Complivit Calcium D3 is a regulator of calcium-phosphorus metabolism.

Release form and composition

Dosage form - orange chewable tablets: round, convex on both sides, with a rough porous surface, from white to white-creamy color (there may be small inclusions of grayish color) with a weak fruity odor. Tablets are packed: in polymer cans of 30, 60, 90, 100 and 120 pcs. and are sealed with a screw cap, each can, covered with a protective shrink cap, is packed in a cardboard box; in jars with a multi-page label containing the text of the instructions for use, 60, 90, 100 and 120 pieces each, the jars covered with a protective heat-shrink cap are placed in a shipping container.

Active substances in 1 tablet Complivit Calcium D3:

  • calcium (in the form of carbonate) - 500 mg;
  • vitamin D3 (colecalciferol) - 200 International units (0.005 mg) in terms of 100% colecalciferol. Complivit Calcium D3 is a combined composition drug that regulates the exchange of phosphorus and calcium in the body (muscles, bones, nails, hair and teeth), reduces bone resorption and increases its density, replenishes calcium deficiency, enhances phosphate reabsorption in the kidneys and calcium absorption in the intestine, contributing to the mineralization of dental tissue and bone tissue.

Calcium is an important element that is a component of the blood coagulation system, and also participates in the regulation of muscle contractions, nerve conduction and hormone production, in the formation of bone tissue and maintaining the stable activity of the heart system.

Adequate calcium intake is most important during the growth of the baby, during pregnancy and lactation.

Colecalciferol is a vitamin that increases the absorption of calcium in the body, is involved in the formation of dental and bone tissue.

When vitamin D3 and calcium are used in combination, an increase in the production of parathyroid hormone (PTH), which is a stimulator of increased bone resorption (leaching of calcium from the bones), is prevented.

Pharmacokinetics

Calcium is absorbed by an active D-vitamin dependent transport mechanism in the proximal small intestine in ionized form. The total amount of calcium absorbed from the gastrointestinal tract is usually 30% of the dose taken.

99% of calcium present in the body is concentrated in the rigid structure of teeth and bones, 1% - in extra- and intracellular fluids. About 50% of the calcium contained in the blood is in a physiologically active ionized form, about 40% of them are associated with proteins (mainly albumin), the remaining 10% are in a complex with phosphates, citrate or other anions.

Calcium is excreted from the body by the sweat glands, kidneys and intestines. Excretion by the kidneys depends on glomerular filtration and tubular calcium reabsorption.

Calcium D3 is absorbed in the small intestine. The total amount of absorbed substance is approximately 80% of the dose taken.

Colecalciferol and its metabolites circulate in the blood. In the liver, the substance is converted to 25-hydroxycalciferol by hydroxylation, after which in the kidneys it is converted into the active form 1,25-hydroxycalciferol, a metabolite that is responsible for enhancing calcium absorption. Unmetabolized vitamin D3 is deposited in muscle and adipose tissue.

Vitamin D3 is excreted by the kidneys and intestines.

Indications for use

Complivit Calcium D3 is used for the prevention and treatment of calcium and / or vitamin D3 deficiency.

For preventive purposes, taking the drug is recommended:

  • children in a period of intensive growth and development;
  • women during pregnancy and lactation;
  • women and men who consume insufficient amounts of dairy products;
  • women and men on a low-calorie diet.

Also Complivit Calcium D3 is used for complex therapy and prevention of osteoporosis and its complications (bone fractures).

Contraindications

  • hypervitaminosis D;
  • high urine calcium (hypercalciuria);
  • increased concentration of calcium in the blood (hypercalcemia);
  • nephrolithiasis;
  • decalcifying tumors (bone metastases, myeloma, sarcoidosis);
  • severe renal failure;
  • active pulmonary tuberculosis;
  • phenylketonuria;
  • hereditary lactose or fructose intolerance, glucose-galactose malabsorption, lactase or sucrase / isomaltase deficiency;
  • hypersensitivity to drug components.

Complivit Calcium D3 is not prescribed for babies under 3 years old.

The drug should be used with caution in case of renal insufficiency, during pregnancy and breastfeeding.

Instructions for the use of Complivit Calcium D3: method and dosage

Complivit Calcium D3 tablets should be taken with meals: dissolve or chew in the mouth and drink water, if required.

For the treatment (as part of complex therapy) of osteoporosis, adults are prescribed 1 tablet 2-3 times a day, for the prevention of this disease - 1 tablet 2 times a day. The duration of the course is determined by the doctor individually.

With a deficiency of calcium and / or vitamin D3, adults and children over 12 years old are recommended to take 1 tablet 2 times a day, children 5-12 years old - 1 tablet 1-2 times a day. The dose for children 3–5 years old is selected by the doctor. The duration of the course is at least 4–6 weeks.

The need for repeated courses and their number are determined individually for each patient.

Side effects

  • from the side of metabolism and nutrition: infrequently (> 1/1000, <1/100) - hypercalciuria, hypercalcemia;
  • from the gastrointestinal tract: rarely (> 1/10 000, <1/1 000) - flatulence, constipation / diarrhea, abdominal pain, nausea, dyspepsia;
  • on the part of the skin and subcutaneous tissue: very rarely (<1/10 000) - allergic reactions such as rash, itching and urticaria.

Overdose

An overdose (hypercalcemia) is manifested by the following symptoms: thirst, abdominal pain, nausea, anorexia, constipation, vomiting, polyuria, muscle weakness, bone pain, headache, increased fatigue, dizziness, mental disorders, fainting, urolithiasis, nephrocalcinosis, in severe cases - cardiac arrhythmias, coma.

With prolonged use of the drug in excessive doses (more than 2500 mg of calcium), soft tissue calcification and kidney damage are possible.

According to laboratory studies, in case of an overdose, hypercalcemia (plasma calcium content of about 2.6 mmol) and hypercalciuria are detected.

If signs of overdose appear, stop taking Complivit Calcium D3, cardiac glycosides and thiazide diuretics (if used) and consult a doctor.

As a therapeutic measure, the stomach is washed, fluid loss is replenished, loop diuretics (for example, furosemide), bisphosphonates, calcitonin and glucocorticosteroids are used, a diet with limited calcium intake is prescribed, and hemodialysis is performed if necessary. During the period of therapy, it is necessary to control the content of electrolytes in the blood plasma, diuresis and renal function. In severe cases, monitoring of the electrocardiogram and measurement of central venous pressure are required.

Patients with renal insufficiency during the period of therapy should be closely monitored. Monitoring of the content of phosphates and calcium in blood serum is required. You should also consider the risk of developing soft tissue calcification.

In order to prevent an overdose, it is necessary to take into account the additional (from other sources) intake of calcium and vitamin D.

With prolonged use of the drug, it is necessary to control the level of calcium and creatinine in the blood serum, especially in patients with an increased tendency to form kidney stones and in the elderly, receiving diuretics or cardiac glycosides at the same time. If there are signs of impaired renal function or hypercalcemia, the dose should be reduced or Complivit Calcium D3 should be canceled.

It is not recommended to simultaneously use antibiotics from the group of quinolones or tetracyclines. If their combined use is imperative, care must be taken.

With caution, Complivit Calcium D3 should be used in immobilized patients with osteoporosis, since they have an increased risk of developing hypercalcemia.

Application during pregnancy and lactation

During pregnancy, the drug can be used to compensate for the lack of calcium and vitamin D3 in the body. It should be borne in mind that the daily dose of calcium should not exceed 1500 mg, vitamin D3 - 600 IU, since hypercalcemia may develop as a result of an overdose, which can have an adverse effect on the fetus.

Complivit Calcium D3 is allowed to be taken during lactation, however, the intake of calcium and vitamin D3 from other sources, both in the mother and the child, should be taken into account.

Pediatric use

Complivit Calcium D3 for babies can be used from 3 years of age.

With impaired renal function

According to the instructions, Complivit Calcium D3 is contraindicated in severe renal failure.

Use in the elderly

Drug interactions

With hypercalcemia, calcium and vitamin D preparations can potentiate the toxic effects of cardiac glycosides. Electrocardiogram and serum calcium should be monitored.

Barbiturates and phenytoin can reduce the activity of vitamin D3.

Glucocorticosteroids reduce calcium absorption, so an increase in the dose may be required.

Calcium reduces the absorption of antibiotics from the quinolone group, so the antibacterial agent should be taken 2 hours before or 6 hours after taking the Complivit Calcium D3 tablet.

Calcium reduces the absorption of levothyroxine and, as a result, its effectiveness. There should be at least a 4-hour interval between their meals.

Calcium can reduce the absorption of tetracycline drugs from the gastrointestinal tract, so it is recommended to take tetracyclines 2 hours before or 4-6 hours after taking the Complivit Calcium D3 tablet.

In order to avoid a decrease in the absorption of bisphosphonates, they should be taken at least 1 hour before taking Complivit Calcium D3.

Cholestyramine and laxatives based on vegetable or mineral oil can reduce the absorption of vitamin D3.

Foods containing phytin (cereals) and oxalates (rhubarb, sorrel, spinach) reduce the absorption of calcium, so the drug should not be taken within 2 hours of consuming these foods.

The combined use of thiazide diuretics increases tubular calcium reabsorption and the risk of hypercalcemia. When taking such a combination, it is necessary to constantly monitor the calcium content in the blood serum. Loop diuretics (including furosemide), on the other hand, increase the excretion of calcium by the kidneys.
