesis or with iron deficiency). With a low hemoglobin level and depletion of the iron depot, the degree of absorption can increase up to 60%. When these indicators are normalized, this value decreases to the normal level. The peak plasma concentration after taking iron supplements occurs after 2-4 hours.

When iron enters the bloodstream, it binds to plasma proteins by transferrins and is transported to specific depots and to the sites of hematopoiesis.

After binding with apoferritin, iron in the form of ferritin is deposited in the spleen, liver, and bone marrow. Iron passes through the placental barrier and is excreted in breast milk in minimal amounts.

Daily loss of iron through desquamation of mucosal and epithelial cells with urine and bile is approximately 1 mg. Iron loss during menstruation is about 1 mg.

A significant amount of iron released as a result of the destruction of hemoglobin is reutilized in the body (20–30 mg per day) and is used mainly for the synthesis of hemoglobin.

Indications for use

According to the instructions, Aktiferrin is prescribed for iron deficiency anemia and iron deficiency caused by:

  • Great blood loss;
  • The period of lactation and pregnancy;
  • Chronic gastritis;
  • Gastrointestinal ulcer;
  • Tumor diseases;
  • Gastric resection;

Contraindications

According to the instructions, Aktiferrin is not prescribed in the following cases:

Instructions for the use of Aktiferrin: method and dosage

Adults and adolescents take the drug in capsules. Aktiferrin is prescribed to them 1 capsule 2-3 times a day. If there is poor tolerance, the dosage is gradually reduced, choosing the optimal one, but the duration of treatment is increased. Children under 6 years of age are prescribed 1 capsule of the drug 1 time per day.

Syrup Aktiferrin is prescribed for children over 2 years old. The permissible daily dosage is 5 ml per 12 kg of the child's weight. The drug in the form of a syrup for preschool children is prescribed 5 ml 1-2 times a day, for school-age children - 5 ml 2-3 times a day.

For newborns and children under 2 years of age, it is preferable to give the drug in drops. The daily dosage is 5 drops per 1 kg of the child's weight. Frequency rate of admission - 2-3 times a day. On average, infants are prescribed 10-15 drops, preschool - 25-35 drops, school - 50 drops 3 times a day.

You can take Aktiferrin during or after a meal with water. Do not chew the capsules.

Side effects

Symptoms

In the case of the use of iron preparations by children, there is a high probability of intoxication, therefore, such medicines should be kept out of the reach. The occurrence of life-threatening conditions is possible with a dose of 1000 mg of ferrous sulfate. At the same time, the following symptoms were recorded: diarrhea, nausea, vomiting, severe abdominal pain, and in severe cases, the development of collapse, drowsiness, oliguria and shock.

Due to toxic hepatitis, there is a risk of developing jaundice, in some cases - disorders of the central nervous system (paralysis, convulsions, coma). Rarely there are blood clotting disorders.

Treatment

Before specific therapy, measures are shown to remove the iron preparation from the gastrointestinal tract (gastric lavage with phosphate and bicarbonate buffer solutions, induction of vomiting), followed by ingestion of milk and raw eggs.

Specific therapy

If the concentration of serum iron is exceeded (> 300-350 mcg / dl), an intravenous drip of 1000-2000 mg deferoxamine (Desferal) is required at the rate of 15 mg / kg of the patient's body weight per hour.

In the case of a course of treatment with iron preparations, the level of serum iron and other indicators of blood serum should be systematically monitored.

In case of an overdose of iron salts in patients with oligo- or anuria, hemodialysis is prescribed.

Special measures in case of severe intoxication - parenteral administration of Dimercaprol (diethylenetriaminepentaacetate calcium).

Due to the risk of neurotoxic complexes formation, diethylenetriaminepentaacetate calcium should be used with caution.
