Ofloxacin is a broad-spectrum antimicrobial drug belonging to the group of fluoroquinolones.

Release form and composition

  • Film-coated tablets: biconvex, round, shell and layer on a cross-section of almost white color (10 pcs. In blisters, 1 package in a cardboard box);
  • Solution for infusion: clear greenish-yellow liquid (100 ml each in colorless or dark glass bottles, 1 bottle in a cardboard box);
  • Eye ointment 0.3%: a homogeneous substance of yellow, white with a yellow tinge or white (5 g each in aluminum tubes, 1 tube in a cardboard box).

Composition of 1 tablet:

  • Active ingredient: ofloxacin - 0.2 g or 0.4 g;
  • Auxiliary components: microcrystalline cellulose, corn starch, talc, calcium stearate, povidone, aerosil;
  • Sheath: hydroxypropyl methylcellulose (hypromellose), titanium dioxide, talc, propylene glycol, polyethylene oxide 4000 (macrogol 4000).

Composition of 1 ml solution:

  • Active ingredient: ofloxacin - 0.002 g;
  • Auxiliary components: sodium chloride, distilled water.

Composition of 1 g of ointment:

Indications for use

Ofloxacin tablets and solution for infusion is recommended for use in infectious and inflammatory diseases of the following organs and systems:

  • Respiratory tract (pneumonia, bronchitis);
  • ENT organs (otitis media, sinusitis, laryngitis, pharyngitis);
  • Skin and soft tissues;
  • Skeletal system (joints and bones);
  • Abdominal cavity (including gastrointestinal tract infections) and biliary tract;
  • Kidneys (pyelonephritis) and urinary tract (cystitis, urethritis);
  • Pelvic organs (salpingitis, oophoritis, endometritis, cervicitis, prostatitis, parametritis) and genitals (colpitis, epididymitis, orchitis).

Also, the drug is used to treat septicemia (solution only), gonorrhea, chlamydia, meningitis, and for the prevention of infections in patients with impaired immune status (including patients with neutropenia).

Ointment

  • Bacterial infections of the eyelids, cornea and conjunctiva (bacterial ulcers and keratitis of the cornea, blepharitis, blepharoconjunctivitis, conjunctivitis);
  • Dacryocystitis, meibomitis (barley);
  • Chlamydia of the eyes;
  • Infectious complications after surgery for eye injury or removal of a foreign body (for prevention).

Contraindications

Absolute for all forms of release:

  • The period of pregnancy and breastfeeding (lactation);
  • Hypersensitivity to the components of Ofloxacin and other quinolone derivatives.

Additionally for tablets and solution:

  • Deficiency of glucose-6-phosphate dehydrogenase;
  • Decrease in the threshold of seizure activity, including the period after a stroke, traumatic brain injury, or inflammatory diseases of the central nervous system (CNS);
  • Epilepsy, including history;
  • Children and adolescents up to 18 years of age (since the growth of the skeleton is not yet complete).

Additionally for the ointment:

  • Chronic conjunctivitis of non-bacterial etiology;
  • Age up to 15 years.

It is required to take pills with caution in case of atherosclerosis of cerebral vessels, cerebrovascular accidents (data in history), organic lesions of the central nervous system, chronic renal failure.

Method of administration and dosage

The dose and regimen of taking the drug in the form of tablets and solution for infusion is selected individually by the attending physician, depending on the severity of the infection and its localization, as well as on the general condition of the patient, sensitivity of microorganisms, and liver and kidney function.

In patients with impaired renal function with creatinine clearance (CC) of 20-50 ml / min, a single dose is 50% of the recommended dose (frequency of administration 2 times a day), or a full single dose is taken once a day. With CC <20 ml / min, the recommended dose on the first day is 0.2 g, then 0.1 g every other day.

Patients on hemo- and peritoneal dialysis should use 0.1 g of Ofloxacin 1 time per day.

In the case of liver failure, the maximum daily dose is 0.4 g.

The duration of the course of therapy depends on the sensitivity of the pathogen and the clinical picture. After the disappearance of the symptoms of the disease and the normalization of the temperature for another 3 days, it is required to continue using the drug.

Tablets

Tablets are taken orally, whole, before meals or during meals with water.

The recommended dosing regimen for adults is 0.2-0.8 g per day, divided into 2 doses. The course of treatment is 7-10 days. A daily dose of up to 0.4 g can be administered in 1 dose, preferably in the morning.

In the case of acute gonorrhea, the tablets are taken once in a dose of 0.4 g.

The duration of the course of therapy: salmonellosis - 7-8 days, uncomplicated infections of the lower urinary tract - 3-5 days.

Solution for infusion

The drug is administered by intravenous drip.

Start therapy with Ofloxacin with intravenous drip 0.2 g for 30-60 minutes. In case of improvement of the patient's condition, they are transferred to taking the drug inside (tablets) while maintaining the dosage.

Recommended doses of Ofloxacin, depending on the disease and location of the infection:

  • Urinary tract - 0.1 g 1-2 times a day;
  • Kidneys and genitals - 0.1-0.2 g each, divided into 2 injections per day;
  • Respiratory tract, ENT organs, skin and soft tissues, bones and joints, abdominal cavity, as well as septic infections - 0.2 g each, divided into 2 injections per day; an increase in the daily dose, if necessary, up to 0.4 g is allowed, while maintaining the frequency of administration;
  • Prevention of infection in patients with a pronounced decrease in immunity - 0.4-0.6 g per day.

Ointment

Ofloxacin ointment is used topically in ophthalmology.

It is necessary to carefully pull down the lower eyelid of the affected eye and insert a strip of ointment about 1 cm long into the conjunctival sac (slightly squeezing the tube). Then release the eyelid, close the eye and move the eyeball to evenly distribute the ointment.

The recommended dosage regimen: 2-3 times a day per eyelid, a strip of ointment 1 cm long - 0.00012 g of Ofloxacin is laid. For the treatment of eye chlamydia, the number of applications is increased up to 5-6 times a day.

The duration of the course of therapy should not exceed 2 weeks; for the treatment of chlamydial infections, the course must be extended to 4-5 weeks.

Side effects

Tablets, solution for infusion

  • Digestive system: nausea, vomiting, flatulence, diarrhea, anorexia, cholestatic jaundice, abdominal pain (including gastralgia), hyperbilirubinemia, increased activity of hepatic transaminases, pseudomembranous enterocolitis;
  • Central nervous system: dizziness, headache, uncertainty of movement, convulsions, tremors, numbness and paresthesias of the extremities, nightmares, intense dreams, psychotic reactions, increased excitability, anxiety, phobias, confusion, depression, hallucinations, color vision disturbance, increased intracranial pressure, diplopia, disorders of taste, hearing, smell and balance;
  • Musculoskeletal system: arthralgia, tendonitis, tendosynovitis, myalgia, tendon rupture;
  • Cardiovascular system: collapse, vasculitis, tachycardia; for the solution - a decrease in blood pressure (BP) (with a sharp decrease in blood pressure, the administration is stopped);
  • Skin: petechiae (punctate hemorrhages), bullous hemorrhagic dermatitis, vasculitis, papular rash;
  • Hematopoietic organs: agranulocytosis, leukopenia, anemia, pancytopenia, thrombocytopenia, aplastic and hemolytic anemia;
  • Urinary system: hypercreatininemia, impaired renal function, acute interstitial nephritis, increased urea;
  • Hypersensitivity reactions: itching, skin rash, urticaria, allergic nephritis, allergic pneumonitis, eosinophilia, Quincke's edema, fever, bronchospasm, Lyell's syndrome, Stevens-Johnson's syndrome, photosensitivity, erythema multiforme; rarely - anaphylaxis;

Ofloxacin for pneumonia caused by pneumococci is not the drug of choice; it is also not recommended for the treatment of acute tonsillitis.

You should not use the drug for a course longer than 2 months. During the period of therapy, it is necessary to avoid exposure to ultraviolet radiation, both natural sunlight and artificial radiation with mercury-quartz lamps and / or in a solarium.

In case of adverse reactions from the central nervous system, allergic responses, pseudomembranous colitis, the drug must be canceled. In the case of pseudomembranous colitis confirmed by colonoscopic and / or histological studies, oral vancomycin and metronidazole are recommended.

Due to the increased risk of developing vaginal candidiasis during therapy with Ofloxacin, women should not use hygienic tampons.

Rarely occurring tendonitis can cause tendon rupture (mainly Achilles), especially in elderly patients. At the first sign of tendonitis, it is required to immediately stop therapy, immobilize the Achilles tendon and seek advice from an orthopedist.

Against the background of the use of Ofloxacin, a complication of the course of myasthenia gravis is possible, in patients with a predisposition - an increase in the frequency of attacks of porphyria.

Since Ofloxacin interferes with the shedding of Mycobacterium tuberculosis, false negative results are possible during bacteriological diagnosis of tuberculosis.

Patients with impaired renal or hepatic function require monitoring of the plasma concentration of Ofloxacin. In the case of severe hepatic and renal insufficiency, the risk of developing toxic reactions increases, therefore a dose reduction is necessary.

It is recommended to avoid taking ethanol (alcoholic beverages) during therapy.

In pediatrics, the drug is used only for the treatment of life-threatening severe infections with the condition that the estimated clinical efficacy exceeds the potential risk of side effects, when less toxic drugs cannot be used. In this case, the daily dose for children is determined depending on body weight: average - 0.0075 g / kg, maximum - 0.015 g / kg.

During therapy, one should refrain from engaging in potentially hazardous activities that require speed of psychomotor reactions and increased concentration of attention, including from driving.

Ointment

When using the ointment, wearing soft contact lenses is not recommended.

Sunglasses should be worn (as photophobia may develop) and prolonged exposure to bright light should be avoided.

Ofloxacin ointment should not be injected into the anterior chamber of the eye or subconjunctivally.

Immediately after applying the ointment, a violation of the clarity of visual perception is possible, this can complicate the process of driving a vehicle and working with complex mechanisms.   • Antacids (food products) containing calcium, magnesium, aluminum, iron salts - reduce the absorption of Ofloxacin, forming insoluble complexes (an interval of at least 2 hours must be observed between their use);
  • Theophylline - Ofloxacin reduces its clearance by 25% (a decrease in theophylline dose is recommended);
  • Cimetidine, methotrexate, furosemide and drugs that block tubular secretion - increase the concentration of Ofloxacin in plasma;
  • Glibenclamide - Ofloxacin increases its concentration in plasma;
  • Indirect anticoagulants-antagonists of vitamin K - requires monitoring of the blood coagulation system;
  • Non-steroidal anti-inflammatory drugs (NSAIDs), derivatives of nitroimidazole and methylxanthines - the likelihood of neurotoxic side effects increases;
  • Glucocorticosteroids (GCS) - the risk of tendon rupture increases, especially in old age;
  • Drugs that alkalinize urine (citrates, carbonic anhydrase inhibitors, sodium bicarbonate) - increase the risk of developing nephrotoxic effects, crystalluria.

Solution for infusion Ofloxacin is compatible with 5% fructose solution, isotonic NaCl solution, Ringer's solution, 5% dextrose (glucose) solution.

Ointment

If it is necessary to simultaneously use Ofloxacin ointment with other eye ointments / drops, the interval between their use should be at least 15 minutes, the last ofloxacin is injected into the conjunctival sac.
