Pharmacological properties
Pharmacodynamics
Bromhexine is a prodrug that is converted to ambroxol in the body. The drug enhances the activity of goblet cell lysosomes, which are part of the epithelium of the respiratory tract. This leads to the release of lysosomal enzymes responsible for the hydrolysis of mucoproteins and mucopolysaccharides. Bromhexine hydrochloride activates the production of surfactant and neutral polysaccharides, and also normalizes mucociliary clearance. This compound provides liquefaction of bronchial secretions of increased viscosity and stickiness and its removal from the bronchi. Bromhexine is characterized by antitussive action.
Pharmacokinetics
Bromhexine crosses the placental and blood-brain barriers. It binds to plasma proteins by 99%. There is also a "first pass" effect through the liver, in which the active component of the drug is biotransformed to form the active metabolite, ambroxol. The therapeutic effect becomes noticeable after 20-30 minutes after ingestion. The half-life is 6.5 hours, increasing in chronic renal failure. Bromhexine is excreted through the kidneys in the form of metabolites by about 85-90% and tends to accumulate in the body.
Indications for use
The use of Bromhexine is advisable in the treatment of respiratory diseases, the course of which is associated with the formation of a viscous secretion that is difficult to separate:
- Chronic bronchitis with a broncho-obstructive component;
- Tracheobronchitis;
- Bronchial asthma;
- Chronic pneumonia;
- Cystic fibrosis.
Contraindications
A contraindication to the use of the drug is the patient's hypersensitivity to bromhexine.
Instructions for the use of Bromhexine: method and dosage
Bromhexine solution, tablets and syrup are taken orally.
The dosage is determined based on the patient's age:
- Children under 2 years of age are advised to take 2 mg 3 times a day;
- Children from 2 to 6 years old - 4 mg 3 times a day;
- Children from 6 to 10 years old - 6-8 mg 3 times a day;
- Children over 10 years old and adults - 8 mg 3 times a day;
- If necessary, the dose can be increased: for adults - up to 16 mg 4 times a day, for children - up to 16 mg 2 times a day.
Application of the drug in the form of inhalation:
- Children under 6 years of age - up to 2 mg 2 times a day;
- Children from 6 to 10 years old - 2 mg 2 times a day;
- Children over 10 years old - 4 mg 2 times a day;
- Adults - 8 mg 2 times a day.
The therapeutic effect usually appears on the 4-6th day of application. The duration of treatment is 4 to 28 days.
Parenteral administration is used in severe cases and in the postoperative period, in order to prevent the accumulation of thick sputum in the bronchi. The drug is administered (intravenously or intramuscularly) 2-3 times a day, 2 mg slowly over 3 minutes.
Side effects
- From the side of the central nervous system: dizziness and headaches;
- From the respiratory system: bronchospasm, cough;
- On the part of the digestive system: a transient increase in the activity of hepatic transaminases in the blood serum, dyspeptic manifestations;
- Dermatological reactions: skin rash, excessive sweating.
Overdose
When too high doses of Bromhexine are taken, symptoms such as diarrhea, dyspepsia, nausea, and vomiting may occur. In this case, the appointment of symptomatic treatment is recommended.

When treating patients suffering from gastric ulcer, as well as those with a history of gastric bleeding, the use of the drug should be carried out under the supervision of a physician.
Patients diagnosed with bronchial asthma are prescribed Bromhexine with caution.
