Release form and composition

Dosage form - cream for external use: white, homogeneous (5 or 15 g in aluminum tubes; in a cardboard box 1 tube and instructions for Gistan-N).

Composition of 1000 mg cream:

  • active substance: mometasone furoate - 1 mg;
  • auxiliary components: propylene glycol - 60 mg; chlorocresol - 1 mg; emulsion wax - 95 mg; sodium citrate - 1 mg; white soft paraffin - 185.64 mg; liquid paraffin - 35.66 mg; cetomacrogol - 2 mg; water - up to 1000 mg.

Pharmacological properties

Pharmacodynamics

Mometasone - the active substance of Histana-N, is a synthetic GCS (glucocorticosteroid). It has anti-inflammatory, anti-exudative and antipruritic effects.

The pharmacological action of the drug is aimed at inducing the release of proteins that inhibit phospholipase A 2 and lipocortins, which control the synthesis of prostaglandins and leukotrienes (inflammatory mediators), by inhibiting the release of arachidonic acid, their common precursor.

The main effects of mometasone:

  • prevention of marginal accumulation of neutrophils, which leads to a decrease in inflammatory exudate and production of cytokines, inhibition of macrophage migration and helps to reduce the processes of infiltration and granulation;
  • a decrease in inflammation, which is provided by a decrease in the formation of a chemotaxis substance (has an effect on late allergy reactions);
  • inhibition of the development of an immediate allergic reaction (the effect is based on inhibition of the production of metabolites of arachidonic acid and a decrease in the release of inflammatory mediators from mast cells).

With daily use in children with allergic dermatitis over 2 years of age, the therapeutic effect manifests itself in 3 weeks.

Pharmacokinetics

When applied topically, mometasone has a low degree of absorption. Approximately 0.4% of the dose is found in the systemic circulation 8 hours after application of the drug to intact skin (without using an occlusive dressing). In the event of damage and inflammation of the skin, the degree of absorption increases.

Indications for use

Gistan-N is prescribed for the treatment of dermatoses (including psoriasis, atopic and seborrheic dermatitis), accompanied by inflammation and itching of the skin, in which the use of GCS is indicated.

Absolute:

  • perioral dermatitis, rosacea;
  • tuberculosis, syphilis;
  • bacterial, fungal or viral (chickenpox, Herpes simplex, Herpes zoster) infectious skin lesions;
  • post-vaccination reactions;
  • pregnancy and lactation (when applied to large areas of the skin, long-term therapy);
  • age up to 2 years;
  • individual intolerance to the components of the drug or other GCS.

Relative (Gistan-N cream is prescribed under medical supervision):

  • application to the intertriginous surface of the skin and facial skin;
  • the use of occlusive dressings;
  • application on large areas of the skin and / or a long course (especially in children).

Gistan-N, instructions for use: method and dosage

Gistan-N cream is applied externally.

The drug is applied once a day in a thin layer to the affected skin areas.

The duration of use varies in the range from 7 to 28 days and depends on the effectiveness of treatment, the tolerability of the drug, the presence and severity of adverse events.

Side effects

With long-term use of Gistan-N and / or application to large areas of the skin (when the total area is larger than the area of the patient's palm), as well as when using occlusive dressings, especially in children and adolescents, it is possible to develop undesirable phenomena characteristic of systemic corticosteroids. including Itsenko-Cushing's syndrome and adrenal insufficiency.

Also during the period of therapy, local reactions may develop: paresthesias, burning, skin atrophy, acne, hypopigmentation, hypertrichosis, pruritus, folliculitis, perioral dermatitis, allergic contact dermatitis, striae, secondary infection, prickly heat, skin maceration.

When using occlusive dressings, the likelihood of the listed adverse reactions increases.

Overdose

  • the main symptoms of a mometasone overdose: inhibition of the function of the hypothalamic-pituitary-adrenal system, including secondary adrenal insufficiency;
  • therapy: symptomatic. If necessary, the electrolyte balance is corrected and Gistan-N is canceled (after long-term treatment, the drug is canceled gradually).

The safety profile of mometasone furoate during pregnancy / lactation has not been studied.
