Iodovidone is an antiseptic drug.

Release form and composition

Dosage form - spray for local and external use 0.5%: transparent red-brown solution, possibly with a slight opalescence (in a cardboard box 1 aerosol aluminum can containing 30 g of spray, complete with a safety cap, a mechanical pump and a spray nozzle, as well as instructions for the use of Yodovidone).

1 spray bottle contains:

• active substance: povidone-iodine - 1.38 g;
• auxiliary substances: sodium phosphate 12-water (sodium phosphate dodecahydrate) - for pH regulation; propylene glycol - 9 g; ethyl alcohol 95% (ethanol) - 6 g; levomenthol - 0.3 g; purified water - to obtain 30 g of the drug.

Pharmacological properties

Pharmacodynamics

Iodovidone is an antiseptic drug that contains a complex of iodine and povidone. The drug is effective at a pH of 2 to 7.

Free active iodine, released from the povidone-iodine complex, determines the bactericidal effect of the drug. As a strong oxidant, it reacts at the molecular level with insoluble fatty acids, as well as almost non-oxidizable OH- and SH-groups of amino acids in the main structural components and enzymes of gram-positive / gram-negative bacteria, microbacteria, various viruses, fungi (especially the genus Candida) and some protozoa.

It should be borne in mind that the complete inactivation of some bactericidal spores and viruses requires a sufficiently long period of drug exposure.

With long-term therapy with povidone-iodine, the development of resistance in microorganisms was not observed.

Pharmacokinetics

The results of tests of the functional activity of the thyroid gland may be affected by prolonged use and / or application of Iodovidone to large areas of the wound surface and mucous membranes, which is associated with possible systemic absorption of iodine.

Once in the body, iodine is converted into iodides, which are excreted mostly by the kidneys and in small amounts with saliva, through the intestines and sweat.

Passing through the placental barrier, iodides penetrate into breast milk.

Indications for use

• infectious and inflammatory pathologies of the ENT organs: aphthous ulcerative stomatitis, acute and exacerbation of chronic tonsillitis, laryngitis, pharyngitis (as part of a combination treatment);
• dental manipulations (for disinfection of the oral cavity before carrying out them);
• diaper rash, abrasions, cuts, wounds (for antiseptic skin treatment).

Contraindications

Absolute:

• heart failure;
• adenoma of the thyroid gland, thyrotoxicosis (dysfunction of the thyroid gland);
• Duhring's dermatitis herpetiformis;
• combined therapy with radioactive iodine;
• III trimester of pregnancy;
• lactation period;
• age under 8;
• individual intolerance to the components of the antiseptic.

Relative (Iodovidone is prescribed under medical supervision) for chronic renal failure (CRF) and in the period up to 9 weeks of pregnancy.

Iodovidone, instructions for use: method and dosage

Spray Iodovidone is applied topically or externally.

For topical use, remove the protective cap from the cylinder and install the nozzle. In order for the solution to enter the spray bottle, press it 2-3 times. Next, the tip of the nozzle is placed into the oral cavity by 2-3 cm. The patient holds his breath and presses the nozzle twice so that after the first pressing the solution is sprayed to the right side of the throat, and after the second - to the left.

The oral cavity and pharynx are treated with Iodovidone 2 to 4 times a day. If necessary, the spray can be used every 4 hours, one injection to the right and to the left.

After irrigation of the oral cavity, patients are advised to abstain from food for 15-30 minutes.

Externally, the drug is applied to the surface of the skin with a spray method. The number of clicks depends on the area of the affected skin area. The frequency of use of Iodovidone is 1-2 times a day. Treatment is continued as needed, but its duration should not exceed 7-10 days.

Side effects

The use of Iodovidone can lead to the development of hypersensitivity - allergic reactions of an immediate / delayed type (anaphylactoid reactions / blisters, skin redness, itching).

Prolonged use of the spray, exceeding 7-10 days, and / or application of the solution to large areas of the skin or wounds may serve the development of the following phenomena:

• iodism: swelling of the larynx or eyes, increased salivation, metallic taste in the mouth;
• systemic reactions: dysfunction of the thyroid gland and kidneys, hyponatremia, metabolic acidosis.

In such cases, as well as if these side effects worsen or any other responses to therapy appear, you should stop using the spray and consult a doctor for advice.

Overdose

The main symptoms: with accidental oral administration of Iodovidone, laryngeal / pulmonary edema, collapse, anuria develops. In cases of intense absorption of iodine over a long period, headache, tremors, agitation, tachycardia may appear.

Therapy: in case of accidental ingestion - gastric lavage, symptomatic treatment. With intensive absorption of iodine, it is recommended to immediately discontinue the drug and consult a doctor.

It is important not to allow the use of Iodovidone in excessive quantities, to avoid its contact with the eyes and the accumulation of excess solution in wounds, cavities and folds.

Patients with impaired thyroid function should use the spray under medical supervision.

In children, the drug should be used in the minimum effective doses for a short period.

With caution, under the supervision of a physician, Iodovidone can be used regularly on the damaged skin surface in patients with CRF.

Coloring on fabrics and leather can be easily washed off with soap and water. If necessary, the use of a solution of sodium thiosulfate or ammonia is allowed.

Application during pregnancy and lactation

Spray Iodovidone from the third trimester of pregnancy and during lactation is not prescribed.

Up to the 9th week of pregnancy, the use of a spray is allowed in cases where the potential effect of therapy outweighs the possible risks of complications.

Pediatric use

For patients under 8 years of age, Iodovidone is not prescribed.

With impaired renal function

In chronic renal failure, the use of the drug is contraindicated.

Drug interactions

Iodovidone is incompatible with hydrogen peroxide, taurolidine, bismuth and silver salts, alkaloids (celandine), salicylic acid, tannin (tannic acid).

The bactericidal effect of the drug can be reduced in the presence of pus or blood on the wound surface, which is due to its binding to proteins and other organic compounds. In such cases, the frequency / frequency of spray application should be increased.

The combined use of povidone-iodine with topical drugs containing enzymes can lead to oxidation and suppression of their enzymatic activity.

When combined with lithium preparations, povidone-iodine has a synergistic effect, which, with prolonged treatment, can lead to reversible suppression of the thyroid gland function. In this regard, patients who are constantly receiving lithium preparations should not apply the spray to large areas of the skin.

The results of a study of the function of the thyroid gland, an analysis for the determination of sugar or hemoglobin in urine / feces against the background of the use of Iodovidone may be false-positive. In this regard, it is recommended to examine the thyroid gland no earlier than after 7-14 days after the end of therapy with povidone-iodine.
