Momat is a glucocorticosteroid drug for external and local use; a hormonal preparation that effectively eliminates the symptoms of skin diseases that are sensitive to hormone therapy, and helps to improve the condition of the skin.
Release form and composition
The drug is produced in the form of a cream / ointment for external use 0.1%; white (5 or 15 g each in aluminum tubes equipped with a closed end nozzle and a pointed cap for piercing the protective membrane).
Composition for 1 g of cream:
- active ingredient: mometasone furoate - 1 mg;
- auxiliary components: white beeswax, petroleum jelly (white soft paraffin), propylene glycol monostearate, cetomacrogol, propyl parahydroxybenzoate, propylene glycol, methyl parahydroxybenzoate, stearyl alcohol, purified water.
Composition for 1 g of ointment:
- active ingredient: mometasone furoate - 1 mg;
- auxiliary components: white beeswax, petroleum jelly (white soft paraffin), propylene glycol monostearate.
Pharmacological properties
Pharmacodynamics
The active ingredient of Momat cream / ointment - mometasone - is a synthetic glucocorticosteroid (GCS), an analogue of endogenous hormones produced by the adrenal cortex, which has an antipruritic, anti-inflammatory and antiexudative effect. GCS induce in the body the release of lipocortin proteins that inhibit phospholipase A2 and control the biosynthesis of inflammatory mediators (prostaglandins and leukotrienes) by inhibiting the release of arachidonic acid, their common precursor.
Pharmacokinetics
Mometasone is absorbed slightly. 8 hours after applying Momat to intact skin, once without applying an occlusive dressing, the active substance penetrates into the systemic circulation in an amount of up to 0.4% (for cream) or 0.7% (for ointment). Mometasone intensively undergoes hepatic metabolism, is excreted mainly by the kidneys and in small quantities with bile.
The absorption of Momat ointment is negligible. 8 hours after a single application to intact skin (without occlusive dressing), about 0.7% of the active substance is found in the systemic circulation. Mometasone is extensively metabolized in the liver, excreted mainly by the kidneys and in residual quantities with bile. The half-life (T 1/2) is about 5.8 hours.
Indications for use
According to the instructions, Momat is used to treat skin diseases with severe inflammation, itching and burning due to dermatoses (atopic / seborrheic dermatitis, psoriasis) that can be treated with glucocorticosteroids.
Contraindications
Absolute:
- perioral dermatitis;
- rosacea;
- infections of the skin of a bacterial, viral (herpes simplex, chickenpox, herpes zoster) or fungal nature;
- post-vaccination reactions;
- tuberculosis, syphilis;
- children under 2 years of age (for the drug in the form of an ointment);
- pregnancy and the period of breastfeeding [application to large areas of the skin and / or long-term therapy];
- hypersensitivity to the main active ingredient, other GCS and any of the auxiliary components of the cream / ointment.
Care should be taken to apply the drug to the skin of the face and in the areas of diaper rash on intertriginous skin; under occlusive dressings; on large areas of the skin and (or) for a long period of time (especially in children).
Instructions for use Momat: method and dosage
Momat cream and ointment should be applied externally, in a thin layer, to the affected skin. The recommended dosage is 1 time per day. The duration of the treatment course depends on its effectiveness, the tolerability of the drug, as well as the presence and severity of negative side reactions.
Side effects
- endocrine system: the use of external forms of GCS for a long time, in the treatment of large areas of skin, under occlusive dressings (especially in pediatric practice) can contribute to the development of adrenal insufficiency and Cushing's syndrome;
- dermatological reactions: in rare cases - acne, folliculitis, irritation / dryness of the skin, burning sensation, hypertrichosis, hypopigmentation, maceration of the skin, perioral dermatitis, prickly heat, accession of a secondary infection, striae, signs of skin atrophy; ≤ 1% of cases - the formation of papules / pustules;
- hypersensitivity reactions: rarely - allergic contact dermatitis, pruritus.
Overdose
An overdose symptom is inhibition of the function of the hypothalamic-pituitary-adrenal system, including secondary adrenal insufficiency.
It is recommended to carry out symptomatic therapy with correction of the electrolyte balance (if necessary) and discontinuation of the drug (after prolonged use - gradual).
special instructions
Due to the application of a cream / ointment for a long time on large areas of the skin, especially under occlusive dressings, a systemic effect of GCS may develop. In connection with the foregoing, patients need careful monitoring for the appearance of signs of suppression of the function of the hypothalamic-pituitary-adrenal system and the development of hypercortisolism (Cushing's syndrome).
Avoid getting Momat ointment and cream in the eyes.
Propylene glycol, which is part of the drug, can cause irritation at the application sites. With such a reaction, its use must be discontinued and a course of appropriate therapy initiated.
It is necessary to take into account the ability of GCS to change the manifestations of some skin diseases, which can complicate the diagnosis. In addition, due to the use of GCS, a delay in wound healing is possible.
A sudden interruption of treatment after a prolonged course of GCS use can provoke the development of rebound syndrome, the manifestation of which is possible in the form of dermatitis, aggravated by intense redness of the skin and a burning sensation. Therefore, after a long course, the use of the drug should be stopped gradually, for example, using an intermittent scheme.
Application during pregnancy and lactation
The efficacy and safety of using Momat ointment and cream during pregnancy and during breastfeeding has not been specifically studied.
It is known that corticosteroids penetrate the placental barrier, therefore, pregnant women need to avoid long-term treatment with high doses of mometasone due to the threat of a negative effect on fetal development.
There is data on the release of GCS in breast milk. If it is necessary to use Momat in high doses and (or) for a long time during lactation, breastfeeding should be interrupted.
Pediatric use
The efficacy and safety of using Momat in children for more than 6 weeks has not been studied.
Since children have a greater surface area to body weight ratio than adults, they are at greater risk of hypothalamic-pituitary-adrenal depression and hypercortisolism.
Long-term GCS therapy can lead to impaired growth and development in children.
In connection with the above, in pediatric practice, Momat is required to be used in the minimum effective doses, the shortest course necessary to achieve the effect.
Drug interactions
A study of the drug interaction of mometasone with local (external) use with other substances / drugs has not been conducted.
