Propicil is a drug of pronounced thyrostatic action.
Release form and composition
Dosage form Propitsil - tablets: round, biconvex, with a dividing line, white (20 or 100 pcs. In a glass bottle, in a cardboard box 1 bottle).
Composition of 1 tablet:
- Active ingredient: propylthiouracil - 50 mg;
- Auxiliary components: colloidal silicon dioxide, povidone, corn starch, pregelatinized corn starch, lactose monohydrate, magnesium stearate.
Indications for use
For patients with thyrotoxicosis, the indications for the use of Propicil are the following conditions / diseases:
- Toxic goiter or toxic adenoma of the thyroid gland;
- Preoperative preparation for resection of the thyroid gland and postoperative treatment;
- Preparation for therapy with radioactive iodine.
Contraindications
- Lactose intolerance, lactase deficiency, glucose-galactose malabsorption;
- Hypothyroidism;
- Leukopenia;
- Chronic active hepatitis;
- Severe adverse reactions with previous therapy with propylthiouracil (severe liver failure, agranulocytosis);
- Individual hypersensitivity.
During pregnancy, therapy is allowed under strict medical supervision after a careful assessment of the potential benefits to the mother and the possible risks to the fetus. Propicil is taken in the lowest dose to avoid miscarriage and the development of goiter and / or hypothyroidism in the fetus.
When breastfeeding, Propicil is the drug of choice, since its concentration in breast milk is 1/10 of the content of propylthiouracil in the serum of a nursing woman. In some cases, a newborn may develop hypothyroidism, therefore, medical supervision is necessary.
Method of administration and dosage
The tablets are taken orally, without chewing, with a sufficient amount of liquid. It is recommended to maintain an interval between doses of 6-8 hours.
For children under 3 years of age, the tablets are dissolved in water immediately before taking until a fine suspension is formed.
Daily dose, depending on the patient's age:
- Newborns (with hyperthyroidism): at the beginning of treatment, 5-10 mg / kg of body weight, divided into 3 doses (if there is no response to therapy, it is recommended to increase the dose by 50-100%); maintenance therapy - 3-4 mg / kg body weight;
- Children 6-10 years old: at the beginning of treatment - 1-3 tablets (50-150 mg); maintenance treatment - 1 / 2 -1 tablet (25-50 mg);
- Children of 10 years, adolescents and adults: early treatment - 1 1 / 2 -2 tablet (75-100 mg); maintenance therapy - 1-3 tablets (50-150 mg); in case of complications and after loading with iodine, it is necessary to increase the initial dose to 6-12 tablets (300-600 mg), distributed over 4-6 doses.
The half-life of maternal immunoglobulin is 2-3 months, by which time spontaneous restoration of the functions of the thyroid gland of the newborn is possible.
In old age, propylthiouracil is recommended in lower doses.
No dose adjustment is required in patients with renal insufficiency or on hemodialysis.
For liver diseases, Propicil is used in prescribed doses, taking into account the recommended contraindications.
The duration of therapy is determined by the individual needs of the patient and can be, for example, with thyrotoxicosis, toxic adenoma of the thyroid gland and diffuse toxic goiter from 1.5 to 2 years.
In the preparatory period before surgery or treatment with radioactive iodine, the duration of drug use should correspond to the individual needs of the patient.
Propicil is allowed to be used both for monotherapy of diffuse toxic goiter and as part of complex treatment with sodium levothyroxine according to the "block-replace" scheme.
Side effects
- Sometimes: skin rash, hives, stomach pain, arthralgia (without symptoms of joint inflammation), goiter in newborns;
- Rarely: agranulocytosis, accompanied by septic complications;
- Very rarely (mainly with high doses of propylthiouracil): liver damage (transient cholestasis, hepatocellular necrosis), allergic reactions, drug fever, thrombocytopenia, lymphadenopathy;
- Selected cases: neuromuscular disorders, polyarthritis, impaired sense of smell and taste, vasculitis, lupus-like syndrome, hearing loss, periarteritis nodosa, gastrointestinal disorders (nausea, vomiting), dizziness, erythropoiesis disorder, hemolysis, positive Coombs' reactions, interstitial, asthma, peripheral edema, alopecia.
During therapy with Propicil, an increase in the thyroid gland is possible.
In the case of diagnosing liver failure, agranulocytosis, vasculitis (conditions that pose a potential threat to the patient's life), further therapy should be carried out under close medical supervision.
Cases of acute intoxication with propylthiouracil were not observed.
With chronic overdose, goiter and hypothyroidism develop, the symptoms of which depend on the degree of decrease in the function of the thyroid gland, due to the specific action of the thyrostatic. There are no special measures / measures for the treatment of this condition. Gastric lavage and endoscopic removal of tablet residues are ineffective due to the rapid absorption of the active substance.
After discontinuation of the drug, as a rule, restoration of thyroid function occurs spontaneously.

The development of agranulocytosis can proceed almost unpredictably and rapidly (within several hours). The patient should be informed about the clinical symptoms of the disease (malaise, fever, stomatitis, tonsillar tonsillitis) and, if they appear, conduct a blood picture.
Before starting therapy, a complete blood count should be done, and during treatment, the functional state of the thyroid gland (the concentration of thyroid hormones and / or TSH in the blood) should be monitored.
When treated with too high doses of propylthiouracil, the already existing goiter may increase, which in the case of intrathoracic or thoracic goiter may lead to compression of the anatomical structures in the mediastinum.
A pathological change in the number of blood cells and an increase in transaminase activity is an indicator for the use of Propicil only under strict medical supervision.
Drug interactions
Block-and-replace therapy in combination with levothyroxine sodium requires an increase in propylthiouracil dose.
The thyrostatic effect of Propicil can reduce the simultaneous or previous use of iodine-containing agents or X-ray contrast agents.
Additional dose adjustment of coumarin and propranolol derivatives may be required, since propylthiouracil can affect the effect of their free active fractions.
Myelotoxic drugs increase the hematotoxicity of Propicil.
