Infezol 100 is a drug for parenteral nutrition that compensates for the deficiency of electrolytes, amino acids and carbohydrates.

Release form and composition

The drug is produced in the form of a solution for infusion, which is a transparent liquid, almost colorless or with a slight yellowish tinge, with a specific odor (250 ml or 500 ml each in glass vials, sealed with rubber stoppers for piercing and closed with plastic lids under aluminum rolling; in a cardboard box of 10 bottles and instructions for use of Infezol 100).

The composition of the active ingredients (per 1 liter of solution for infusion):

  • L-alanine - 15.5 g;
  • lysine monoacetate - 10.02 g (corresponds to 7.1 g of L-lysine);

All amino acids are essential components for protein synthesis. Infusion solutions, which include amino acids, including Infezol 100, are administered as parenteral nutrition simultaneously with carbohydrates, fats, fluids and electrolytes. The plasma concentration of free amino acids is highly variable. This applies to both individual organic compounds and their total concentration.

The drug is intended for intravenous administration, so its bioavailability is 100%.

Pharmacokinetics

The introduced amino acids are used as a substrate for protein synthesis. If some of the amino acids are not consumed for the synthesis of proteins, then they are simply excreted from the body unchanged (about 5%) or undergo further breakdown (in contrast to glucose and fatty acids, which can be reserved in certain stores).

Amino acids are broken down by deamination of the α-amino group, which is metabolized to urea and then excreted through the kidneys. The carbon skeleton of amino acids enters the Krebs cycle, where it is converted first into acetyl-CoA, then into acetoacetyl-CoA, and finally into pyruvate.

Indications for use

  • parenteral nutrition (if enteral nutrition is impossible);
  • deficiency of proteins of various origins: during operations, burns, blood loss, etc. (treatment and prevention);
  • replenishment of fluid loss.

Contraindications

Absolute:

  • acute left ventricular failure;
  • hemodynamic disorders in the acute stage (including shock);
  • total tissue hypoxia;
  • severe liver failure;
  • traumatic brain injury;
  • poisoning with methanol;
  • children under 2 years of age.

Relative (Infezol 100 is used with caution, after assessing the expected benefit and possible risk):

  • chronic heart failure (I – IIA degree);
  • kidney disease;
  • metabolic disorders of amino acids;
  • a shift in the acid-base balance of the body towards an increase in acidity (acidosis);
  • hyponatremia, hyperkalemia;

Infezol 100, instructions for use: method and dosage

Solution Infezol 100 is intended for intravenous drip (slow). During the procedure, the patient must be closely monitored.

The dosage is determined specifically for each patient, taking into account his condition, as well as the need for fluids, amino acids and electrolytes. At the beginning of the infusion, the infusion rate should be slower within the first hour, it is brought to the desired value.

For adults, the drug is prescribed in a daily dose of 10–20 ml / kg of body weight, which corresponds to 1–2 g of amino acids per kg of body weight. The maximum dose is 20 ml / kg (2 g / kg) per day. For adult patients, the amount of fluid administered (within the framework of appropriate treatment) should not exceed 40 ml / kg of body weight per day.

When prescribing Infezol to 100 children, it is necessary to take into account the child's age, the level of his physical development and the severity of the disease. The average doses of solution for infusion are as follows:

  • children aged 2–6 years - 10 ml / kg of body weight (or 1 g of amino acids per kg of body weight) per day;
  • children and adolescents aged 6-14 years - 15 ml / kg of body weight (or 1.5 g of amino acids per kg of body weight) per day;
  • adolescents 14-18 years old - doses used in adults.

Infezol 100 solution is administered at a rate of not more than 1 ml / kg of body weight per hour (or 0.1 g of amino acids per kg of body weight per hour).

The maximum daily doses of the drug are calculated based on the patient's protein requirement.

The solution for infusion is used until the patient completely switches to the enteral route of nutrition.

Side effects

Side effects usually do not occur, since Infezol 100 is well tolerated. Sometimes allergic reactions (in the form of urticaria and other anaphylactic responses) may occur. Very rarely, as a rule, at an excessively high rate of administration of the solution, the patient develops nausea, vomiting, hyperhidrosis, tachycardia and chills.

Overdose

Overdose of Infezol 100 can lead to overhydration, electrolyte imbalance and amino acid intoxication.

With the rapid introduction or infusion of large doses of potassium-sparing solutions, hyperkalemia may occur. In these cases, the maximum serum potassium concentration exceeds 6.5 mmol / L. Signs of hyperkalemia include sensory disturbances, muscle weakness, and cardiac conduction and cardiac arrhythmias.
