Release form and composition

Wessel Douai F is available in 2 dosage forms:

  • Solution for injection - transparent yellow or light yellow solution (2 ml in ampoules made of transparent dark glass, 5 or 10 ampoules in a blister strip, 1 or 2 packages in a cardboard box);
  • Capsules - gelatinous, soft, brick-red, oval (25 pcs. In a blister, 2 blisters in a cardboard box).

The composition of 1 ampoule includes:

  • Active substance: sulodexide - 600 LE (lipasemic units);
  • Auxiliary components: sodium chloride - 18 mg; water for preparation of injections - up to 2 ml.

Wessel Douay F, whose active ingredient is sulodexide, is a biological drug that is a natural mixture of glycosaminoglycans (GAGs). It consists of dermatan sulfate (20%) and a heparin-like fraction, the molecular weight of which is 8000 daltons (80%).

The mechanism of action of sulodexide is based on two main properties: the affinity of the fast-acting heparin-like fraction for antithrombin III (ATIII) and the affinity of the dermatan fraction for heparin cofactor II (CGII).

The angioprotective effect is due to the regeneration of the functional and structural integrity of the vascular endothelial cells, as well as the normalization of the density of the negative electric charge that the pores of the vascular basement membrane have. Also, sulodexide improves the rheological parameters of the blood by reducing the content of triglycerides (the substance activates a lipolytic enzyme - lipoprotein lipase, which is responsible for the hydrolysis of triglycerides included in LDL).

The effectiveness of the use of Wessel Douet F in diabetic nephropathy is explained by the ability of sulodexide to inhibit the synthesis of the extracellular matrix and to reduce the thickness of the basement membrane due to a decrease in the proliferation of mesangium cells.

The profibrinolytic effect is associated with an increase in the concentration of tissue plasminogen activator in the blood and a decrease in the level of its inhibitor.

The solution for injection has an anticoagulant effect, which is not significantly expressed at the recommended dosage (1 ampoule per day) and is explained by the presence of an affinity for the cofactor II of heparin, which consistently reduces the concentration of factor X and thrombin, and antithrombin.

Antithrombotic activity is due to all types of action that characterize sulodexide: effect on the walls of blood vessels (angioprotective effect), blood coagulation (slightly pronounced anticoagulant effect), fibrinolysis (profibrinolytic effect) and inhibition of platelet adhesion.

Pharmacokinetics

Sulodexide is absorbed in the small intestine. After oral administration of the labeled drug, the first peak concentration of the active substance in the blood plasma is determined after 2 hours, and the second peak concentration is determined after 4–6 hours, after which sulodexide is no longer detected in the plasma. The concentration of the active ingredient is restored after about 12 hours, and then stably remains at the same level until about 48 hours. The constant concentration of sulodexide in blood plasma is determined 12 hours after ingestion, which is presumably due to its slow release from the absorption organs, including the vascular endothelium.

When administered intravenously or intramuscularly, sulodexide is absorbed rather quickly, and the rate of absorption is determined by the rate of circulation at the injection site. Its plasma content after a single intravenous bolus injection of Wessel Douet F at a dose of 50 mg after 15, 30 and 60 minutes was 3.86 ± 0.37 mg / l, 1.87 ± 0.39 mg / l and 0.98 ± 0.09 mg / L, respectively.

The substance is distributed in the vascular endothelium, and its content is 20-30 times higher than that in other tissues.

Sulodexide is metabolized in the liver and excreted mainly in the urine. Studies that used a radioactive labeled preparation confirmed that 55.23% of the active substance was excreted through the kidneys during the first 4 days.

Indications for use

  • Cerebral circulation disorders, including periods of early recovery and acute periods of ischemic stroke;
  • Angiopathies with an increased risk of thrombus formation, including conditions after myocardial infarction;
  • Vascular dementia;
  • Dyscirculatory encephalopathy due to hypertension, diabetes mellitus, atherosclerosis;
  • Deep vein thrombosis, phlebopathy;
  • Occlusive lesions of peripheral arteries of diabetic and atherosclerotic genesis;
  • Antiphospholipid syndrome, thrombophilic conditions (simultaneously with acetylsalicylic acid, as well as following low molecular weight heparins);
  • Microangiopathy (neuropathy, retinopathy, nephropathy) and macroangiopathy (cardiopathy, encephalopathy, diabetic foot syndrome) in diabetes mellitus;
  • HTT therapy is heparin-induced thrombotic thrombocytopenia (the drug does not cause HTT and does not aggravate the course of the disease).

Contraindications

  • Diseases and hemorrhagic diathesis accompanied by low blood clotting;
  • I trimester of pregnancy;
  • Hypersensitivity to drug components.

Instructions for use of Wessel Douai F: method and dosage

Wessel Douai F can be administered intramuscularly or intravenously (after dilution in saline in a volume of 150-200 ml), and also taken orally (between meals).

At the beginning of therapy for 15-20 days, the drug is used intramuscularly in 1 ampoule daily, then for 30-40 days it is taken 2 times a day, 1 capsule.

The full course must be repeated at least 2 times a year. Depending on the indications and response to therapy, the doctor may change the dosage regimen.

Side effects

During therapy, development is possible:

  • Allergic reactions: rash;
  • Organs of the gastrointestinal tract: epigastric pain, vomiting, nausea;
  • Others: burning, pain and hematoma at the injection site.

Overdose

The only symptom of an overdose by Wessel Douai F is bleeding. In this case, it is recommended to immediately administer a 1% solution of protamine sulfate, which is used for bleeding caused by heparin.

There is a positive experience of using Wessel Douai F for the prevention and treatment of vascular complications in women with type 1 diabetes in the II-III trimesters of pregnancy, as well as in the development of gestosis (late toxicosis of pregnant women).

  • Antithrombin III content (norm - 210-300 mg / l);

Application during pregnancy and lactation
