Injectran is a drug that regulates metabolism in cartilage tissue, regenerating, chondrostimulating, analgesic and anti-inflammatory action.

Release form and composition

The drug is produced in the form of a solution for intramuscular (i / m) administration: a clear, colorless or yellowish liquid (1 or 2 ml in a glass ampoule: 5 or 10 ampoules complete with an ampoule scarifier in a cardboard box with a corrugated insert; 5 ampoules in a blister strip with or without aluminum foil, 1 or 2 packs complete with an ampoule scarifier in a cardboard box; if ampoules have a point or a break ring, the ampoule scarifier is not included in the kit; for hospital - 4, 5 or 10 blisters in cardboard box, 50 or 100 - in a cardboard box; each box also contains instructions for use of Injectran).

Pharmacological properties

Pharmacodynamics

Injectran is a tissue repair stimulator. Chondroitin sulfate, the active ingredient of the drug, is the main component of proteoglycans, which, in combination with collagen fibers, make up the cartilage matrix. The active ingredient, with its chondroprotective properties, has the following effect:

• enhances metabolic processes in the tissues of fibrous and hyaline cartilage and subchondral bone;
• stimulates the synthesis of proteoglycans by cells of cartilage tissue (chondrocytes);
• inhibits the activity of enzymes leading to degradation (destruction) of articular cartilage;
• has a beneficial effect on phosphorus-calcium metabolism in the cartilage tissue, accelerates its regeneration;
• participates in the formation of the basic substance of cartilage and bone tissue.

Injectran exhibits anti-inflammatory and analgesic properties, helps to reduce the penetration of pain factors and inflammatory mediators into the synovial (filling the joint cavity) fluid through macrophages and synoviocytes enclosed in the synovial membrane, inhibits the production of leukotrienes (LT) and prostaglandins (PG).

Chondroitin sulfate inhibits the degeneration of connective tissue and lowers calcium loss, stimulates the processes of bone tissue repair, prevents the progression of osteoarthritis and osteochondrosis. The active substance is involved in the regeneration of the cartilaginous surfaces of the joints and the joint capsule, counteracts the development of collapse of the connective tissue, and ensures the normal production of joint fluid.

Thanks to the action of Injectran, the mobility of the joints improves and the intensity of pain decreases. After the completion of the course of treatment, the clinical effect of the drug persists for a long period. In the treatment of degenerative changes in the joints occurring with the development of secondary synovitis, the effect is achieved already 2-3 weeks after the start of the drug. Since the drug is structurally similar to heparin, it is potentially able to block the formation of fibrin thrombi in the synovial and subchondral microcircular bed.

Pharmacokinetics

When administered intramuscularly, the active substance is easily absorbed. 30 minutes after the i / m injection, the agent in significant concentrations is determined in the blood, and after 15 minutes it is detected in the synovial fluid. The maximum concentration (C _max) of the drug in the blood plasma is observed 60 minutes after administration, and then, over 2 days, the level of its content gradually decreases. The substance accumulates mainly in the cartilage tissue, in the articular cartilage C _max is observed after 48 hours. In the process of penetration of the agent into the joint cavity, it passes the synovium without obstacles. The drug is excreted mostly by the kidneys within 24 hours.

Indications for use

Injectran is recommended for the treatment of degenerative-dystrophic diseases of the joints and spine:

• osteoarthritis of the peripheral joints;
• intervertebral osteochondrosis and osteoarthritis.

Also, the drug is used for fractures in order to form callus.

Contraindications

• thrombophlebitis;
• bleeding, predisposition to bleeding;
• childhood;
• pregnancy and lactation;
• hypersensitivity to any of the constituents of the drug.

Injectran, instructions for use: method and dosage

Injectran injections are administered intramuscularly every other day, 1 ml (100 mg).

If the drug is well tolerated, starting from the fourth injection, the dose is increased to 2 ml (200 mg), the course of therapy is 25–35 injections. If necessary, 6 months after the end of the application of the drug, it is possible to prescribe a second course, the duration of which is determined by the attending physician individually.

In case of fractures for the formation of callus, the solution is injected every other day for 3-4 weeks (10-14 injections every other day).

Side effects

• skin and subcutaneous tissue: urticaria, itching, skin rash, erythema, dermatitis;
• immune system: angioedema, allergic reactions;

Overdose
