Tabex is a drug for the treatment of nicotine addiction.

Release form and composition

Dosage form Tabex - film-coated tablets: round, convex on both sides, light brown in color, at the break - from white to beige (50 pcs. In blisters nested in cardboard inserts with a calendar of admission, 2 blisters in a pack cardboard).

Active substance: cytisine, in 1 tablet - 1.5 mg.

Auxiliary components: lactose monohydrate, magnesium stearate, talc, microcrystalline cellulose.

Shell composition: opadry II brown 85F26948 [talc, macrogol 3350, titanium dioxide (E171), partially hydrolyzed polyvinyl alcohol, dyes of iron oxide red (E172), iron oxide yellow (E172) and iron oxide black (E172)].

Pharmacological properties

Pharmacodynamics

The active component of Tabex tablets is the alkaloid cytisine, which has a cholinomimetic effect. This compound increases blood pressure, activates the ganglia of the autonomic nervous system, promotes the production of adrenaline from the adrenal medulla, reflexively stimulates the respiratory center.

Despite the great similarity in the mechanisms of action of cytisine and nicotine, cytisine has a higher therapeutic index and is less toxic. As a competitor, cytisine inhibits the interaction of nicotine with receptors that are sensitive to it, which causes a gradual decrease and complete disappearance of nicotine (tobacco) addiction.

Pharmacokinetics

Pharmacokinetic studies of Tabex in humans have not been conducted to date. Animal experiments have shown that cytisine is absorbed from the gastrointestinal tract at a high rate, but only partially. The degree of resorption of the substance after oral administration in mice was 42%, and in rabbits - 34%. The maximum concentration of cytisine in the blood was reached in mice after 2 hours, and in rabbits - 35 minutes after ingestion. The drug was mainly accumulated in the adrenal glands, kidneys, and liver.

When administered intravenously (iv), the half-life of cytisine from the body of mice is 200 minutes. When Tabex is taken orally, 18% of the dose in mice is excreted within 24 hours. In rabbits, the half-life after IV administration is 37 minutes.

Indications for use

According to the instructions, Tabex is used to facilitate smoking cessation.

Contraindications

Absolute:

  • atherosclerosis;
  • Heart arythmy;
  • severe arterial hypertension;
  • unstable angina;
  • recently suffered a stroke (within 1 month before the start of drug use);
  • acute myocardial infarction;
  • lactase deficiency, galactosemia, glucose-galactose malabsorption syndrome;
  • age under 18 and over 65;
  • pregnancy and the period of breastfeeding;
  • hypersensitivity to any component of Tabex.

Carefully:

  • high blood pressure;
  • other forms of ischemic heart disease not specified in contraindications (asymptomatic myocardial ischemia, stable angina pectoris, microvascular angina pectoris, vasospastic angina pectoris);
  • heart failure;
  • obliterating arterial diseases;
  • cerebrovascular disease;
  • diabetes;
  • stomach ulcer;
  • gastroesophageal reflux disease;
  • liver failure;
  • renal failure;
  • the presence of chromaffin adrenal tumors;
  • hyperthyroidism;
  • some forms of schizophrenia;
  • long experience of smoking;
  • age over 40–45 years.

Instructions for the use of Tabex: method and dosage

Tabex tablets should be taken orally: swallow whole and drink them with a sufficient amount of liquid.

It is recommended to start treatment after the patient has made a firm decision to quit smoking.

In the first three days, 1 tablet is prescribed 6 times a day at 2-hour intervals with a gradual reduction in the number of smoked cigarettes. The tablets should be taken between smoking episodes in order to lengthen the interval between smoking cigarettes and to minimize their number. The intervals of 2 hours should be strictly observed.

If during this period it is not possible to reduce the daily consumption of cigarettes, the treatment should be interrupted for 2-3 months, and then start again.

In case of a good effect during the first three days, treatment is continued according to the following scheme:

  • days 4-12: 1 tab. 5 times a day at intervals of 2.5 hours;
  • days 13-16: 1 tab. 4 times a day at intervals of 3 hours;
  • days 17-20: 1 tab. 3 times a day at intervals of 5 hours;
  • days 21-25: 1 tab. 1-2 times a day.

It is strongly recommended to completely stop smoking during the first 5 days of taking Tabex.

Consultations, including those aimed at psychological support of the patient, help to increase the effectiveness of drug therapy.

Side effects

According to clinical studies and post-marketing observations, Tabex is generally well tolerated. If side effects occur, they are mild or moderate and are most often associated with cessation of smoking. Most of them occur in the first days of treatment and go away on their own.

Possible side effects:

  • from the central nervous system: headache, insomnia or drowsiness, dizziness, increased irritability;
  • from the gastrointestinal tract: changes in taste and appetite, dry mouth, nausea, diarrhea / constipation, abdominal pain;
  • from the respiratory system: shortness of breath;
  • on the part of metabolism and nutrition: increased sweating, weight loss;
  • on the part of the cardiovascular system: a slight increase in blood pressure, tachycardia, palpitations;
  • on the part of the musculoskeletal and connective tissue: muscle pain;
  • others: chest pain.

Overdose

Symptoms of an overdose of Tabex include: general weakness, dilated pupils, tachycardia, nausea, vomiting, clonic seizures, paralysis of the respiratory center. This condition occurs as a result of non-compliance by the patient with the dosage regimen and taking the drug in doses several times higher than the therapeutic ones.

In case of an overdose, an urgent specialist consultation is required. Also recommended is gastric lavage, intake of activated carbon, infusion of 5% or 10% glucose solution, as well as water-salt solutions. Respiratory analeptics, cardiotonics, anticonvulsants and other symptomatic therapy are often prescribed. Continuous monitoring of heart rate, blood pressure and respiratory function is required.

Drug interactions

After quitting smoking, the side effects of clozapine, olanzapine, ropinirole, theophylline may increase.

Pravastatin, lovastatin, fluvastatin, simvastatin increase the risk of muscle pain.

Tabex can reduce the effect of antihypertensive drugs.

With the simultaneous use of carbachol, acetylcholine, pyridostigmine, dystigmine, rivastigmine, galantamine, cholinomimetic side effects may increase (tear and salivation, pupil constriction, increased muscle tone or sudden muscle contractions, frequent urination, nausea, colic, vomiting, bronchial secretion with as a result, an asthma attack may occur).
