Release form and composition
Dosage form of Phlogenzym - enteric coated tablets: round biconvex, with a smooth greenish-yellow surface, slight marbling or dotted blotches in the color of the shell is allowed, with a specific odor (20 pcs. In a blister, in a cardboard box 2, 5 or 10 blisters; 800 pcs. in a polyethylene bottle).
1 tablet contains:
- active ingredients: bromelain - 450 units. FIP (Federation International Pharmaceutical units), rutoside (rutin) - 100 mg, trypsin - 1440 UNITS. FIP;
- auxiliary components: corn starch, stearic acid, lactose hydrate, colloidal anhydrous silicon dioxide, magnesium stearate, talc, purified water;
- shell composition: triethyl citrate, methacrylic acid and methyl methacrylate copolymer (1: 1), macrogol 6000, vanillin, talc.
Pharmacological properties
Phlogenzyme is a combination of proteolytic enzymes (enzymes) of plant and animal origin (bromelain and trypsin) and are highly active. It also includes rutin. The combination of enzymes has pleiotropic (multiple) effects, influencing in various ways biochemical and pathophysiological processes. The therapeutic effect of the active components of the drug is realized through anti-inflammatory, secondary analgesic, immunomodulating, decongestant, thrombolytic, fibrinolytic and antiplatelet actions.
Pharmacodynamics
Phlogenzyme enteric-coated tablets transiently pass the upper gastrointestinal tract without injuring the inner lining of the stomach. Then, the active components of the drug are absorbed in the small intestine through the resorption of intact molecules (pinocytosis, endocytosis). The drug proteases bind to transport proteins in the blood (alpha-1-antitrypsin and alpha-2-macroglobulin) to form a reversible protease-antiprotease complex. In the latter, the antigenic determinants of exogenous proteases of Phlogenzyme are masked, which avoids allergic reactions. Antiprotease (alpha-2-macroglobulin) forms a complex with enzymes, as a result of which it turns into an active form that performs the function of an extracellular regulator of growth factors and pro-inflammatory cytokines through their transfer,clearance and elimination.
Formation of an active protease-antiprotease complex ensures safe movement of proteolytic enzymes along the vascular bed to the inflammation focus and the site of injury without being tied to the site of localization in the body. The complex retains and inhibits the excretion of proteolytic enzymes of Phlogenzyme from the body, prolongs the time of their circulation in the vascular bed and, accordingly, enhances the therapeutic effect. After penetrating wounds and foci of inflammation, proteolytic enzymes break down (hydrolyze) damaged tissues and proteins, eliminating cellular debris (detritus) and providing accelerated cleansing and wound healing.
The drug has a positive effect on the course of the inflammatory process and promotes modulation of the body's defense reactions, guaranteeing the physiological course of inflammation at various stages of the process. Phlogenzyme proteolytic enzymes activate the breakdown of inflammatory mediators, stimulate interferonogenesis, break down membrane deposits and immune complexes, and enhance the activity of natural killer cells and phagocytes. Proteases of the drug reduce the content of anti-inflammatory cytokines INF-γ, TNF-α, IL-1β, IL-8, IL-6 and increase the production of anti-inflammatory cytokines IL-10, IL-4, etc., as well as normalize the content of blood antibodies and immunoglobulins … Enzymes of the drug prevent the occurrence of pathological manifestations, which sometimes accompany immunocomplex and autoimmune processes,restore the body's immunological reactivity.
Phlogenzyme proteases reduce the concentration of transforming growth factor β, an increased level of which causes excessive scarring. Proteolytic enzymes regulate the expression of angiogenesis factors (fibroblast growth factor, vasculoendothelial growth factor and some other factors), modulation of the wound process, and synchronization of the processes of basement membrane formation. As a result, enzymes improve reparative processes, prevent the onset of adhesive disease that develops after surgical interventions in the abdominal cavity, and prevent the appearance of keloid and hypertrophic scars.
Enzymes of the drug provide destruction and removal of damaged tissues, as well as accelerated resorption of edema and hematomas due to the normalization of trophic processes and microcirculation in the affected area, improvement of the permeability of the walls of blood vessels, enhanced elimination of fibrin and protein detritus deposits, reduction of interstitial infiltration carried out by plasma proteins. Proteolytic enzymes optimize the inflammatory process by direct proteolysis of inflammatory mediators, reducing tissue edema and oncotic pressure, normalizing microcirculation, eliminating ischemia and reducing pressure on nerve endings, which provides a secondary analgesic effect of proteases. Also active components of Phlogenzym prevent the occurrence of purulent complications and trophic disorders and reduce the risk of thromboembolic complications,associated with prolonged immobilization.
The enzymes of the drug accelerate the regeneration of the functions of organs and tissues, support the physiological course of the process of their recovery, reduce inflammation at the site of injury, activate the delivery of nutrients and oxygen to the wound area, and have a positive effect on improving microcirculation.
Phlogenzyme improves the rheological parameters of blood (fluidity and viscosity) due to a decrease in platelet aggregation (adhesion), normalization of the functional state of the vascular wall and blood cells, a decrease in the density of adhesive molecules, improvement of the plasticity (deformability) of erythrocytes, increased fibrinolytic activity of blood serum, stabilization of endothelial permeability.
Proteolytic enzymes reduce the concentration of micro- and macroaggregates and activated forms of platelets (spheroechinocytes), which reduces the risk of blood clots in blood vessels, and at the same time participate in the processes of lysis of the formed blood clots. They also reduce the content of atherogenic lipids, minimizing the risk of developing atherosclerosis and vascular disorders, and providing an increase in high-density lipoprotein levels.
Phlogenzyme improves the blood supply to the bronchi and lung tissues in chronic diseases of the respiratory tract, including those associated with smoking, promotes thinning of sputum, reducing coughing and facilitating breathing, restores the function of drainage in the bronchi, normalizes the function of ciliated epithelium and the viscous properties of bronchial secretions.
Proteolytic enzymes increase the effectiveness of antibiotic therapy, while eliminating its unwanted effects (irritable bowel syndrome, dysbiosis). Proteases restore intestinal endoecology, normalize the microbiota balance, and improve the breakdown of substrates.
Pharmacokinetics
Hydrolase and unabsorbed rutin are excreted through the intestine. Removal of resorbed enzymes is carried out by the cells of the mononuclear phagocytic system.
Indications for use
According to the instructions, Phlogenzyme is indicated for the prevention and use of the following pathologies in the complex therapy:
- surgery: purulent-inflammatory complications after surgery, lowering the risk of thromboembolic complications against the background of prolonged immobilization, postoperative rehabilitation and recovery (improvement of reparative and regenerative processes), prevention of the development of adhesive disease and the formation of a keloid scar;
- cardiology: prevention of angina attacks, coronary heart disease, reducing the risk of developing vascular accidents and repeated heart attacks;
- angiology: chronic angiopathies (including obliterating atherosclerosis of the arteries of the lower extremities), thrombophlebitis of superficial veins, acute deep vein thrombosis, post-thrombotic disease, microcirculation disorders, lymphedema (lymphatic edema);
- pulmonology: chronic obstructive pulmonary disease, pneumonia;
- traumatology: ligament and tendon injuries, bone fractures, sports injuries, hematomas and soft tissue bruises, improved integration of endoprostheses, burns, osteosynthesis;
- rheumatology: ankylosing spondylitis, rheumatoid arthritis, reactive arthritis, soft tissue rheumatism;
- gynecology: acute and chronic forms of inflammatory pathologies of the pelvic organs (salpingo-oophoritis, adnexitis), vascular complications in menopause, a decrease in the severity and frequency of complications of hormone replacement therapy;

Contraindications

Care should be taken to prescribe Phlogenzym to patients on hemodialysis during pregnancy and breastfeeding.
Instructions for the use of Phlogenzym: method and dosage
Phlogenzym tablets are taken orally, swallowed whole and washed down with water (200 ml), no later than 0.5 hours before meals.
After surgery, it is recommended to start taking the drug in 1–2 days.
Recommended dosage:
- treatment: 3 pcs. 3 times a day, course duration - 2 weeks. Then, if necessary, the patient can be transferred to receive the drug in a dose of 2 pcs. 3 times a day for two or more weeks;
- prevention: 2 pcs. 3 times a day for four or more weeks, patients on bed rest - the entire period of immobilization. To reduce the risk of vascular catastrophes (heart attack) and prevent venous thrombosis of the lower extremities, courses of taking the drug should be repeated 3-4 times a year.
With hormone replacement therapy, in order to reduce the risk of developing venous thrombosis of the lower extremities, the drug is prescribed in a dose of 2 pcs. 3 times a day by repeated courses for 2 weeks with an interval of 3 weeks 3-4 times a year.
Side effects
- possible: a change in the smell and consistency of the stool, increased stool frequency (stopped by a temporary decrease in the dose of the drug);
- rarely: the development of allergic reactions (itching, rash);
- others: against the background of violation of the recommended conditions of use - nausea, general weakness, bloating and abdominal pain, a feeling of intestinal fullness, headache, dizziness, exanthema.
Overdose
Cases of overdose of Phlogenzyme are currently unknown.

In the treatment of infectious and inflammatory processes, Phlogenzyme does not replace the use of antibiotics.
