Pharmacological properties
Pharmacodynamics
Bifosin is one of the external broad-spectrum antifungal drugs.
The active ingredient of the drug, bifonazole, is an imidazole derivative. Shows a fungicidal effect against dermatophytes (Microsporum, Trichophyton, Epidermophyton), fungistatic - against yeast-like and mold fungi (Scopulariopsis brevicaulis, Aspergillus ferrus), Malassezia furfur.
Shows activity against bacteria: gram-positive bacillus Corynebacterium minutissimum (with a minimum inhibitory concentration in the range of 0.000 5-0.002 mg / ml) and against gram-positive cocci (with a minimum inhibitory concentration of 0.004-0.016 mg / ml), except for Enterococcus spp.
The main target of Bifosin is ergosterol, which is an essential component of the fungal membrane. The drug suppresses its synthesis at two stages of formation. Due to this, structural and functional damage to the cytoplasmic membrane of the fungi occurs.
The minimum effective concentration of bifonazole is 5 ng / ml with a duration of exposure of at least 6 hours. The concentration of 3 ng / ml allows to inhibit the growth of the rapidly proliferating mycelium of Trichophyton mentagrophytes.
It has a fungistatic effect on yeast-like fungi of the genus Candida, in concentrations from 20 ng / ml turning into fungicidal.
Pharmacokinetics
Bifonazole penetrates well into the affected skin layers. The absorption of the substance is 0.6–0.8%, the plasma concentration is not determined. The absorption of bifonazole when the cream is applied to the affected skin areas is 2–4%, the plasma concentration is 2 ng / ml.
6 hours after the application of the agent, the concentration of the substance in the skin reaches or many times exceeds the minimum effective for the main fungi that lead to the development of dermatomycosis: 1 mg / cm 2 in the upper layer of the epidermis (Stratum corneum) and 5 mg / cm 2 in the one located directly under the epidermis papillary layer (Stratum papillare).
The duration of the substance in the skin is from 36 to 48 hours (when applying the cream - from 48 to 72 hours).
T 1/2 (half-life) from the skin - 19–32 hours (the value of the indicator depends on the density of the skin).
Indications for use
The use of Bifosin is advisable in the treatment of skin lesions caused by dermatophytes, moldy, yeast-like fungi, Corynebacterium minutissimum and Malassezia furfur:
