Release form and composition

The dosage form of Momat Rino is a metered-dose nasal spray: a suspension of a thick consistency, translucent or opaque, almost white or white (60 and 120 doses each in polyethylene bottles with a dispensing device, 1 bottle in a cardboard box).

Spray composition:

  • active substance: mometasone furoate (in the form of monohydrate), in 1 dose - 50 mcg;
  • additional components: avicel RC-591 (microcrystalline cellulose, sodium carmellose), benzalkonium chloride, citric acid monohydrate, glycerol, polysorbate-80, sodium citrate dihydrate, water for injection.

Pharmacological properties

Pharmacodynamics

Mometasone furoate is a synthetic corticosteroids for local use with anti-inflammatory and antiallergic effects (when used in doses that do not cause systemic effects).

Mometasone increases the production of lipomodulin, an inhibitor of phospholipase A, as a result of which the release of arachidonic acid decreases, the synthesis of prostaglandins and cyclic endoperoxides (products of arachidonic acid metabolism) is inhibited. Prevents the marginal accumulation of neutrophil cells, thereby reducing inflammatory exudate and production of lymphokines, stopping the migration of macrophages, reducing the processes of infiltration and granulation.

The drug prevents the development of an immediate type of allergic reaction by inhibiting the production of metabolites of arachidonic acid and reducing the release of inflammatory mediators from mast cells. Reduces the severity of the inflammatory process by reducing the formation of the chemotaxis substance (affects the "late" allergy reactions).

Pharmacokinetics

The systemic bioavailability of mometasone with intranasal administration is less than 1% (with a sensitivity of the method of determination of 0.25 pg / ml). In the gastrointestinal tract, the suspension is practically not absorbed, and the small amount that can get there after nasal inhalation undergoes active primary metabolism even before excretion with bile and urine.

Indications for use

  • nasal polyposis, accompanied by impaired sense of smell and nasal breathing, in adults;
  • acute rhinosinusitis with mild to moderate symptoms in children over 12 years of age and adults (provided there are no signs of severe bacterial infection);
  • acute sinusitis and exacerbation of chronic sinusitis in children from 12 years of age and adults (including the elderly) - as an aid in antibiotic therapy;
  • seasonal and year-round allergic rhinitis in children from 2 years of age and adults;
  • prevention of moderate and severe seasonal allergic rhinitis in children from 12 years of age and adults (2-4 weeks before the expected start of the dusting season).

Contraindications

  • recent trauma with damage to the nasal mucosa or surgery (until the wound heals);
  • hypersensitivity to any component of the drug.

Use in childhood: for allergic rhinitis, Momat Rino is contraindicated in children under 2 years of age, for sinusitis - up to 12 years, with polyposis - up to 18 years.

According to the instructions, Momat Reno should be used with caution (according to strict indications, only as directed by a doctor, after assessing the benefits / risks) in the following cases:

  • untreated local infection in which the nasal mucosa is involved;
  • active and latent tuberculosis infection of the respiratory tract;
  • active systemic viral, bacterial, fungal infection or Herpes simplex infection with lesion.

Instructions for use of Momat Rino: method and dosage

Spray Momat Rino is used intranasally. Inhalation is carried out using the dispensing nozzle provided in the kit.

Before using the spray for the first time, you must calibrate it - press the dispensing nozzle 10 times until splashes appear. This indicates the readiness of the drug for use. If the product is not used for 14 or more days in a row, before use, you should make 2 test pressing in the air until splashes appear.

For inhalation, you need to tilt your head and inject a spray into each nasal passage as recommended by your doctor.

The dispensing tip must be cleaned regularly. This is essential for its correct operation. To do this, remove the protective cap, and then the spray tip and rinse them thoroughly in warm water, rinse under a tap and dry in a warm place, after which the tip must be attached to the bottle and closed with a protective cap.

Do not pierce the nasal applicator or try to open it with sharp objects (for example, needles), otherwise this will lead to damage and, possibly, inappropriate further dosing of the drug.

After cleaning, before the first use, it is necessary to calibrate - press the dispensing nozzle 2 times into the air.

Before each use of Momat Reno, shake the bottle vigorously.

Seasonal and perennial allergic rhinitis

For prevention and treatment, children over 12 and adults are prescribed 2 inhalations in each nostril once a day (total daily dose is 200 mcg). During therapy, after improvement of the condition, it is possible to reduce the dose to a maintenance one - 1 inhalation in each nasal passage once a day (total daily dose - 100 μg).

If it is not possible to reduce the symptoms of the disease when using Momat Rino in the recommended therapeutic dose, it is possible to increase the daily dose to 400 μg - 4 inhalations in each nostril 1 time per day. After reducing the severity of symptoms, the dose is reduced.

Children from 2 to 11 years old are prescribed 1 inhalation in each nasal passage once a day (the total daily dose is 100 mcg). Young children require adult help when using the drug.

Momat Rino nasal spray begins to act 12 hours after the first inhalation.

Acute sinusitis and exacerbation of chronic sinusitis (adjuvant therapy)

For prevention and treatment, children from 12 years old and adults are prescribed 2 inhalations in each nostril 2 times a day (total daily dose - 400 mcg).

If it is not possible to reduce the symptoms of the disease when using Momat Rino in the recommended therapeutic dose, it is possible to increase the daily dose to 800 mcg - 4 inhalations in each nostril 2 times a day. After a decrease in the severity of symptoms, the dose is reduced.

Acute rhinosinusitis without evidence of severe bacterial infection

Children from 12 years old and adults are prescribed 2 inhalations 2 times a day.

Polyposis of the nose

  • infectious and parasitic diseases: often - upper respiratory tract infections (this side effect rarely occurs with nasal polyposis if the drug is used 2 times a day), pharyngitis;
  • from the nervous system: often - headache;
  • from the gastrointestinal tract: often - irritation of the pharynx (with nasal polyposis in the case of using the drug 2 times a day); unspecified frequency - violation of taste and smell;
  • on the part of the organ of vision: unknown frequency - glaucoma, increased intraocular pressure, cataract;
  • from the immune system: unknown frequency - hypersensitivity reactions, including shortness of breath, bronchospasm, angioedema, anaphylactic reactions.

The incidence of adverse events in children with Momat Rino was comparable to that with placebo.

  • from the nervous system: headache (3%);
  • from the respiratory system, chest and mediastinal organs: nosebleeds (6%), irritation of the nasal mucosa (2%), sneezing (2%).

Intranasal GCS, especially when used in high doses and (or) for a long time, can cause the development of systemic side effects.

Overdose

With prolonged treatment with the drug in high doses, as well as in the case of the simultaneous use of other GCS, it is possible to suppress the function of the hypothalamic-pituitary-adrenal system.

Given the low systemic bioavailability of mometasone, it is unlikely that overdose (intentional or accidental) will require any therapeutic measures other than observation.

When carrying out long-term therapy, patients need to undergo periodic examinations by a doctor in order to timely identify possible changes in the nasal mucosa and the development of systemic side effects.

The safety and efficacy of mometasone in the treatment of bleeding polyps, unilateral irregular polyps, polyps that completely cover the nasal cavity, and polyps associated with cystic fibrosis have not been studied.

When using GCS in higher doses than recommended, clinically significant suppression of adrenal function is possible. When prescribing Momat Rino in high doses during periods of stress and elective surgery, additional intake of systemic corticosteroids may be required.

With prolonged use of a nasal spray in high doses, systemic side effects may develop. This risk is significantly lower compared to taking systemic corticosteroids and may differ from patient to patient. Potential systemic reactions include a range of psychological and behavioral effects, including anxiety, sleep disturbance, psychomotor hyperactivity, depression and aggression (especially in children), as well as adrenal suppression, glaucoma, cataracts, Cushingoid features, Cushing's syndrome, growth retardation in children and adolescents.

Children who have been receiving mometasone for a long time should be monitored regularly for growth.
