Release form and composition

Tizalud is available in the form of tablets: white with a yellow tint or white, round, biconvex, with a dividing line (2 mg or 4 mg each: 10 pcs. In blisters, 3 packs in a cardboard box; 30 pcs. In polymeric cans, in a cardboard box 1 can).

1 tablet contains:

  • active substance: tizanidine hydrochloride - 2.288 mg or 4.576 mg, which corresponds to the content of 2 mg or 4 mg of tizanidine;
  • auxiliary components: anhydrous lactose (lactopress), sodium carboxymethyl starch (primogel), microcrystalline cellulose, magnesium stearate.

Pharmacological properties

Pharmacodynamics

Tizalud is a centrally acting muscle relaxant with the active substance tizanidine. The clinical efficacy of tizanidine is due to its ability to exert a stimulating effect on presynaptic alpha 2 -adrenergic receptors, suppressing the release of excitatory amino acids from intermediate neurons of the spinal cord. As a result, in the spinal cord, the polysynaptic transmission of excitation is inhibited, which causes a decrease in muscle tone. Along with muscle relaxant properties, the drug has a central moderately pronounced analgesic effect.

Due to a decrease in spasticity and clonic seizures, resistance to passive movements decreases, and the ability to increase the volume of active movements increases. Shown for use in painful muscle spasms, chronic spasms of spinal and cerebral genesis.

Pharmacokinetics

After oral administration, tizanidine is rapidly absorbed. The maximum concentration (C max) in plasma is reached after 1–2 hours. The bioavailability of the drug is 34%. Simultaneous food intake does not affect the pharmacokinetics of tizanidine.

The volume of distribution is 2.6 l / kg.

Plasma protein binding is 30% of the dose taken.

The linear nature of the pharmacokinetics of the drug remains in the dose range from 4 mg to 20 mg.

Metabolism occurs in the liver quickly, up to 95% of tizanidine is exposed to it with the formation of inactive metabolites.

The half-life (T 1/2) is 2-4 hours.

Through the kidneys, 70% of the dose is excreted in the form of metabolites, unchanged - 2.7%.

In renal failure with creatinine clearance (CC) up to 25 ml / min, plasma C max of tizanidine increases 2 times, and the total concentration (AUC) - 6 times. There is a deceleration of T 1/2 to 14 hours.

Indications for use

  • muscle spasm caused by functional and static diseases of the cervical and lumbar spine, accompanied by pain syndrome;
  • painful muscle spasm that arose after surgery, including for a herniated disc, osteoarthritis of the hip joint;
  • spasticity of skeletal muscles in neurological diseases such as multiple sclerosis, chronic myelopathy, degenerative diseases of the spinal cord, infantile cerebral palsy in patients over 18 years of age, the consequences of cerebrovascular accident.

Contraindications

  • severe liver dysfunction;
  • the simultaneous use of fluvoxamine, ciprofloxacin and other strong inhibitors of the CYP1A2 isoenzyme;
  • hereditary lactose intolerance, lactase deficiency, glucose-galactose malabsorption syndrome;
  • age up to 18 years;
  • breast-feeding;
  • hypersensitivity to the components of the drug.

With caution, Tizalud tablets should be prescribed for arterial hypotension, bradycardia, renal failure, concomitant use of oral contraceptives, patients with congenital prolongation of the QT interval syndrome, patients over the age of 65 years.

Instructions for use Tizaluda: method and dosage

Tizalud tablets are taken orally.

Recommended dosage:

  • painful muscle spasm: 2-4 mg 3 times a day. If necessary, to relieve severe muscle spasm, you can additionally take 2-4 mg of the drug before bedtime;
  • treatment of muscle spasticity caused by neurological pathologies: the initial dose is 2 mg 3 times a day. With an interval of 3–7 days, the dose is gradually (by 2–4 mg) increased to an optimal daily dose of 12–24 mg, divided into 3–4 doses. The maximum daily dose is 36 mg.

The initial daily dose of Tizalud for patients with renal insufficiency with CC less than 25 ml / min should be 2 mg. Then, taking into account the effectiveness and tolerability of the drug, the dose can be gradually increased. To obtain a more pronounced effect, it is first recommended to use a single dose of an increased dose, and then proceed to increasing the frequency of administration.

Cancellation of Tizalud should be done by gradually reducing the dose of the drug, since a sharp cessation of its use can cause a withdrawal syndrome in the patient.

Side effects

  • from the nervous system: sleep disturbances, insomnia, drowsiness, hallucinations, dizziness;
  • from the musculoskeletal system: muscle weakness;
  • on the part of the cardiovascular system: bradycardia, lowering of blood pressure (including pronounced, up to loss of consciousness and collapse);
  • from the digestive system: dry mouth, dyspepsia, nausea, increased activity of liver enzymes, liver failure, hepatitis;
  • others: increased fatigue, hypercreatininemia; against the background of abrupt withdrawal after long-term treatment and / or the use of high doses of the drug, as well as after therapy in combination with antihypertensive drugs - increased blood pressure (BP), tachycardia, in some cases, acute cerebrovascular accident is possible.

Overdose

Symptoms: drowsiness, a significant decrease in blood pressure, nausea, vomiting, dizziness, anxiety, respiratory failure, miosis, coma.

Treatment: immediate gastric lavage, intake of activated charcoal, appointment of forced diuresis, symptomatic therapy.

The use of Tizaluda in a daily dose of 12 mg and above must be accompanied by regular monitoring of liver function (during the first 4 months of treatment - once a month). An examination of the functional state of the liver is also required for those patients who have such clinical signs of liver dysfunction as unexplained nausea, feeling of fatigue, anorexia. If the activity indices of ACT (aspartate aminotransferase) and ALT (alanine aminotransferase) consistently exceed 3 times or more the upper limit of the norm, the use of tizanidine should be canceled.

Tizalud should be used with caution in patients with congenital prolongation of the QT interval.

The use of alcohol and the ingestion of ethanol-containing drugs is contraindicated while taking tizanidine.

Influence on the ability to drive vehicles and complex mechanisms

According to the instructions, Tizalud can cause drowsiness, dizziness and have a negative effect on the speed of psychomotor reactions and the patient's ability to concentrate. In this regard, during the period of treatment, it is recommended to observe extreme caution when driving and working with complex mechanisms.

Application during pregnancy and lactation

It is not recommended to use Tizalud during gestation, unless, in the opinion of the doctor, the expected effect of treatment for the mother outweighs the possible risk to the fetus.

The appointment of tizanidine during lactation is contraindicated, since it is excreted in small quantities in breast milk. In case of clinical need for taking the drug, breastfeeding should be discontinued.

Pediatric use

Due to the limited experience with the use of tizanidine in children under 18 years of age, the use of Tizalud is not recommended for the treatment of patients of this age category.

With impaired renal function

With caution, Tizalud should be prescribed for renal failure.

For violations of liver function

The use of Tizalud is contraindicated in case of severe liver dysfunction.

Use in the elderly

Drug interactions

The concomitant use of Tizaluda with inhibitors of the CYP1A2 isoenzyme (including fluvoxamine, ciprofloxacin, amiodarone, mexiletine, propafenone, cimetidine, enoxacin, pefloxacin, norfloxacin, rofecoxib, ticlopidine, oral contraceptives) causes a significant increase in AUC. The consequence of this combination may be a condition that is accompanied by a clinically significant and prolonged decrease in blood pressure, drowsiness, delayed psychomotor reactions, and weakness. In some cases, there may be loss of consciousness, up to collapse.

When combined with antihypertensive drugs, the risk of a pronounced decrease in blood pressure and the development of bradycardia increases.

Sedative drugs, ethanol increase the sedative effect of tizanidine.
