Release form and composition

The composition of the drug for 1 dose:

  • active substance: nedocromil sodium (micronized, dried) - 2 mg (calculated on dry matter);
  • auxiliary components: hydrofluoroalkane (HFA-227), levomenthol, povidone K30, macrogol (polyethylene glycol) 600.

Pharmacological properties

Pharmacodynamics

The active substance of the Tayled Mint aerosol is nedocromil, an anti-allergic agent that, when used systemically, reduces the symptoms of allergic inflammation in the respiratory system.

Nedocromil is a stabilizer of mast cell membranes, inhibiting the early and late stages of an allergic reaction. The drug prevents the degranulation of mast cells. Prevents the release of eicosanoids from them and the release of inflammatory mediators (prostaglandin D2, leukotrienes, bradykinin, histamine, slow-reacting substance of anaphylaxis and other pro-inflammatory substances). Inhibits the release of cytokines from mast cells, alveolar macrophages and bronchial epithelium, chemotactic activity of neutrophils and eosinophils, as well as activation of sensory nerves in isolated bronchial muscle, leading to the development of bronchospasm.

In animal models, the drug suppressed citric acid-induced cough, antigen-induced bronchial hyperreactivity, airway edema, antigen-induced bronchospasm, and the late stage of an allergic reaction. Also inhibited bronchial hyperreactivity, developing under the influence of nonspecific factors such as sulfur dioxide, cold air, fog, adenosine, cigarette smoke and exercise.

In bronchial asthma, Tayled Mint also inhibits antigen-induced intermediate and late allergic reactions, reduces bronchial hyperreactivity.

As a result of its use after the development of an early allergic or asthmatic reaction, nedocromil suppresses not only bronchial hyperreactivity, but also a late allergic / asthmatic reaction.

Treatment with the drug significantly reduces the release of histamine into the lumen of the bronchi after provocation with a hyperosmotic solution of sodium chloride or an antigen.

In bronchial asthma, the anti-inflammatory effect of nedocromil is due to inhibition of the antigen-induced entry of eosinophils into the lumen of the bronchi and a decrease in the number of activated eosinophils in the submucous layer of the bronchi (after 16 weeks of regular use of Tiled Mint aerosol).

Due to the described properties, the drug prevents the development of bronchospasm after contact with an allergen or any provoking factor (physical stress, cold air, cigarette smoke, stress).

When applied topically at the level of the bronchial tree, Tayled Mint has an anti-inflammatory effect. Long-term continuous therapy can reduce bronchial hyperreactivity, improve respiratory function, and reduce the frequency and intensity of coughing and asthma attacks.

The greatest effectiveness of nedocromil is achieved with mild and moderate forms of bronchial asthma. Tiled Mint reduces nighttime symptoms and the need for daytime use of bronchodilator drugs.

The therapeutic effect of Tiled Mint develops by the end of the first week of its use.

Pharmacokinetics

After aerosol inhalation, approximately 10% of the dose enters the respiratory tract, the rest is deposited in the nasopharynx and oral cavity and is swallowed. No more than 2% of nedocromil is absorbed from the gastrointestinal tract, therefore, the drug determined in the plasma enters there mainly due to absorption from the respiratory tract.

The maximum plasma concentration (C max) is determined after about 60 minutes. The half-life (T 1/2) is from 1 to 2 hours. The connection with plasma proteins is noted at 89% and is reversible.

Nedocromil does not undergo biotransformation in the human body. It is excreted unchanged: about 70% of the dose - by the kidneys, about 30% - through the intestines.

Indications for use

The use of Tiled Mint aerosol is indicated for the following diseases / conditions:

  • bronchial asthma of various origins, including asthmatic bronchitis and asthma of physical exertion (treatment, including complex and prophylactic);
  • bronchospastic syndrome against a background of provoking factors such as allergens, atmospheric pollution or other irritants.

Tiled Mint is not intended to relieve an acute bronchospastic attack.

Contraindications

  • children under 2 years old;
  • hypersensitivity to any component of Tiled Mint.

Tiled Mint, instructions for use: method and dosage

Aerosol Tiled Mint is intended for inhalation use. Inhalation should be done daily.

Adults and children from 2 years of age are usually prescribed 2 inhalations (4 mg of nedocromil) 2-3 times a day. In severe cases of the disease and in cases of a high concentration of allergens, it is possible to increase the dose to 2 inhalations 4 times a day.

After achieving control over the symptoms of the disease, the dose is gradually reduced to a maintenance dose, which is 2 inhalations twice a day.

Two inhalations, carried out once a few minutes before the exposure of the provoking factor, for several hours provide protection against bronchospasm caused by inhaled allergens, cold air and any other irritants, as well as physical exertion (with exercise asthma).

With the simultaneous appointment of a bronchodilator, it should be used before the Tayled Mint aerosol.

Tiled Mint can be added to glucocorticosteroid (GCS) therapy to reduce their maintenance dose. In some cases, it is possible to completely reverse them. The GCS dose should be reduced gradually (by 10% weekly) under strict medical supervision. If a decrease in their dose is impossible, Tiled Mint should be continued until glucocorticosteroid therapy is fully restored.

The specifics of using the inhaler

Before using for the first time, shake the bottle and press the dosing valve 4 times to release the drug into the air (idle). With a break in the use of the inhaler for more than 3 days, to bring it into working condition, it is enough to make 2 sprays into the air.

Inhalation rules:

  1. Remove the protective cap. Inspect the outer and inner surfaces of the plastic case, make sure they are clean.
  2. Shake the inhalation device vigorously.
  3. Holding the inhaler vertically, place your thumb on the base of the can.
  4. Exhale as fully as possible, insert the mouthpiece between your teeth and clasp it tightly with your lips (you do not need to bite).
  5. Starting to inhale air through your mouth, press on the base of the can and continue to take a calm deep breath.
  6. Hold the breath. Remove the inhaler. Continue holding your breath for as long as possible.
  7. Close the inhaler with a cap.

For patients with impaired coordination and children, an aerosol holding chamber can be used to facilitate inhalation. A regular mouthpiece (not a synchronizer) is compatible with most aerosol retention chambers.

After inhalation, the plastic mouthpiece must always be covered with a dust cap. It must be kept clean to avoid the buildup of excess aerosol, which can block the inhaler. In this regard, at least once every 3 days, the mouthpiece should be washed and dried naturally overnight.

Inhaler cleaning rules:

  1. Remove the plastic protective cap and remove the cartridge from the mouthpiece.
  2. Rinse the mouthpiece through the top with hot tap water (approximately 45 ° C) for 1 min. Do the same on the other side of the mouthpiece. In case of severe clogging of the inhaler (when the aerosol outlet is blocked), remove the protective cap, disconnect the can and soak the mouthpiece for 20 minutes. in hot water.
  3. Shake off excess water by tapping the mouthpiece on a hard surface (the side that connects to the outlet of the metal can).
  4. Leave to dry overnight.
  5. Make sure the mouthpiece is completely dry.
  6. Attach it to a metal can and make sure the white rubber cover is pressed firmly.

It is forbidden to try to unlock the mouthpiece with any sharp object, it is fraught with breakage of the inhaler. Do not put a metal can in water. Do not remove the white rubber coating from the can.

Side effects

In most cases, the side effects listed below, the development of which is possible with the use of an aerosol, are mild and transient:

  • from the digestive system: unpleasant or unusual taste in the mouth, abdominal pain, dyspepsia, nausea, vomiting;
  • on the part of the respiratory system: dryness in the mouth, throat irritation, rhinitis, pharyngitis, hoarseness, cough, irritation and possible infection of the upper respiratory tract, short-term bronchospasm;
  • from the central nervous system: headaches, dizziness.

Overdose

Overdose cases have not been reported.

Animal studies have not revealed the toxic effects of nedocromil when used in high doses. Multiple studies in humans have not confirmed the adverse effects of the drug. Therefore, it is unlikely that any health problems will arise if the recommended dose of Tiled Mint is exceeded. However, if an overdose is suspected, symptomatic treatment is recommended to eliminate the symptoms that have arisen.

It is important to warn patients about the need for regular daily use of the drug, even in the absence of symptoms of the disease. Application during pregnancy and lactation

In animal studies, no adverse effects of nedocromil on the course of pregnancy, development of the fetus and calf were found.
