Scandonest is a fast acting local anesthetic.

Release form and composition

Dosage form - injection solution: transparent, colorless (1.8 ml each in transparent glass cartridges, sealed on both sides with butyl rubber stoppers, which are protected by aluminum caps, 10 or 20 cartridges in a blister, in a cardboard box 2, 3, 4, 5 or 6 blisters and instructions for use of Scandonest).

Active ingredient: mepivacaine hydrochloride, in 1 ml - 30 mg.

Auxiliary components: water for injection, sodium hydroxide and sodium chloride.

Pharmacological properties

Pharmacodynamics

Scandonest is a fast acting local anesthetic. Its active ingredient is mepivacaine hydrochloride, an amide-type substance.

Anesthetic properties are based on the inhibition of ionic flows, which are responsible for the development and conduction of impulses at the level of neuronal membranes.

Indications for use

Scandonest is used for conduction and infiltration local anesthesia during therapeutic and surgical interventions in the oral cavity.

Contraindications

  • severe myasthenia gravis;
  • severe liver disease;
  • porphyria;
  • children under 4 years of age;
  • pregnancy;
  • hypersensitivity to the components in Scandonest.

Carefully:

  • renal failure;
  • progression of cardiovascular failure;
  • conditions accompanied by a decrease in hepatic blood flow (for example, with liver disease, chronic heart failure, diabetes mellitus);
  • pseudocholinesterase deficiency;
  • inflammatory disease or infection of the injection site;
  • age over 65.

In large doses, Scandonest is not recommended or should be used with extreme caution in the following cases:

  • severe arterial hypotension;
  • decompensated chronic heart failure;
  • severe degree of impaired conduction of the heart, including atrioventricular block;
  • bradycardia.

Scandonest, instructions for use: method and dosage

Scandonest is used for conduction and infiltration anesthesia.

The usual dose ranges from 1-4 ml.

The dose for an adult administered within 2 hours should not exceed 6 ml, within 24 hours - 10 ml.

For children over 4 years old, the dose is determined individually, depending on the age and weight of the child, as well as the type of operation / manipulation performed. The average dose is 0.5 mg / kg body weight.

Elderly people are prescribed ½ of the recommended adult dose.

The drug is administered at a rate of not more than 0.5 ml for 15 seconds, i.e. 1 cartridge per minute. To avoid accidental injection of the solution into a vein, it is necessary to control aspiration.

It is recommended to use the lowest dose that can provide anesthesia.

Side effects

  • from the central nervous system: dilated pupils, motor restlessness, drowsiness, headache, visual and auditory disturbances, motor and sensory block, nystagmus, diplopia, trismus, weakness, dizziness, tremor, convulsions, impaired consciousness up to its loss;
  • from the vascular system: chest pain, a sharp decrease in blood pressure, arrhythmias, collapse (peripheral vasodilation), tachycardia or bradycardia (in some cases, it can turn into atrioventricular block);
  • from the digestive system: nausea, involuntary defecation, vomiting;
  • from the respiratory system: apnea, dyspnea;
  • from the urinary system: involuntary urination;
  • on the part of the blood: methemoglobinemia;
  • allergic reactions: local - skin rash, itching, urticaria, edema and inflammation at the injection site; systemic - pharyngeal edema, bronchospasm, angioedema; in severe cases, cardiopulmonary insufficiency caused by anaphylactic shock;
  • others: numbness and paresthesia of the tongue and lips, hypothermia, prolongation of anesthesia.

Side effects usually occur in case of hypersensitivity to the components in Scandonest, with increased absorption or accidental extravascular administration of the solution.

Overdose

The main symptoms are convulsions, anaphylactic shock.

Therapy is determined by the indications:

  • seizure control: administration of 10–20 mg of diazepam; it is undesirable to use barbiturates, since they can increase the depression of the central nervous system, it is possible to use muscle relaxants and drugs with an anticonvulsant effect;
  • stimulation of the cardiovascular system and pulmonary ventilation: the introduction of hydrocortisone; solutions of α- and β-adrenostimulants or atropine sulfate can be used;
  • relief of anaphylactic shock: you need to put the patient on his back, provide oxygen supply; intravenous infusion therapy is prescribed with the use of anti-shock drugs.

As an antiacidic agent, sodium bicarbonate can be injected intravenously.

To avoid deterioration of the condition due to an increase in oxygen demand, it is not recommended to use analeptics of central action.

In the event of serious symptoms of an overdose, measures are shown aimed at maintaining respiratory function, if possible with the use of oxygen, it is required to control blood pressure and pulse rate.

Scandonest is not recommended for use during treatment with monoamine oxidase inhibitors and within 10 days after their cancellation.

Influence on the ability to drive vehicles and complex mechanisms

After using the drug, care should be taken when driving a car or performing any potentially hazardous work that requires quick reactions and increased attention.

Application during pregnancy and lactation

Scandonest is not prescribed during pregnancy.

Mepivacaine in very small amounts can be excreted in breast milk. At the end of the anesthetic effect, breastfeeding can be continued.

Pediatric use

For patients under 4 years of age, the drug is not prescribed.

With impaired renal function

Scandonest for renal failure is prescribed under medical supervision.

For violations of liver function

  • severe liver disease: therapy is contraindicated;
  • conditions in which there is a decrease in hepatic blood flow (in particular, against the background of chronic heart failure, diabetes mellitus, liver diseases): the drug is used with caution.

Use in the elderly

Scandonest for elderly patients is prescribed under medical supervision.

Drug interactions

  • monoamine oxidase inhibitors (selegiline, furazolidone, procarbazine): the risk of lowering blood pressure increases;
  • anticoagulants (enoxaparin, dalteparin, heparin, ardeparin, warfarin): the likelihood of bleeding increases;
  • vasoconstrictors (phenylephrine, methoxamine, epinephrine): the local anesthetic effect of Scandonest is lengthened;
  • narcotic analgesics: an additive effect develops (this combination can be used for epidural anesthesia, but taking into account respiratory depression);
  • drugs that have a depressing effect on the central nervous system: their effect is enhanced:
  • cholinesterase inhibitors (anti-myasthenic drugs, thiotepa, cyclophosphamide): the metabolism of mepivacaine decreases;
  • muscle relaxants: their effect is enhanced and lengthened;
  • anti-myasthenic drugs: antagonism appears in the effect on skeletal muscles, especially when high doses are used (additional correction of the treatment regimen for myasthenia gravis is required).

In the case of treatment of the injection site of Scandonest with a disinfectant solution containing heavy metals, the risk of developing local edema and soreness increases.
