Release form and composition

Dosage forms of release:

  • capsules: hard gelatinous, size No. 0, yellow; contents of capsules - powder with granules or particles of various sizes, pressed into cylinders, disintegrating when pressed, from yellow to yellow-green, greenish-yellow or yellow-brown color (10, 15, 20 pcs. in blisters made of polyvinyl chloride film and aluminum foil or 30, 50, 60, 90, 100 pcs. in polymer cans; in a pack of cardboard: 1 can; 3, 5, 6 blister packs of 10 pcs.; 2, 4, 6 blister packs per 15 pcs.; 3, 5 blister packs of 20 pcs.);
  • gel for external use 2%: transparent, homogeneous, from yellow to yellow-green or light brown (20, 25, 30, 40, 50, 60, 70, 80, 90 or 100 g each in orange glass jars, in polymer cans or in aluminum tubes; 1 can / tube in a cardboard box).

Composition of 1 capsule:

  • active substance: troxerutin - 300 mg;
  • excipients: microcrystalline cellulose - 44.1 mg, talc - 9.25 mg, sodium carboxymethyl starch [sodium starch glycolate (type A), primogel] - 9.25 mg, povidone (plasdon K90, Kollidon 90F) - 3.7 mg;
  • capsule shell: iron oxide yellow dye, titanium dioxide, gelatin.

Composition of 100 mg gel:

  • active substance: troxerutin - 2 mg;
  • excipient: sodium hydroxide 30% - 0.7 mg; carbomer 940 - 0.6 mg; benzalkonium chloride - 0.0575 mg; edetate disodium - 0.05 mg; purified water - up to 100 mg.

Pharmacological properties

Pharmacodynamics

Semi-synthetic bioflavonoid (rutin derivative) of the benzopyran class, has P-vitamin activity, has an angioprotective, venotonic, anti-edematous and anti-inflammatory effect, reduces capillary fragility and permeability.

The pharmacodynamic functions of the bioflavonoid troxerutin are associated with its participation in the inhibition and redox processes of hyaluronidase. Keeping hyaluronidase, troxerutin normalizes the hyaluronic acid of the cell membranes and reduces their permeability. It has antioxidant activity, as a result of which it prevents the oxidation of ascorbic acid, lipids and adrenaline. Reduces capillary fragility and permeability, prevents damage to the basement membrane of endothelial cells when exposed to various elements. Troxerutin increases the density of the vascular wall, reduces diapedesis of blood cells and exudation of the liquid part of the plasma. Reduces exudative inflammation in the vascular wall, reducing the adhesion of platelets to its surface. Slows down aggregation and increases the rate of deformation of erythrocytes.

The use of troxerutin is possible not only in the initial, but also in the later stages of therapy for chronic venous insufficiency. It is allowed to use the drug both in the form of monotherapy with one of the components, and in complex therapy. Troxerutin reduces swelling and a feeling of heaviness in the legs, reduces the intensity of cramps and pain, and improves tissue trophism.

Troxerutin relieves the symptoms associated with hemorrhoids (exudation, pain, bleeding, itching).

Troxerutin helps to slow the progression of diabetic retinopathy by affecting the resistance and permeability of the capillary walls.

The elimination of the development of microthrombosis of retinal vessels is facilitated by the influence of troxerutin on the rheological properties of blood.

Pharmacokinetics

  • capsules: absorption from the gastrointestinal tract is rapid, the maximum concentration of troxerutin in the blood plasma is achieved on average 1.5 hours after ingestion. Absorption is approximately 10-15%, with an increase in the dose of the drug increases the bioavailability. The half-life (T 1/2) is approximately 4-8 hours, the therapeutic concentration of the substance is maintained for 8 hours in the blood plasma. The second maximum concentration of troxerutin in blood plasma caused by enterohepatic recirculation is observed 30 hours after taking the drug. Troxerutin is partially metabolized in the liver with the formation of glucuronide and trihydroethylquercitin, mainly excreted through the intestine (up to 65–70%), a smaller part of the drug is excreted unchanged by the kidneys (up to 25%);
  • gel: when the gel is applied to the affected area, it quickly penetrates the epidermis, is found in the dermis after 30 minutes, and in the subcutaneous fatty tissue after 2–5 hours.

Indications for use

  • superficial periphlebitis and / or thrombophlebitis;
  • chronic venous insufficiency;
  • soft tissue hematomas and post-traumatic edema.

Capsules are used as part of a combination therapy for the following conditions / diseases:

  • hemorrhoids (to relieve symptoms);
  • trophic ulcers and / or trophic disorders in chronic venous insufficiency;
  • post-thrombotic syndrome;
  • phlebeurysm;
  • after removal and / or sclerotherapy of varicose veins of the lower extremities - in the form of auxiliary therapy;
  • diabetic retinopathy and angiopathy.

An additional indication for the use of Troxerutin in the form of a gel is varicose dermatitis.

Contraindications

  • Children and adolescents up to 18 years old;
  • hypersensitivity to drug components.

Additionally for capsules:

  • I trimester of pregnancy and lactation;
  • peptic ulcer of the duodenum, stomach, chronic gastritis in the acute stage.

An additional contraindication for Troxerutin in the form of a gel is a violation of the integrity of the skin.

In chronic renal failure, the drug should be used with extreme caution (with prolonged use).

Instructions for the use of Troxerutin: method and dosage

Capsules

Troxerutin capsules are taken orally, during meals, it is recommended to swallow them whole with a sufficient volume of water.

Recommended dosage: at the initial stage - 3 times a day, 1 pc., The result is achieved in about 14 days. Further, the reception is continued in the same dose or it is reduced to the average therapeutic dose - 2 times a day, 1 pc., Or suspended (in this case, the effect obtained remains for at least 4 weeks).

If necessary, you can increase the dose, but only after consulting your doctor. The average course of treatment is 3-4 weeks.

Supportive therapy - 1 pc. 1 time per day, with diabetic retinopathy - 2 pcs. 3 times a day. The course is 3-4 weeks.

Gel

Gel Troxerutin is applied externally.

A column of gel 4–5 cm high is applied in a thin layer to the affected skin and rubbed with light movements 3-4 times a day until completely absorbed. The daily dose should not exceed 20 cm of gel.

If necessary, it is possible to apply the gel under elastic stockings or bandages. The duration of treatment is 10 days.

Side effects

The use of capsules can cause the following adverse reactions from systems and organs:

  • immune system: hypersensitivity reactions;
  • nervous system: headache;
  • vascular system: flushing of the face;
  • gastrointestinal tract: stomach pain, nausea, vomiting, diarrhea, flatulence, erosive and ulcerative lesions of the gastrointestinal tract;
  • skin and subcutaneous tissues: pruritus, erythema, skin rash.

When applying the gel, in rare cases, allergic skin reactions have been noticed - eczema, urticaria, dermatitis.

Overdose

The following symptoms are possible with a systemic overdose of troxerutin (taking capsules or accidentally swallowing a large amount of gel): flushing of the face, nausea, agitation, headache.

In the presence of these adverse reactions, you should rinse the stomach and take activated charcoal (within an hour after ingestion).

Cases of gel overdose when applied externally have not been described.

special instructions

In the treatment of deep vein thrombosis or superficial thrombophlebitis, the use of Troxerutin capsules does not exclude the prescription of anti-thrombotic and anti-inflammatory drugs.

For the treatment of edema caused by concomitant diseases of the kidneys, liver and heart, capsules are ineffective.

Do not exceed the recommended doses and maximum treatment times for self-administration of the drug.

If during the use of the drug the severity of the symptoms of the disease does not decrease or the symptoms are aggravated, you should immediately consult a specialist.

The gel should only be applied to an undamaged surface.

It is required to avoid getting the gel in the eyes, mucous membranes and open wounds.

The gel is used in combination with ascorbic acid to enhance its effect in conditions characterized by increased vascular permeability (including influenza, scarlet fever, measles and allergic reactions).

If the drug is unintentionally swallowed, nausea, vomiting, burning sensation in the mouth, salivation are permissible. In such a case, gastric and oral cavity lavage is recommended, if necessary, symptomatic treatment.

When the gel gets into the eyes, mucous membranes and open wounds, local irritation is noted - burning, lacrimation, pain, hyperemia. In this case, it is necessary to wash off the agent with a large volume of isotonic sodium chloride solution or distilled water until the severity or complete disappearance of these reactions is significant.

Application during pregnancy and lactation

During the first trimester of pregnancy, the use of Troxerutin is contraindicated. The use of the drug in the II and III trimesters is determined by the attending physician and is possible only when the intended benefit to the mother outweighs the potential risk to the fetus.

There is no information on the penetration of the drug into breast milk. Its use during lactation is not recommended.

Pediatric use

According to the instructions, Troxerutin is contraindicated in persons under the age of eighteen due to insufficient data on the effectiveness and safety of its use.

With impaired renal function

Prolonged use in patients with chronic renal failure requires special care.

Drug interactions

With a single intake of troxerutin inside (capsules), it enhances the effect of ascorbic acid on the permeability and resistance of the vascular wall.
