Release form and composition
The drug is produced in the form of film-coated tablets: biconvex, oblong, with a risk on one side, from cream to white (7 or 20 pieces in a blister, 1 blister in a cardboard box and instructions for using Cetirizine-Akrikhin).
1 tablet contains:
- active substance: cetirizine dihydrochloride - 10 mg;
- additional components: microcrystalline cellulose - 39 mg; lactose monohydrate - 31 mg; corn starch - 20 mg; povidone - 4 mg; magnesium stearate - 2.5 mg; sodium carboxymethyl starch - 2 mg; silicon dioxide - 1.2 mg; sodium lauryl sulfate - 0.3 mg;
- film shell: hypromellose - 3.4 mg; macrogol 6000 - 0.6 mg.
Pharmacological properties
Pharmacodynamics
Cetirizine-Akrikhin belongs to the group of blockers of peripheral histamine H 1 receptors (antiallergic drugs).
The active ingredient of the drug, being a competitive antagonist of histamine, has practically no m-anticholinergic and antiserotonin effects. The tool prevents the occurrence and facilitates the course of allergic reactions, demonstrates anti-exudative and antipruritic effects. Cetirizine affects the early histamine-dependent phase of allergic reactions, reduces the movement of inflammatory cells. Prevents the release of inflammatory mediators involved in the late stage of an allergic reaction. Cetirizine-Akrikhin provides a decrease in capillary permeability, relieves spasm of smooth muscles, prevents the development of tissue edema.
When using cetirizine in therapeutic doses, in most cases, the sedative effect is not manifested. In the case of course treatment, the development of tolerance to the antiallergic action of the drug is not observed. After taking Cetirizine-Akrikhin, the onset of its therapeutic effect in 50% of patients is noted after 20 minutes, in 95% of patients - after 60 minutes, it lasts for 24 hours.
Pharmacokinetics
When administered orally, the active substance is intensively absorbed from the gastrointestinal tract (GIT). In blood plasma, the maximum concentration (C max) is recorded after approximately 30-60 minutes. Food intake leads to a slight decrease in the rate of absorption of cetirizine, but does not significantly affect the amount of absorption.
It binds to plasma proteins by an average of 93%, the volume of distribution (V d) of cetirizine is approximately 0.5 l / kg. The drug does not pass through the blood-brain barrier (BBB) and the cell membrane. When used in a dose of 10 mg for 10 days, the agent does not accumulate.
In the liver, cetirizine in small amounts undergoes metabolic transformation with the formation of an inactive metabolite. Approximately 70% of the administered dose is eliminated by the kidneys, mainly unchanged. The half-life (T 1/2) after a single dose of the drug in a single dose is on average 10 hours.
In children 2–12 years old, T 1/2 of cetirizine decreases to 5–6 hours.
In persons with impaired renal function [with creatinine clearance (CC) less than 11-31 ml / min] and in patients receiving hemodialysis treatment (with CC below 7 ml / min), T 1/2 of the drug increases by 3 times, clearance decreases by 70%.
In the elderly and in patients with chronic diseases, there is an increase in T 1/2 of cetirizine by 50% and a decrease in clearance by 40%.
Indications for use
- pollinosis (hay fever);
- perennial and seasonal allergic rhinitis;
- urticaria, including chronic idiopathic;
- allergic conjunctivitis;
- itchy allergic dermatoses, including neurodermatitis and atopic dermatitis;
- Quincke's edema.
Contraindications
Absolute:
- glucose-galactose malabsorption, lactase deficiency, lactose intolerance;
- pregnancy and lactation;
- age up to 6 years;
- hypersensitivity to any component of the drug.
Cetirizine-Akrikhin should be used with extreme caution in patients with chronic renal failure (CRF) of moderate / severe severity (dosage adjustment is necessary).
Cetirizine-Akrikhin, instructions for use: method and dosage
Cetirizine-Akrikhin tablets are taken orally, preferably in the evening, regardless of food intake. They should be swallowed whole, without chewing or crushing, with a little water.
Recommended dosage regimen:
- children 6–12 years old: 2 times a day, ½ tablet (5 mg);
- adolescents over 12 years old and adults: 1 tablet (10 mg) once a day.
Patients with chronic renal failure need correction of the dosage regimen (the recommended dose should be reduced by 2 times).
Side effects
As a rule, Cetirizine-Akrikhin is well tolerated. In some cases, the development of the following adverse events is possible, in the event of which the therapy with the drug should be urgently stopped and immediately consult a doctor.
- nervous system: dizziness, drowsiness / insomnia, headache, agitation, dyskinesia, dystonia, depression, aggression, tics, tremors, paresthesias, fainting, convulsions, confusion, hallucinations;
- hematopoietic organs: thrombocytopenia;
- sense organs: nystagmus, impaired visual perception, impaired accommodation;
- respiratory system: pharyngitis, rhinitis;
- cardiovascular system: tachycardia;
- genitourinary system: urinary incontinence, urinary disorder;
- digestive system: nausea, dryness of the oral mucosa, diarrhea, abdominal pain; functional liver disorders - increased activity of hepatic transaminases, gamma-glutamyl transferase (GGT), alkaline phosphatase (ALP), an increase in the concentration of bilirubin;
- skin and subcutaneous tissue: rash, itching, urticaria, angioedema;
- metabolism: increased body weight;
- allergic reactions: hypersensitivity, including the development of anaphylactic shock;
- others: malaise, fatigue, asthenia, edema.
Overdose
Symptoms of an overdose of Cetirizine-Akrikhin when taken at a dose of 50 mg may include the following disorders: malaise, weakness, fatigue, itching, headache, dizziness, drowsiness, sedation, stupor, mydriasis, tremor, urinary retention, diarrhea, confusion. In children, when using excessively high doses, there may be increased irritability and anxiety, signs of anticholinergic action - dry mouth, urinary retention, constipation.
Treatment: provoking vomiting or gastric lavage, taking activated charcoal, supportive and symptomatic therapy. Hemodialysis is ineffective, and the specific antidote is unknown.

Although until now there is no information on the interaction of the drug with ethanol, it is not recommended to consume alcoholic beverages during therapy with cetirizine.
Influence on the ability to drive vehicles and complex mechanisms
When driving vehicles and carrying out other activities that require quick reactions and increased concentration of attention, care should be taken during the period of application of Cetirizine-Akrikhin.
Application during pregnancy and lactation
Pregnant women are not allowed to use Cetirizine-Akrikhin.
If drug treatment is necessary during lactation, breastfeeding should be discontinued.
Pediatric use
For children under 6 years of age, therapy with Cetirizine-Akrikhin tablets is contraindicated.
With impaired renal function
With extreme caution, Cetirizine-Akrikhin should be used in patients with chronic renal failure (CRF) of moderate / severe severity. Such patients need correction of the dosage regimen - the recommended dose should be reduced by 2 times.
For violations of liver function
Patients with impaired liver function need an individual selection of the dose of the drug, especially against the background of concomitant renal failure.
Use in the elderly
Elderly patients in the presence of normal renal activity do not require changes in the dose of Cetirizine-Akrikhin.
Drug interactions
- myelotoxic drugs: the manifestations of cetirizine hematoxicity are aggravated;
- theophylline (in a daily dose of 400 mg): the total clearance of cetirizine decreases by 16%, while the kinetics of theophylline is not affected.
It is not recommended to combine antiallergic agent with other drugs without a doctor's prescription.
