Acriol Pro is a local anesthetic drug for superficial anesthesia of the skin and mucous membranes of the genital organs.

Release form and composition

The drug is produced in the form of a cream for local and external use, which is a white or almost poor homogeneous mass, sometimes with a weak specific odor (5, 30 or 100 g in aluminum tubes, 1 tube in a cardboard box and instructions for use of Acriol Pro).

Composition for 100 g of cream:

  • active substances: prilocaine - 2.5 mg; lidocaine - 2.5 g;
  • auxiliary components: sodium hydroxide, carbomer, PEG-54 hydrogenated castor oil, purified water.

Pharmacological properties

Pharmacodynamics

The active ingredients of the drug, lidocaine and prilocaine, are amide-type local anesthetics. They penetrate the layers of the epidermis and dermis, causing skin anesthesia. The degree of local anesthesia depends on the dose of Acriol Pro and the duration of the cream application.

After applying the drug to intact (intact) skin under an occlusive dressing for 1-2 hours, the duration of anesthesia is 2 hours. The cream's safety and efficacy were the same for the elderly (65-96 years old) and for younger people.

Since Acriol Pro acts on superficial vessels, temporary redness or blanching of the skin is possible. Such reactions occur more quickly in patients with atopic dermatitis. They can appear within 30-60 minutes after applying the cream, which indicates a faster penetration of the drug through the skin.

When taking material by puncture biopsy (4 mm in diameter) in 90% of patients, adequate anesthesia of intact skin with the introduction of a needle to a depth of 2 mm is provided in 60 minutes after applying the cream, and when a needle is inserted to a depth of 3 mm - in 120 minutes. The effectiveness of Acriola Pro does not depend on pigmentation or skin color.

When using combined vaccines against rubella, measles and mumps, when vaccinating against hepatitis B, as well as when administering intramuscular combination vaccines against whooping cough, poliomyelitis, diphtheria, tetanus and Haemophilus influenzae type b infection, the use of Acriol Pro did not affect the rate of disappearance or the appearance of specific antibodies in serum, the average antibody titer, or the number of patients who have achieved a positive or protective antibody titer after vaccination.

After applying the cream to the mucous membrane of the genital organs, the analgesic effect is achieved in a shorter period of time, since the absorption of the drug is faster. In women, within 5-10 minutes after applying Acriol Pro to the genital mucosa, anesthesia is achieved, which is sufficient to relieve pain caused by the use of an argon laser. The total duration of such anesthesia varies from 5 to 45 minutes (on average, 15–20 minutes).

When treating trophic ulcers of the lower extremities, the duration of anesthesia is up to 4 hours. The cream does not affect the healing process of ulcers or the bacterial flora.

Pharmacokinetics

Systemic absorption of Acriol Pro depends on several factors: the dose applied, the thickness of the skin at the site of application, the duration of application, the presence or absence of skin diseases and shaving. When using the cream on the ulcerative surfaces of the lower extremities, the absorption of the drug is also affected by the characteristics of the ulcers themselves, for example, their size (if the ulcers are large, the degree of absorption is higher).

In an adult, after applying 60 g of the drug to intact thigh skin (at the rate of 1.5 g of cream per 10 cm 2), about 3% of lidocaine and 5% of prilocaine were absorbed into the systemic circulation for 3 hours.

The absorption is slow. In blood plasma, the maximum concentrations of lidocaine and prilocaine were noted 4 hours after the application of the cream. When Acriol Pro was applied to intact skin 8–12 hours after shaving, the maximum plasma concentrations of lidocaine and prilocaine were significantly lower than the toxic level in both young and elderly patients.

When the drug was applied to trophic ulcers of the lower extremities (at the rate of 5–10 g for 30 min), the maximum plasma concentrations of the active substances were reached faster - after 1–2.5 hours. There was no accumulation of lidocaine, prilocaine and their metabolites even in the case of repeated use.

After applying the cream to the vaginal mucosa (in an amount of 10 g for 10 minutes), the maximum plasma concentrations of lidocaine and prilocaine were reached approximately 35 minutes after the start of application.

Indications for use

Acriol Pro cream is used in adults for the following indications:

  • superficial anesthesia of the skin during punctures, injections (including vaccination), vascular catheterization, as well as superficial surgical procedures, including epilation and minor cosmetic procedures;
  • superficial anesthesia of the mucous membranes of the genital organs before injections of local anesthetics and painful procedures or manipulations;
  • superficial anesthesia of trophic ulcers of the lower extremities during mechanical cleaning (to remove necrotic tissues, fibrin and pus).

In children, Acriol Pro is used for superficial anesthesia of the skin during punctures, injections (including vaccination), vascular catheterization, as well as superficial surgical procedures (for example, when removing molluscum contagiosum).

Contraindications

Acriol Pro should not be used in premature infants (who were born before 37 weeks of gestation), newborns weighing up to 3 kg, as well as in patients with hypersensitivity to local anesthetics of the amide type or auxiliary components of the cream.

The drug is prescribed with caution in atopic dermatitis (widespread neurodermatitis), idiopathic or hereditary methemoglobinemia, glucose-6-phosphate dehydrogenase deficiency, as well as in patients taking class III antiarrhythmic drugs (for example, amiodarone).

Acriol Pro, instructions for use: method and dosage

Acriol Pro cream is intended for external (local) use. The drug is applied to the skin or mucous membranes.

Superficial anesthesia of intact skin

The dose, method of application and time of application depend on the indication for which Acriol Pro is prescribed:

  • needle insertion (for example, vascular catheterization during blood sampling): the cream is applied in a thick layer and covered with an airtight bandage; the recommended dose is about 2 g per 10 cm 2; the duration of the application is 1 hour (the maximum application time is 5 hours);
  • minor surgical operations (removal of warts, curettage of molluscum contagiosum, epilation, minor cosmetic procedures): the cream is applied in a thick layer and covered with an occlusive dressing, the recommended dose is 1.5–2 g per 10 cm 2; application time is 1 hour (maximum 5 hours);
  • anesthesia of large areas of freshly shaved skin (in outpatient treatment), including before the epilation procedure: Acriol Pro is applied in a thick layer and covered with a sealed bandage, the maximum dose is 60 g, and the maximum application area is 600 cm 2; duration of application 1 hour (maximum 5 hours);
  • anesthesia of large areas during superficial procedures (in a hospital), for example, when taking the skin using the split flap method: the cream is applied in a thick layer and covered with an airtight bandage, the recommended dose is 1.5–2 g per 10 cm 2; duration of application 2 hours (maximum 5 hours).

Superficial anesthesia of trophic ulcers of the lower extremities

With mechanical cleaning of trophic ulcers, a single dose of the drug is about 1-2 g per 10 cm 2. The cream is applied in a thick layer, an occlusive dressing is applied on top. The consumption of the cream for one procedure should not exceed 10 g. The minimum application time is 30 minutes.

The opened tube with Acriol Pro is intended for single use. The remains of the cream should be discarded after use on one patient.

When treating ulcers, in the tissues of which the drug penetrates poorly, the application time can be increased to 60 minutes. Surgical treatment of ulcers should begin no later than 10 minutes after removing the dressing with cream.

The manipulations with the cream on trophic ulcers of the lower extremities were performed up to 15 times within 1-2 months, while the effectiveness of Acriol Pro did not decrease, and the frequency of local reactions did not increase.

Superficial anesthesia of the genitals in men and women

Before injections of local anesthetics, Acriol Pro is applied in a thick layer to the skin of the genitals. For men, the dose of the drug is 1 g per 10 cm 2, and the duration of the application is 15 minutes; for women - 1–2 g per 10 cm 2, and the application time is 60 min.

When anesthetizing the mucous membranes of the genital organs (for example, before injections of local anesthetics or before removing genital warts), the recommended dose is 5 to 10 g and depends on the area of the treated surface. Acriol Pro is applied to the entire surface of the mucous membrane, including the folds. No sealed bandage required. The duration of the application is 5-10 minutes. The procedure should be carried out immediately after removing the cream.

Recommended doses for children

In children, Acriol Pro is used according to indications. The cream is applied in a thick layer and covered with an airtight bandage. The dose of the drug should correspond to the area of the treated surface and not exceed 1 g per 10 cm 2.

The area of application and the duration of the application depend on the age of the child:

  • 0–3 months: 1 g per 10 cm 2 (maximum skin area allowed for processing), application time - 1 hour (strictly no more than 1 hour);
  • 3–12 months: 2 g per 20 cm 2 (maximum), application time - 1 hour;
  • 1–6 years: 10 g per 100 cm 2 (maximum), application time - 1 hour (total not more than 4 hours);
  • 6–12 years: 20 g per 200 cm 2 (maximum), application time - 1 hour (total not more than 4 hours).

A 3.5 cm strip of cream corresponds to a dose of 1 g. With an increase in the application time, the analgesic effect decreases.

In atopic dermatitis, it is necessary to reduce the duration of the application.

Recommendations for applying Acriola Pro

To open the protective membrane on the aluminum tube, use the screw cap. After opening the tube, it is necessary to squeeze out a sufficient amount of cream, and then apply it to the site of the proposed procedure.

Occlusive stickers can be used on intact skin. When anesthetizing trophic ulcers, use sealed polyvinyl chloride (PVC) bandages. The applied cream layer is closed so that the layer is thick and does not squeeze out. Then the edges of the bandage are gently smoothed out, avoiding cream dripping. After the end of the application time, the bandage is removed, and the remains of the cream are removed from the surface.

Side effects

Possible side effects when applying Acriol Pro to intact skin:

  • skin and subcutaneous fat: often - transient local reactions in the area of application (redness, pallor, swelling); infrequently - itching, burning, feeling of warmth (at the first moment after application to the skin);
  • general disorders: rarely - cyanosis and / or methemoglobinemia; allergic reactions (including anaphylactic shock); pinpoint hemorrhages or hemorrhagic rash (especially in children with molluscum contagiosum and atopic dermatitis after prolonged application); irritation of the cornea of the eye (if the cream accidentally gets into the eyes).

Possible undesirable effects when applying Acriol Pro on trophic ulcers of the lower extremities:

  • skin and subcutaneous fat: often - transient reactions at the site of application of the cream (redness, pallor, swelling), a feeling of warmth, itching and slight burning (at the first moment after application); infrequently - skin irritation;
  • general disorders: rarely - allergic reactions (up to anaphylactic shock).

Overdose

If the dosage regimen is observed, an overdose of Acriol Pro is unlikely.

Possible symptoms of intoxication include excitation of the central nervous system, and in severe cases, depression of the heart and nervous system. In addition, it is possible to develop a clinically significant increase in the level of methemoglobin in the blood.

If severe neurological symptoms (seizures, depression of the central nervous system) occur, symptomatic therapy is required, including the appointment of anticonvulsants and mechanical ventilation (if necessary). Patients with methemoglobinemia are injected with methylene blue (it is an antidote). The patient should be monitored for several hours after intoxication, since the systemic absorption of Acriol Pro is rather slow.

In newborns, when taking blood samples from the heel, the effectiveness of Acriol Pro has not been established.

Care should be taken when applying the cream around the eyes, as the drug can irritate and damage the cornea. In case of contact with eyes, rinse immediately with isotonic sodium chloride solution or water.

In infants under the age of 3 months after the application of Acriol Pro, a temporary (up to 13 hours) increase in the concentration of methemoglobin in the blood was quite often observed, but this fact, most likely, has no clinical significance.

With the simultaneous use of class III antiarrhythmic drugs, it is necessary to provide continuous ECG monitoring to assess the state of cardiac activity.

Acriol Pro should not be applied to a damaged eardrum and generally avoid getting the cream into the middle ear.

You can not apply the drug to open wounds, and in children and to the mucous membranes of the genital organs (since there is not enough data on the systemic absorption of the cream when applied to the mucous membranes of the genital organs in children).

Prilocaine and lidocaine at concentrations above 0.5–2% can have antiviral and bactericidal effects, therefore, when applying the drug immediately before the subcutaneous administration of live vaccines, it is recommended to take special care (for example, when staging BCG).

In children under 12 years of age, Acriol Pro is not recommended to be used in conjunction with drugs that cause methemoglobinemia.

Influence on the ability to drive vehicles and complex mechanisms

Acriol Pro does not affect the patient's psychomotor abilities.

Application during pregnancy and lactation

Lidocaine and prilocaine pass through the placental barrier and can be absorbed into the fetal tissue, however, animal studies have not revealed any negative effects of the drug on fetal development, pregnancy, postnatal development or the process of childbirth.

The active ingredients of Acriola Pro are secreted in breast milk in quantities that do not pose a risk to the baby.

Pediatric use

Acriol Pro is contraindicated in premature infants who were born before the 37th week of pregnancy, as well as in newborns weighing less than 3 kg.

Drug interactions

In case of combined use with drugs that cause methemoglobinemia, an increase in the level of methemoglobin in the blood is possible.

Other local anesthetics and drugs similar in structure to lidocaine and prilocaine (eg tocainide) may increase the systemic effects of Acriol Pro when used in high doses.

Along with the cream, class III antiarrhythmic drugs are used with caution.

Beta-blockers, cimetidine and other drugs that reduce the clearance of lidocaine can lead to an increase in the plasma concentration of lidocaine to toxic values. However, this occurs only with repeated use of large doses of lidocaine for a long time, and therapy with Acriol Pro is short-term.
