Release form and composition

Composition of one tablet:
- active substance: trimetazidine dihydrochloride - 35 mg;
- auxiliary components (core): microcrystalline cellulose, hypromellose (methocel K 4M Premium EP), aerosil (colloidal silicon dioxide), magnesium stearate;
- shell: hypromellose (hydroxypropyl methylcellulose), titanium dioxide, talc, macrogol (polyethylene oxide 4000), dye azorubin (E122) (acid red 2C for pharmaceuticals).
Pharmacological properties
Pharmacodynamics
Trimetazidine dihydrochloride, the active substance of Triducard, has antihypoxic and antianginal effects. By directly affecting the muscle cells of the heart (cardiomyocytes) and brain neurons, trimetazidine normalizes their metabolism and functioning. Increased energy potential, activation of the oxidative decarboxylation process and rationalization of oxygen consumption (by blocking fatty acid oxidation and enhancing the reactions of aerobic glycolysis) determine the cytoprotective effect of the drug.
Trimetazidine promotes myocardial contractility, prevents a decrease in the intracellular content of adenosine triphosphoric acid and phosphocreatinine. In acidosis, it improves the functioning of ion channels, prevents the accumulation of calcium and sodium in the muscle cells of the heart, and normalizes the intracellular concentration of potassium ions.
Due to the action of trimetazidine, the concentration of phosphates and the amount of intracellular acidosis, caused by reperfusion and myocardial ischemia, decrease.
In addition, the active substance suppresses the damaging effect of free radicals, helps to preserve the integrity of cell membranes, prevents the activation of neutrophils in the ischemic zone, reduces the release of the enzyme creatine phosphokinase from the cells and the severity of ischemic myocardial damage, and also increases the duration of the electric potential.
With angina pectoris, Triducard reduces the frequency of attacks (due to a decrease in nitrate consumption). Approximately 2 weeks after the start of treatment, blood pressure drops decrease, vestibular test results improve, exercise tolerance increases, hearing normalizes, tinnitus and dizziness decrease.
Trimetazidine helps to restore the functional activity of the retina in vascular diseases of the eyes.
Pharmacokinetics
- absorption and distribution: after oral administration, trimetazidine dihydrochloride is rapidly and almost completely absorbed in the gastrointestinal tract; bioavailability - 90%. After a single dose of trimetazidine at a dose of 35 mg, the maximum plasma concentration is reached after 2 hours and is approximately 55 ng / ml. The substance easily penetrates the histohematogenous barriers. 16% of the drug binds to plasma proteins;
- Excretion: the half-life (T 1/2) is 4.5–5 hours; excreted from the body by the kidneys (unchanged about 60%).
Indications for use
- ischemic heart disease - for the prevention of angina attacks (in complex treatment);
- dizziness due to vascular diseases;
- chorioretinal vascular disorders;
- cochleovestibular disorders of ischemic origin (including hearing impairment, tinnitus).
Contraindications
- severe renal failure (creatinine clearance less than 15 ml / min);
- severe liver failure;
- age up to 18 years;
- pregnancy and lactation period;
- increased individual sensitivity to any component of the drug.
Instructions for use of Triducard: method and dosage
Triducard is taken orally during meals.
Recommended daily dosage: 2 tablets (70 mg) in 2 divided doses; the duration of the course of therapy is determined by the doctor.
