Symbicort Turbuhaler is a combined anti-inflammatory and bronchodilator drug for inhalation use.

Release form and composition

Symbicort Turbuhaler is produced in the form of a metered powder for inhalation, consisting of white or almost white granules, usually round in shape (dosage 80 / 4.5 mcg and 160 / 4.5 mcg - 60 or 120 doses in plastic inhalers "turbuhaler", in a cardboard box 1 inhaler; dosage 320/9 μg - 60 doses in plastic inhalers "turbuhaler", in a cardboard box one inhaler).

Composition of 1 dose (delivered):

  • active ingredients: micronized budesonide - 80 μg, 160 μg or 320 μg, formoterol fumarate dihydrate - 4.5 μg (with 80 μg and 160 μg budesonide) or 9 μg (with 320 μg budesonide);
  • auxiliary components: lactose monohydrate.

Indications for use

  • bronchial asthma, adequately controlled by inhaled β 2 -adrenomimetics and long-acting glucocorticosteroids (GCS), or insufficiently controlled by inhaled β 2 -adrenomimetics and short-acting GCS - as treatment on demand; Symbicort Turbuhaler at a dosage of 80 / 4.5 mcg and 160 / 4.5 mcg can be used to relieve attacks and as maintenance therapy;
  • chronic obstructive pulmonary disease (COPD) - symptomatic treatment of severe disease with forced expiratory volume (FEV) of less than 1 / 2 of the estimated calculated level, and regularly recurring exacerbations history if disease symptoms persist despite treatment with long-acting bronchodilators.

Contraindications

Absolute:

  • children under 6 years of age;
  • children under 12 years of age (for a dosage form containing 320 mcg budesonide + 9 mcg formoterol);
  • hypersensitivity to any component of the drug.

Relative (Symbicort Turbuhaler is used with caution):

  • active or inactive tuberculosis;
  • the presence of viral, bacterial or fungal respiratory infections;
  • severe arterial hypertension;
  • diabetes;
  • thyrotoxicosis;
  • aneurysm of any localization;
  • uncontrolled hypokalemia;
  • pheochromocytoma;
  • idiopathic hypertrophic subaortic stenosis;
  • prolongation of the QT interval;
  • severe heart failure;
  • cardiac ischemia;
  • tachyarrhythmia.

Method of administration and dosage

The drug is not intended for the initial treatment of mild asthma, intermittent (episodic) and persistent course.

The dose is selected individually, taking into account the severity of the disease. In cases where the patient requires a combination of doses of active substances that differs from those in the Symbicort Turbuhaler preparation, GCS and / or β 2 -adrenomimetics are prescribed separately in different inhalers.

With bronchial asthma, there are two schemes for prescribing the drug:

  1. Use only as a constant maintenance therapy (for the relief of seizures, a short-acting β 2 -adrenostimulant is additionally used).
  2. Use as a supportive treatment and for the relief of seizures, both continuously and on demand, when symptoms develop.

For use as permanent maintenance therapy, adolescents from 12 to 17 years old, as well as adult patients 18 years of age and older, are prescribed 1-2 inhalations of Symbicort Turbuhaler twice a day (at a dosage of 80 / 4.5 mcg or 160 / 4.5 mcg). If necessary, in patients over 18 years of age, the dose can be increased to 4 inhalations twice a day. Children aged 6–11 years are prescribed 1–2 inhalations of the drug at a dose of 80 / 4.5 mcg twice a day.

When achieving control of asthma symptoms while using Symbicort Turbuhaler twice a day, it is recommended to reduce the dose to the lowest effective dose. If the attending physician decides that maintenance therapy should be carried out in combination with long-acting bronchodilators, it is necessary to switch to taking the drug once a day.

For the timely relief of seizures, the patient should have a separate inhaler with a short-acting β 2 -adrenostimulator. An increase in the frequency of its use indicates a deterioration in overall disease control. In this case, a revision of anti-asthma treatment is required.

When used as supportive therapy and for relief of seizures, the patient should always have Symbicort Turbuhaler with him. It is not recommended to use the drug for the treatment and relief of seizures in children and adolescents under 18 years of age.

The recommended dose for adult patients: 2 inhalations per day - one inhalation in the morning and one in the evening; either both inhalations only in the morning or only in the evening. For inhalation use the drug at a dosage of 80 / 4.5 mcg or 160 / 4.5 mcg. In some patients, it may be necessary to increase the dose to 2 inhalations of Symbicort Turbuhaler 160 / 4.5 mcg twice a day. When symptoms of an attack appear, one additional inhalation is prescribed, if during the next few minutes the symptoms increase, another inhalation is prescribed, but in total no more than 6 inhalations are carried out to stop one attack.

The maximum number of inhalations per day, as a rule, does not exceed 8, but sometimes, for a short time, it is possible to increase the daily number of inhalations to 12. Patients using more than 8 inhalations per day should contact their doctor to review therapy.

Symbicort Turbuhaler at a dosage of 320/9 mcg is prescribed only to patients over 12 years old, the recommended dose is 1 inhalation twice a day (for persons over 18 years old, the dose can be increased to 2 inhalations twice a day). Once the symptoms are under control, the dose should be reduced to the lowest effective dose.

With COPD Symbicort Turbuhaler is prescribed only to adult patients: 2 inhalations twice a day (the drug is in a dosage of 160 / 4.5 mcg) or 1 inhalation twice a day (the drug is in a dosage of 320/9 mcg).

Elderly patients do not need dose adjustment.

Instructions for use of the turbuhaler inhaler

Before the first inhalation, the turbuhaler should be prepared for work. To do this, unscrew and remove the cap, and then, while holding the inhaler in an upright position with the red dispenser down, turn the dispenser all the way, first in one direction, and then also all the way to the other. Perform the described procedure two times. While turning the dispenser, do not hold the inhaler by the mouthpiece.

The procedure for preparing the turbuhaler for work is carried out once, it does not need to be repeated before each use of the inhaler.

To inhale one dose, the patient must perform the following manipulations:

  1. Unscrew and remove the cap, and then, while holding the inhaler in an upright position with the red dispenser down, turn the dispenser all the way, first in one direction, and then also all the way to the other (a characteristic click is heard).
  2. Exhale, insert the mouthpiece into your mouth, wrap your lips tightly around it and inhale strongly through your mouth. Without exhaling, remove the inhaler from your mouth.

If more than one dose is required at a time, repeat the above steps.

After the end of inhalation, close the inhaler with a cap.

To prevent the development of a fungal infection of the oropharynx, it is recommended to rinse your mouth with water after each inhalation.

Side effects

  • respiratory system: often - candidiasis of the mucous membrane of the pharynx and oral cavity, hoarseness, cough, slight irritation in the pharynx; rarely - bronchospasm;
  • cardiovascular system: often - palpitations; less often, tachycardia; rarely - extrasystole, supraventricular tachycardia, atrial fibrillation; very rarely - fluctuations in blood pressure, angina pectoris;
  • central nervous system and sensory organs: often - headache; less often - dizziness, nausea, psychomotor agitation, sleep disturbances, anxiety; very rarely - taste disturbances, behavioral disturbances (mainly in children), depression;
  • musculoskeletal system: often - tremor; less often - convulsions;
  • metabolism: rarely - hypokalemia; very rarely - symptoms of systemic effects of GCS (including hypofunction of the adrenal glands, decreased bone mineral density, growth retardation in children, glaucoma and cataracts), hyperglycemia;
  • dermatological and allergic reactions: less often - bruising; rarely - dermatitis, urticaria, itching, exanthema, Quincke's edema, anaphylactic reactions.

The systemic effect of GCS for inhalation is most often observed when high doses of the drug are taken for a long time.

When using β 2 -adrenomimetics, it is possible to increase the content of glycerol, free fatty acids, ketone derivatives and insulin in the blood.

special instructions

Abrupt withdrawal of the drug is not recommended. If treatment is no longer required, the dose should be reduced gradually.

Symbicort Turbuhaler at a dosage of 80 / 4.5 mcg and 320/9 mcg is not intended for the treatment of severe bronchial asthma.

The maintenance dose of the drug should be taken regularly, even in the absence of symptoms of the disease.

Symbicort Turbuhaler is not recommended to be prescribed in case of exacerbation or significant deterioration of the course of bronchial asthma.

During treatment, the glucose concentration should be monitored in persons with diabetes mellitus.

One dose of the drug contains less than 1 mg of lactose, which is not critical for patients with lactose intolerance.

Symbicort Turbuhaler does not affect the ability to drive a car or other transport and perform work associated with increased concentration. However, you should take into account some side effects of the drug (for example, dizziness) and be careful when they develop.

Drug interactions

If it is necessary to prescribe a drug with ketoconazole or other potent inhibitors of CYP3A4, the interval between their doses should be maximized, since the plasma concentration of budesonide may increase.

Simultaneous use with β-blockers (including eye drops) is not recommended, since they can weaken the effect of formoterol.

When administered together with disopyramide, phenothiazine, quinidine, procainamide, antihistamines, tricyclic antidepressants and monoamine oxidase inhibitors, it is possible to lengthen the QT interval with and increase the likelihood of ventricular arrhythmias.

Oxytocin, levothyroxine, ethanol and levodopa can reduce the tolerance of the heart muscle to β 2 -adrenomimetics.

With the simultaneous use of the drug Symbicort Turbuhaler with procarbazine, furazolidone and monoamine oxidase inhibitors, an increase in blood pressure is possible; with xanthine derivatives, diuretics and mineral derivatives of GCS - the hypokalemic effect of β 2 -adrenomimetics may be enhanced; with other β-adrenergic receptor agonists - an increase in the side effects of formoterol is possible.

When carrying out general anesthesia with halogenated hydrocarbons in patients taking Symbicort Turbuhaler, the risk of arrhythmias increases.

Budesonide does not interact with other drugs used to treat bronchial asthma.
