Anastrozole is a drug with an antitumor effect.

Release form and composition

The dosage form of Anastrozole is film-coated tablets: round, biconvex, almost white or white; at the break there are two layers - a white or almost white core and a film sheath (7, 10, 14 or 30 pieces in blisters or blister packs, 1-10 blisters / packs in a cardboard box; 7, 10, 14, 20, 28, 30, 40, 50, 60, 100 or 120 pieces in cans, 1 can in a cardboard box).

Composition of 1 tablet:

  • active substance: anastrozole - 1 mg;
  • auxiliary components: sodium carboxymethyl starch - 2 mg, microcrystalline cellulose - 15.6 mg, lactose monohydrate (milk sugar) - 72 mg, magnesium stearate - 0.9 mg, polyvinylpyrrolidone (povidone) - 3 mg, colloidal silicon dioxide - 0.5 mg;
  • shell: titanium dioxide - 0.9 mg, macrogol 4000 - 0.45 mg, hypromellose - 1.65 mg.

Indications for use

  • early hormone-positive breast cancer in postmenopausal women (adjuvant therapy);
  • early hormone-positive breast cancer in postmenopausal women after using tamoxifen for 2-3 years (adjuvant therapy);
  • advanced breast cancer in postmenopausal women (treatment).

Contraindications

Absolute:

  • premenopausal period;
  • moderate / severe hepatic impairment (the safety profile for this group of patients has not been studied);
  • renal failure, severe (with creatinine clearance <20 ml / min);
  • combined use with tamoxifen or estrogen-containing drugs;
  • pregnancy and lactation;
  • age up to 18 years (the safety profile for this group of patients has not been studied);
  • hypersensitivity to the components of the drug.

Relative (Anastrozole is prescribed with caution in the presence of the following diseases / conditions):

  • cardiac ischemia;
  • hypercholesterolemia;
  • osteoporosis;
  • functional disorders of the liver;
  • lactose intolerance / lactase deficiency / glucose-galactose malabsorption (the drug contains lactose).

Method of administration and dosage

Anastrozole is taken orally, preferably at the same time. The tablets must be swallowed whole and washed down with water. Food intake does not affect the effectiveness of the drug.

Anastrozole is prescribed 1 tablet per day. Long-term therapy is indicated. The drug is canceled in cases of signs of disease progression. The recommended duration of adjuvant therapy is 5 years.

No dose adjustment is required for mild / moderate renal and mild hepatic impairment.

Side effects

  • respiratory system, chest and mediastinal organs: bronchitis, pharyngitis, sinusitis, shortness of breath;
  • lymphatic system and blood: lymphedema, anemia, venous thromboembolism;
  • genitourinary system: dryness of the vaginal mucosa, vulvovaginitis, urinary tract infections, vaginal bleeding (as a rule, during the first weeks after discontinuation or when changing the previous hormonal treatment to Anastrozole);
  • nervous system: drowsiness, headache, paresthesia, depression, dizziness, insomnia, anxiety, carpal tunnel syndrome (usually observed in the presence of risk factors for the development of the disease);
  • cardiovascular system: increased blood pressure, flushing of the face;
  • musculoskeletal system and connective tissues: arthralgia / joint stiffness, arthritis, trigger finger, muscle / bone pain;
  • digestive system: nausea, dyspepsia, diarrhea, constipation, vomiting, gastrointestinal disorders;
  • hepatobiliary system: hepatitis, increased activity of alkaline phosphatase, aspartate aminotransferase, alanine aminotransferase, increased concentration of bilirubin and gamma-glutamyl transferase activity;
  • skin and subcutaneous tissue: thinning hair, skin rash, alopecia, sweating;
  • organ of vision: cataract;
  • metabolism and nutrition: anorexia, weight gain, hypercholesterolemia, hypercalcemia (with / without an increase in the concentration of parathyroid hormone); possibly a decrease in bone mineral density due to a decrease in the concentration of circulating estradiol, which increases the risk of developing bone fractures and osteoporosis;
  • allergic reactions: urticaria, anaphylactoid reaction, erythema multiforme, Stevens-Johnson syndrome, cutaneous vasculitis (including isolated cases of purpura), angioedema;
  • others: secondary infections, asthenia, back / abdominal / chest pain, neoplasms, flu-like syndrome.

special instructions

The efficacy of Anastrozole in estrogen receptor-negative tumors has not been demonstrated, except in cases where there is a previous positive clinical response to tamoxifen. If there is any doubt about the hormonal status of the patient, menopause must be confirmed by the determination of sex hormones in the blood serum.

Combined use with drugs containing estrogens is contraindicated (since they neutralize the effect of Anastrozole).

If uterine bleeding persists during therapy, you should consult a gynecologist.

For osteoporosis or an increased risk of osteoporosis at the beginning of treatment and regularly throughout the course of treatment, bone mineral density should be assessed by densitometry, such as dual energy X-ray absorptiometry (DEXA). If necessary, therapy or prevention of osteoporosis should be prescribed, as well as careful monitoring of the patient's condition. Anastrozole lowers the concentration of circulating estradiol, which can lead to a decrease in bone mineral density. At the moment, there is no sufficient information regarding the positive effect of bisphosphonates on the loss of bone mineral density associated with taking Anastrozole, or the effectiveness of their use for prevention.

There are no data on the combined use of Anastrozole with drugs analogous to luteinizing hormone releasing hormone (LHRH).

The effectiveness of combined use with chemotherapy has not been established.

Safety data for long-term treatment with Anastrozole are not available.

With the use of Anastrozole more often than with tamoxifen therapy, the development of ischemic diseases was noted, however, there was no statistical significance.

Since during the period of using Anastrozole, such disorders as dizziness, drowsiness, asthenia, headache may develop, this can adversely affect the ability to drive vehicles and mechanisms, as well as perform other work that requires increased concentration of attention and rapid psychomotor reactions (care must be taken).

Drug interactions

There is no clinically significant drug interaction between Anastrozole and other commonly prescribed drugs.

Medicines containing estrogens reduce the pharmacological action of Anastrozole (the combination is contraindicated).

Combined use with tamoxifen is not recommended (due to the likelihood of weakening the pharmacological action of anastrozole).
