Biprol is a drug from the group of beta-blockers (selective), which is used in the treatment of arterial hypertension and angina pectoris. The international name of the drug is bisoprolol.

Composition and form of release

Biprol is available in 5 and 10 mg tablets.

The active substance is bisoprolol fumarate.

Pharmacological action of Biprol

This drug has hypotensive (lowering blood pressure), antianginal and antiarrhythmic effects. The hypotensive effect is associated with a decrease in the minute blood volume, as well as stimulation of peripheral vessels of the sympathetic nervous system. After the start of taking the drug, the hypotensive effect occurs in 2-5 days. A stable effect of therapy is expected in 1-2 months.

The maximum concentration according to the instructions of Biprol is noted 1-3 hours after its administration. The drug is metabolized in the liver, and 50% is excreted by the kidneys unchanged, and 2% is excreted in the bile.

Contraindications to the use of Biprol

Hypersensitivity to the constituent components, hypersensitivity to other drugs from the same group. According to the instructions, Biprol should not be used in shock, pulmonary edema, chronic heart failure 4 f.cl., AV blockade of 2-3 degrees, acute heart failure, arterial hypotension (especially due to acute myocardial infarction), bronchial asthma and other obstructive pulmonary diseases. Concomitant use of Biprol or analogs with MAO inhibitors is contraindicated. It is undesirable to take the drug in patients with pheochromocytoma and metabolic acidosis.

According to the instructions, Biprol should be used with caution in liver failure, chronic renal failure, myasthenia gravis, diabetes mellitus, thyrotoxicosis, depression, psoriasis, 1 tbsp. The use of Biprol and analogs in pregnant women is possible only when the benefits of using this drug justify the risk of side effects in the fetus.

According to the instructions, Biprol is prescribed once a day, 5 mg. The drug is taken in the morning on an empty stomach. If necessary, the daily dose can be increased to 10 mg. The maximum possible daily dose is 20 mg.

Side effects of Biprol

According to reviews, Biprol rarely causes serious side effects. From the side of the nervous system, the development of fatigue, weakness, dizziness, sleep disorders, anxiety, depression, asthenia is possible. From the side of the cardiovascular system, there may be: bradycardia, myocardial conduction disturbance, AV block, the development of arrhythmias, increased manifestations of cardiovascular failure, orthostatic hypotension, the occurrence of angiospasm (impaired peripheral circulation, cold extremities, aggravation of Raynaud's disease symptoms). From the side of the senses, visual disturbances, dry eyes, conjunctivitis may occur. On the part of the gastrointestinal tract, the following symptoms may occur: nausea, vomiting, abdominal pain, stool disturbances, changes in taste, hepatic dysfunction (cholestasis). From the respiratory system, there may be difficulty breathing (when using high doses of Biprol), in predisposed patients, bronchospasm may occur. Allergic reactions are also possible on the drug (rash, hives, itching). People with diabetes who receive insulin may experience hypoglycemia (low blood sugar).

Overdose

Overdose symptoms: fainting, convulsions, bradycardia (decrease in heart rate less than 50 beats per minute), extrasystole, arrhythmia, severe hypotension, difficulty breathing, bronchospasm.

Treatment of an overdose consists in gastric lavage, the appointment of adsorbing agents. In the case of the development of AV block, intravenous atropine is indicated. With hypotension, intravenous administration of plasma-substituting solutions is indicated, in case of ineffectiveness, dopamine, dobutamine, epinephrine are prescribed. With the development of heart failure, cardiac glycosides, diuretics are used. If seizures occur, diazepam should be given intravenously.

Interaction of Biprol with other drugs

With intravenous administration of iodine-containing X-ray contrast agents while taking Biprol and analogues, the risk of developing anaphylactic reactions is increased.

According to the instructions, Biprol changes the action of insulin and tableted hypoglycemic drugs. The drug masks the symptoms of hypoglycemia (low blood sugar).

Nonsteroidal anti-inflammatory drugs, glucocorticoids and estrogens, when used simultaneously with Biprol, according to reviews, weaken its hypotensive effect.

This drug, together with cardiac glycosides and calcium channel blockers (eg, verapamil) and other antiarrhythmic drugs, increases the risk of bradycardia, heart failure, and AV block.

According to the instructions, Biprol together with nifedipine and diuretics can lead to a significant decrease in blood pressure.

The combination of this drug with antidepressants and antipsychotic drugs increases the depression of the central nervous system.
