Tetraderm is a drug for external use with anti-inflammatory, antibacterial, antifungal (fungicidal), glucocorticoid and wound healing effects.
Release form and composition
Dosage form - cream for external use: a uniform consistency, almost white or white (15 and 30 g each in aluminum tubes, in a cardboard box 1 tube and instructions for use of Tetraderm).
Active ingredients in 1 g of cream:
- dexpanthenol - 50 mg;
- econazole nitrate - 10 mg;
- gentamicin (in the form of sulfate) - 1 mg;
- mometasone furoate - 0.5 mg.
Auxiliary components: liquid paraffin (liquid paraffin, mineral oil), macrogol 25 cetostearyl ether, propylene glycol, cetostearyl alcohol, sodium dihydrogen phosphate dihydrate (monobasic sodium phosphate), macrogol 6 cetostearyl ether, purified water.
Pharmacological properties
Pharmacodynamics
Tetraderm is a combined external agent, the action of which is due to the properties of its active components:
- dexpanthenol is a derivative of pantothenic acid, a water-soluble B vitamin (vitamin B 5), which is a part of coenzyme A. Penetrates into all layers of the skin. Increases the strength of collagen fibers, accelerates mitosis, normalizes cell metabolism, stimulates skin regeneration, and has a weak anti-inflammatory effect. In case of damage to the skin or tissues, there is an increase in the need for pantothenic acid, and this deficiency can be compensated for by external use of dexpanthenol;
- econazole is a synthetic imidazole derivative. It has antibacterial and antifungal effects. It inhibits the biosynthesis of ergosterol, which regulates the permeability of the cell wall of pathogenic microorganisms. It penetrates well into tissues, easily dissolves in lipids. Active against yeast-like fungi of the genus Candida, Microsporum, dermatophytes Trichophyton, Corynebacterium minutissimum, Epidermophyton, Malassezia furfur (the causative agent of pityriasis versicolor) and some gram-positive bacteria (staphylococci, streptococci);
- gentamicin is an antibacterial agent from the aminoglycoside group. Has a bactericidal effect, provides a highly effective external treatment of primary and secondary bacterial skin infections. It is active against gram-positive Staphylococcus aureus (coagulase-negative, coagulase-positive and some strains that produce penicillinase) and against gram-negative microorganisms Proteus vulgaris, Escherichia coli, Aerobacter aerogenes, Klebsiella pneumoniae, Pseudomonas aeruginosa;
- mometasone is a synthetic glucocorticosteroid (GCS) with a local anti-inflammatory, antipruritic and antiexudative effect. It induces the release of proteins known collectively as lipocortins, which inhibit phospholipase A2 and control the biosynthesis of inflammatory mediators - prostaglandins and leukotrienes (due to the ability to inhibit the release of their common precursor arachidonic acid).
Pharmacokinetics
- dexpanthenol: Absorbed after application to the skin and converted to pantothenic acid. Binds to plasma proteins. Does not undergo metabolism, is excreted by the kidneys unchanged;
- econazole: penetrates into all layers of the skin and the nail plate. In therapeutic concentrations, it is noted in the dermis, horny and other layers of the epidermis. Undergoes insignificant systemic absorption, after which it is excreted through the intestines and kidneys (less than 1% of the dose);
- gentamicin: after application to the skin, it is absorbed in insignificant quantities, therefore it practically does not have systemic effects;
- mometasone: characterized by low absorption. 8 hours after a single application of Tetraderm to the skin without damage (without using an occlusive dressing), about 0.4% of the dose is found in the systemic circulation. The absorption of the substance is increased when there is damage or inflammation on the skin. It undergoes intensive metabolism in the liver. It is excreted mainly by the kidneys, in small quantities - with bile.
Indications for use
- dermatoses of an inflammatory nature with concomitant mycotic / bacterial infection or a high risk of secondary infection: atopic dermatitis (including diffuse neurodermatitis), simple and allergic dermatitis;
- dermatomycosis (candidiasis, dermatophytosis, versicolor versicolor), eczema and limited neurodermatitis, especially when localized in large folds of the skin and in the groin area;
- simple chronic lichen (limited neurodermatitis).
Contraindications
- application in the field of open wounds;
- perioral dermatitis;
- herpes simplex;
- chicken pox;
- lupus;
- rosacea;
- post-vaccination skin reactions;
- cutaneous manifestations of syphilis;
- age up to 18 years;
- hypersensitivity to any active or auxiliary component of Tetraderm cream.
Tetraderm cream should be used with caution in the following cases:
- violation of the integrity of the skin;
- lesions of large areas of the skin;
- pregnancy and the period of breastfeeding;
- long-term treatment;
- the need for occlusive dressings.
Tetraderm, instructions for use: method and dosage
Tetraderm cream is intended for external use. It should be applied in a thin layer to the affected skin and rubbed gently. Frequency rate of application - 2 times a day.
The duration of therapy is determined individually, depending on the severity of the disease, the size and location of the lesion. Typically, the course of treatment is 1-2 weeks, with dermatomycosis of the feet - 2-4 weeks.
If in the course of therapy there is no improvement in the condition or new symptoms appear, you should consult your doctor.
Side effects
- on the part of the skin: rarely (from ≥ 1/10000 to <1/1000) - itching, dryness and irritation of the skin, burning sensation, perioral dermatitis (rash or irritation around the mouth), skin atrophy (thinning and loss of elasticity), striae (stretch marks), acne, skin maceration (softening of the upper layers of the skin), prickly heat (skin irritation resulting from increased sweating), allergic contact dermatitis, hypopigmentation (lightening), hypertrichosis (excess hair growth), pustule and / or papule formation (rash); the frequency is unknown (it is not possible to establish the frequency according to the available data) - urticaria, skin hyperemia;
- from the nervous system: the frequency is unknown - paresthesia (numbness and tingling sensation);
- others: rarely - the addition of a secondary infection, folliculitis (inflammation of the hair follicle).
With long-term use of Tetraderm, application to large areas of the body and / or use under occlusive dressings, side effects may occur inherent in systemic glucocorticosteroids, including Itsenko-Cushing's syndrome and adrenal insufficiency.
Overdose
Mometasone with long-term topical use in high doses can cause suppression of adrenal function with the development of secondary adrenal insufficiency and symptoms of hypercortisolism, including Itsenko-Cushing's syndrome.
With a single overdose, gentamicin does not lead to any adverse reactions. With prolonged use and / or in high doses (exceeding the recommended ones), the growth of insensitive microflora, including fungal, is possible in the lesion.
Econazole and dexpanthenol in case of overdose do not cause side effects.
Treatment is symptomatic. If necessary, adjust the electrolyte balance. Acute symptoms of hypercortisolism are usually reversible. With the development of a fungal infection and in the case of uncontrolled growth of insensitive microorganisms, Tetraderm is canceled and appropriate therapy is carried out.
special instructions
Tetraderm is not recommended for use longer than 4 weeks.
Do not apply the cream to the periorbital area. Avoid contact with eyes. Avoid applying on the face and scalp. If possible, do not use under occlusive dressings.
Tetraderm should not be applied to open wounds and used to treat varicose trophic ulcers.
Sensitive areas of the body (armpits, perianal region and groin folds) are more at risk of stretch marks. Therefore, it is recommended to limit the duration of treatment for these areas.
In the event of irritation or signs of hypersensitivity, Tetraderm should be discontinued and a doctor should be consulted for alternative therapy.
With prolonged external use of the drug, the growth of insensitive microflora (including fungal) to the action of gentamicin is possible. In this case, you should also cancel Tetraderm and consult a doctor for the selection of alternative therapy.
Systemic absorption of gentamicin can be increased when the cream is applied to large areas of the body, especially in the presence of skin lesions and with prolonged treatment. As a result, symptoms of ototoxicity and other adverse reactions characteristic of gentamicin with its systemic use may develop.
There is a possibility of cross-allergic reactions to antibiotics from the aminoglycoside group.
With the external use of mometasone, side effects may occur, noted with systemic use of glucocorticosteroids, including inhibition of the function of the adrenal cortex. Systemic absorption of local GCS can be enhanced when Tetraderm is applied to large body surfaces, with prolonged treatment and the use of occlusive dressings. In these cases, side effects characteristic of systemic GCS may develop.
Mometasone can delay the healing process and can alter the appearance of certain skin conditions, making diagnosis difficult.
After prolonged therapy, abrupt withdrawal of GCS can cause the development of rebound syndrome, i.e. the appearance of dermatitis with severe redness of the skin and a burning sensation. For this reason, after a long course, Tetraderm should be canceled gradually.
Influence on the ability to drive vehicles and complex mechanisms
There is no data that would indicate a negative impact of Tetraderm on the speed of reactions and the ability to concentrate.
Application during pregnancy and lactation
Adequate and strictly controlled studies on the use of Tetraderm during pregnancy have not been conducted, therefore, the appointment of the drug is possible only in cases where the expected benefits are higher than the potential risks. In this case, you should not apply the cream to large areas of the body or for a long time.
Mometasone, like other glucocorticosteroids, passes into the mother's milk. If treatment is necessary during lactation, breastfeeding should be discontinued.
Pediatric use
Tetraderm is not used to treat children and adolescents under 18 years of age.
Drug interactions
Studies on possible drug interactions of Tetraderma with other drugs have not been conducted, but it is not recommended to use it in combination with other topical agents.
