Ultracaine D-S is an anesthetic with a vasoconstrictor component for local use in dentistry.

Release form and composition

Dosage form - solution for injection: colorless liquid with a transparent structure (2 ml in a glass ampoule without color, in a cardboard box 10 ampoules; 1.7 ml in a glass cartridge without color, 10 cartridges in a block made of corrugated cardboard, in a cardboard box 10 blocks).

1 ml of solution contains:

  • active ingredients: articaine hydrochloride - 40 mg, epinephrine hydrochloride - 0.006 mg (or 0.005 mg of epinephrine base, its content in the solution corresponds to 1: 200,000);
  • auxiliary components: sodium chloride, sodium metabisulfite (sodium disulfite), water for injection.

Indications for use

The use of Ultracaine D-S is indicated for infiltration and conduction anesthesia during dental operations:

  • uncomplicated extraction of one or more teeth;
  • pain relief of carious tooth cavities;
  • grinding teeth before prosthetics.

Contraindications

  • atrioventricular (AV) block II and III degree, bradycardia and other severe conduction and sinus node dysfunction;
  • acute decompensated heart failure;
  • tachyarrhythmia, paroxysmal tachycardia;
  • severe arterial hypertension or hypotension;
  • the use of non-cardioselective beta-blockers (including propranolol);
  • methemoglobinemia;
  • hypoxia;
  • angle-closure glaucoma;
  • hyperthyroidism;
  • pheochromocytoma;
  • anemia (including B 12 deficiency anemia);
  • age up to 4 years;
  • hypersensitivity to articaine and other local anesthetics of the amide type, epinephrine, sulfites (including manifestations in the form of bronchial asthma) and other auxiliary components of the drug.

Caution should be exercised in cases of bleeding disorders, severe agitation, angina pectoris, postinfarction cardiosclerosis, atherosclerosis, history of stroke, cerebrovascular accident, severe liver and / or kidney dysfunction, pulmonary emphysema, chronic bronchitis, cholinesterase deficiency, sugar diabetes mellitus.

Since articaine crosses the placenta, during pregnancy, the use of Ultracaine D-S is possible only in special cases, when the benefits of anesthesia for the mother outweigh the potential threat to the fetus.

Method of administration and dosage

The drug is used by injection into the tissues of the oral cavity, where there is no inflammation.

Do not inject into inflamed tissue!

The level of injection pressure should correspond to the level of tissue sensitivity.

Intravenous administration of the drug is contraindicated.

Before the direct administration of the solution, it is imperative to conduct an aspiration test, this will prevent accidental intravascular administration of the drug.

Recommended dosage:

  • anesthesia for uncomplicated tooth extraction in the upper jaw: 1.7 ml per 1 tooth. To create a depot in the area of the transitional fold, the injection should be made into the submucosa from the vestibular side. Additional administration of 1–1.7 ml of solution is allowed to achieve complete anesthesia without palatal injection. It is possible to simultaneously remove several teeth located nearby;
  • removal of premolars of the lower jaw: infiltration anesthesia - 1.7 ml per 1 tooth. In the absence of complete anesthesia, an additional 1–1.7 ml is injected into the submucosa from the vestibular side in the region of the transitional fold of the lower jaw. If additional measures did not give the desired effect, mandibular anesthesia is performed - conduction block of the mandibular nerve;
  • anesthesia for making incisions and suturing in the palate: 0.1 ml per 1 injection to create a palatine depot;
  • processing of cavities, grinding teeth under crowns (except for lower molars): 0.5–1.7 ml per 1 tooth (taking into account the complexity and duration of treatment) in the transitional fold region from the vestibular side.

The maximum dose of articaine for performing one treatment procedure in adult patients is at the rate of 7 mg per 1 kg of body weight.

Recommended dosage for children over 4 years of age: not more than 5 mg of articaine per 1 kg of body weight.

For patients with severe renal and / or hepatic insufficiency, in old age, the solution is used in the minimum effective dose that ensures the required depth of anesthesia is achieved.

Side effects

  • allergic reactions: possibly conjunctivitis, skin hyperemia, rhinitis, angioedema (urticaria, edema of the upper and / or lower lips, cheeks, edema of the vocal cords, difficulty breathing and swallowing), anaphylactic shock;
  • from the central nervous system: often - headache; possibly - paresthesia, dizziness, hypesthesia, visual disturbances of a transient nature (in the form of double vision, blurred vision, blindness that occurs during or after injection); sometimes - nerve damage (as a result - the development of facial nerve paralysis); dose-dependent reactions - stupor (sometimes - loss of consciousness), respiratory disorders (sometimes - respiratory arrest), muscle tremors, muscle twitching (sometimes - generalized convulsions);
  • on the part of the cardiovascular system: lowering blood pressure (BP), heart failure, bradycardia, shock; very rarely - heart rhythm disturbance, tachycardia, increased blood pressure;
  • from the digestive system: nausea, vomiting;
  • local reactions: inflammation or swelling at the injection site; with intravascular administration - ischemia at the injection site, tissue necrosis.

The solution should be taken from the ampoule only with a new sterile syringe and needle. Compliance with the rules of asepsis will prevent infectious infections, including viral hepatitis.

Do not use solution from a damaged cartridge.

An open cartridge can be used to anesthetize only one patient.

In patients with cholinesterase deficiency, the use of the solution is indicated only if absolutely necessary because of the possible excessively strong and prolonged action of the drug.

Before anesthesia, the doctor should be informed about the presence of diseases of the cardiovascular system, cerebrovascular disorders and other pathologies in the patient, since with angina pectoris, chronic heart failure, heart rhythm disturbances, myocardial infarction, arterial hypertension, pathology of coronary vessels, history of stroke, emphysema, chronic bronchitis, hyperthyroidism, diabetes mellitus, the presence of severe anxiety, it is advisable to use drugs that do not contain epinephrine, including Ultracain D.

You can take food with a full restoration of sensitivity in the oral cavity.

Ultracaine D-S usually does not affect the patient's ability to drive vehicles and mechanisms. However, the doctor must individually recommend to each patient when to start potentially hazardous activities, including driving.

Drug interactions

Due to the increased risk of developing severe bradycardia and hypertensive crisis, the use of the solution is contraindicated in patients taking propranolol or other cardio-selective beta-blockers.

With the simultaneous use of Ultracaine D-S:

  • tricyclic antidepressants, monoamine oxidase inhibitors can enhance the effect of epinephrine, causing an increase in blood pressure;
  • opioid analgesics enhance the effect of the drug, increase the risk of respiratory depression;
  • cholinesterase inhibitors slow down the metabolism of local anesthetics;
  • drugs that depress the central nervous system enhance their effect;
  • heparin, acetylsalicylic acid increase the risk of bleeding at the injection site;
  • muscle relaxants increase the intensity and duration of their action;
  • oral hypoglycemic agents reduce their effectiveness;
  • halothane is able to increase cardiac sensitivity to catecholamines and, as a consequence, the risk of developing cardiac arrhythmias;
  • local anesthetics slow down your metabolism;
  • Medicines for the treatment of myasthenia gravis reduce their effectiveness, especially when high doses of anesthesia are administered;
  • disinfectant solutions containing heavy metals when treating the injection site increase the risk of swelling and soreness.
