Cordipine is a dihydropyridine derivative, a calcium channel blocker with antianginal and antihypertensive effects.
Release form and composition
Dosage form - film-coated tablets (10 pcs. In strips, 5 strips in a cardboard box).
Active ingredient: nifedipine, 1 tablet - 10 mg.
Additional components: pregelatinized starch, sodium lauryl sulfate, povidone, talc, lactose monohydrate, colloidal anhydrous silicon dioxide, corn starch.
Shell composition: macrogol, titanium dioxide, talc, methacrylic acid copolymer, quinoline yellow dye (E104).
Indications for use
- Arterial hypertension and relief of hypertensive crises;
- Stable angina pectoris and Prinzmetal angina pectoris (relief of attacks and their prevention);
- Raynaud's disease.
Contraindications
Absolute:
- Arterial hypotension (systolic pressure below 90 mm Hg);
- Collaptoid states;
- Cardiogenic shock;
- Unstable angina;
- Severe heart failure;
- Arterial hypotension;
- Severe aortic stenosis;
- Acute myocardial infarction (first week of the acute period);
- Severe myocardial insufficiency;
- Liver failure;
- Porphyria;
- Hypovolemia;
- Syndrome of impaired absorption of glucose / galactose, galactosemia, lactose deficiency;
- Age under 18;
- Hypersensitivity to drug components.
Relative:
- Mitral valve stenosis;
- Severe bradycardia;
- Severe cerebral circulation disorders;
- Obstruction of the gastrointestinal tract;
- Obstructive cardiomyopathy with myocardial hypertrophy;
- Sick sinus syndrome;
- Myocardial infarction with concomitant left ventricular failure;
- Tachycardia;
- Malignant arterial hypertension;
- Renal failure;
- Hemodialysis;
- Diabetes;
- Simultaneous use of beta-blockers, digoxin-containing agents;

Method of administration and dosage
Cordipine should be taken orally, swallowing the tablets whole and drinking plenty of liquid. If there is a need for a rapid development of the effect, the tablet can be chewed.
The dosage regimen in each case is determined by the doctor individually.
The initial daily dose is usually 30 mg (1 tablet 3 times a day). The maintenance daily dose is 30-40 mg (1 tablet 3-4 times a day). If the effect is insufficient, it is possible to increase the dose to 60-80 mg per day, in some severe cases the daily dose is increased to 90-120 mg, dividing the dose into 3-4 doses.
For relief of an attack of angina pectoris and hypertensive crisis, 2-3 tablets are prescribed sublingually.
Side effects
- Nervous system: vertigo, headache, sleep disturbances, increased fatigue, paresthesia;
- Cardiovascular system: chest pain, orthostatic hypotension, cardiac arrhythmias (asystole, bradycardia, tachycardia), exacerbation of ischemic heart disease;
- Allergic reactions: urticaria, itching, rash;
- Gastrointestinal tract: gingival hyperplasia, constipation / diarrhea, heartburn, nausea, intrahepatic cholestasis, increased hepatic transaminase activity, liver dysfunction;
- Hematopoietic organs: thrombocytopenia, aplastic anemia, leukopenia, agranulocytosis;
- Urinary system: an increase in the amount of urine, impaired renal function;
- Musculoskeletal system: muscle weakness, myalgia, tremors, muscle cramps;
- Senses: blurred vision;
- Others: peripheral edema, hot flashes, hyperemia of the skin, gynecomastia.
special instructions
At the beginning of treatment with Cordipin, especially in the case of a combination with beta-blockers, hypotension may develop.
Special observation at the beginning of therapy is required for elderly people, as well as for patients with pulmonary hypertension, unstable angina pectoris, hypertrophic cardiomyopathy, severe liver dysfunction, diabetes mellitus.
In patients with severe coronary insufficiency due to reflex tachycardia, myocardial ischemia may worsen (manifested by an increase in pain attacks).
If it is necessary to conduct an inhalation test with methacholine to assess bronchial hyperreactivity, Cordipin should be canceled.
The drug may affect the results of some laboratory parameters, including the parameters of alkaline phosphatase, lactate dehydrogenase, alanine aminotransferase and aspartate aminotransferase. These changes are not always accompanied by clinical manifestations, however, in some cases, symptoms of jaundice or cholestasis may appear.
Discontinue treatment with nifedipine gradually. abrupt withdrawal, especially after prolonged therapy, can lead to the development of withdrawal syndrome.
In some patients, especially at the beginning of the use of Cordipin, side effects from the nervous system are possible, therefore, in the initial stages of therapy, it is recommended to refrain from driving vehicles and performing potentially dangerous types of work. Further, the degree of restrictions is determined individually.
During treatment with the drug, one should refrain from drinking alcoholic beverages, since alcohol in combination with nifedipine can cause dangerous hypotension.
Drug interactions
- Other antihypertensive drugs and diuretics: there is a synergistic effect of these drugs, a significant decrease in blood pressure is possible;
- Beta-blockers: the hypotensive effect and antianginal effect are potentiated, reflex tachycardia is stopped;
- Tricyclic antidepressants, ranitidine, cimetidine, phenothiazine derivatives: the hypotensive effect of nifedipine is potentiated;
- Calcium preparations, rifampicin: the effect of nifedipine decreases;
- Digoxin, carbamazepine, phenytoin, theophylline: their serum concentration increases;
- Quinidine: its concentration in the blood decreases;
- Ethanol and ethanol-containing drugs: increased hypotensive effect;
- Organic nitrates (nitroglycerin, isosorbide): the antianginal effect and hypotensive effect are potentiated (it is recommended to avoid such a combination because of the risk of severe hypotension);
- Fluconazole, phenobarbital: the plasma concentration of nifedipine decreases;
- Fluoxetine: side effects of nifedipine are enhanced;
- Cimetidine: the plasma concentration of nifedipine increases and, as a result, its effects are enhanced;
- Anticholinergic drugs: deterioration of attention and memory impairment in gerontological patients;
- Saline mania: the risk of developing myasthenia gravis increases;
- Diltiazem: development of severe hypotension is possible;
- Fentanyl: additionally lowers blood pressure (if anesthesia with fentanyl is required, Cordipine should be canceled 36 hours before).
During treatment, it is recommended to refrain from drinking grapefruit juice, as it can enhance the effect of the drug.
