Pharmacological properties
Pharmacodynamics
Betahistine affects mainly the histamine H 1 and H 3 receptors localized in the vestibular nuclei of the central nervous system and the vessels of the inner ear, exerting a direct agonistic effect on them. The drug improves microcirculation and capillary permeability, as well as normalizes the endolymph pressure created in the cochlea and ear labyrinth. At the same time, betahistine activates blood flow in the basilar artery and accelerates the regeneration of vestibular function after unilateral vestibular neurectomy, providing acceleration and facilitation of central vestibular compensation (through antagonism with H 3 -histamine receptors). The drug is characterized by a pronounced central effect, being an inhibitor of Н 3-receptors located in the nuclei of the vestibular nerve. It also weakens the generation of action potentials in the neurons of the medial and lateral vestibular nuclei, and this effect is dose-dependent.
Betahistine helps to alleviate the condition in patients with vestibular dizziness and Meniere's syndrome. A stable therapeutic effect is recorded 14 days after the start of therapy.
Pharmacokinetics
Betahistine is absorbed at a high rate, and only slightly binds to blood plasma proteins. The maximum concentration of the active substance of Vestikap in plasma is reached within 3 hours after administration. Betahistine is metabolized to form inactive metabolites: demethylbetahistine and 2-pyridylacetic acid, which is the main metabolite. From 85 to 90% of the dose taken is excreted in the urine in the form of 2-pyridylacetic acid within 24 hours. Excretion of unchanged betahistine and demethylbetahistine through the kidneys is negligible. Only a small part of betahistine and its metabolites is excreted with feces.
