Release form and composition

The drug is available in the form of tablets of prolonged action, film-coated, 600 mg (10 pieces in a blister, 2 blisters in a cardboard box).

The active ingredient of Vasonite is pentoxifylline, as auxiliary components, the drug contains:

  • Microcrystalline cellulose - 13.5 mg;
  • Colloidal silicon dioxide - 3 mg;

Pharmacological properties

Pharmacodynamics

Pentoxifylline refers to xanthine derivatives, which leads to an improvement in microcirculation in areas with impaired blood supply. It improves the rheological parameters of blood (fluidity) due to the effect on the deformability of erythrocytes that have undergone pathological changes. Pentoxifylline also normalizes the elasticity of erythrocyte membranes, inhibits the aggregation of platelets and erythrocytes and reduces the increased viscosity of blood.

The mechanism of action of the active substance Vasonite is expressed in the inhibition of phosphodiesterase and the accumulation of cyclic adenosine monophosphate (cAMP) in the blood cells and cells that make up the smooth muscles of the vessels. Pentoxifylline reduces the concentration of fibrinogen in the blood plasma and activates fibrinolysis, which leads to a decrease in blood viscosity and an improvement in its rheological parameters, and also improves the saturation of tissues with oxygen in the areas in which circulatory disorders are diagnosed, in particular, in the central nervous system, extremities and, to a lesser extent, in kidneys. With occlusive lesion of the peripheral arteries, accompanied by intermittent claudication, Vasonite helps to reduce pain at rest, eliminate cramps of the calf muscles at night and increase walking distance. With disorders of cerebral circulation, pentoxifylline improves symptoms. The substance is characterized by a slight myotropic vasodilating effect and expansion of the coronary vessels, as well as a slight decrease in the total peripheral vascular resistance.

Pharmacokinetics

After oral administration, pentoxifylline is almost completely absorbed from the gastrointestinal tract. It is released in a prolonged form, which ensures continuous release of the substance and its uniform absorption in the body. Pentoxifylline is metabolized in the liver, undergoing a "first pass" effect with the formation of two pharmacologically active metabolites: 1-5-hydroxyhexyl-3,7-dimethylxanthine (metabolite I) and 1-3-carboxypropyl-3,7-dimethylxanthine (metabolite V). The level of metabolites I and V in blood plasma, respectively, is 5 and 8 times higher than that of pentoxifylline itself. When Vazonit is ingested in tablet form, the maximum content of pentoxifylline and its active metabolites in the blood plasma is observed 3-4 hours after ingestion, and the therapeutic effect lasts for about 12 hours. The drug is excreted mainly through the kidneys (about 94%) in the form of metabolites. It also passes into breast milk. With severe liver dysfunctions, the excretion of metabolites slows down. With impaired liver function, an increase in bioavailability and an increase in the half-life are observed.

Indications for use

According to the instructions, Vasonite is used in the following cases:

  • Chronic and acute disorders of cerebral circulation of ischemic origin;
  • Dyscirculatory and atherosclerotic encephalopathy, angioneuropathy (Raynaud's disease, paresthesia);
  • Circulatory disorders of the eye (chronic and acute circulatory failure in the choroid or retina of the eye);
  • Disturbances of peripheral circulation against the background of diabetic, atherosclerotic and inflammatory processes (including intermittent claudication caused by obliterating endarteritis, atherosclerosis, diabetic angiopathy);
  • Trophic tissue disorders arising against the background of a violation of venous or arterial microcirculation (trophic ulcers, frostbite, post-thrombophlebitic syndrome, gangrene);
  • Dysfunction of the middle ear of vascular origin, accompanied by hearing loss.

Also, Vasonite is prescribed for the purpose of symptomatic treatment of the consequences of cerebral circulation disorders of atherosclerotic genesis (dizziness, impaired concentration and memory).

Contraindications

Contraindications to the use of Vasonite are:

  • Massive bleeding;
  • Retinal hemorrhage;
  • Acute myocardial infarction;
  • Acute hemorrhagic stroke;
  • Hypersensitivity to drug components and other methylxanthine derivatives;
  • Pregnancy and lactation period;
  • Age less than 18 years (the safety and efficacy of the drug for this age category have not been established).

Instructions for use of Vasonite: method and dosage

The drug should be taken orally after meals, without violating the integrity of the tablet and drinking plenty of liquid.

In most cases, it is recommended to take 1 tablet of 600 mg Vasonite in the morning and in the evening. The maximum daily dose is 1200 mg.

The duration of treatment and the dosage regimen is determined by the doctor in accordance with the clinical picture of the disease and the therapeutic effect achieved.

When treating patients with chronic renal failure (CC less than 30 ml / min), a 600 mg dose of Vasonite should not be used.

In patients with severely impaired liver function, dose reduction should be made taking into account individual tolerance.

Treatment of patients with low blood pressure, as well as patients at risk due to a possible decrease in blood pressure (hemodynamically significant cerebral stenosis, severe coronary artery disease) is recommended to start with low doses. In such cases, only a gradual increase in dose is allowed.

Side effects

  • From the digestive system: diarrhea, nausea and vomiting, epigastric pain, dry mouth, decreased appetite, feelings of fullness and pressure in the stomach, intestinal atony, cholestatic hepatitis, exacerbation of cholecystitis, increased activity of alkaline phosphatase and liver enzymes;
  • From the side of the central nervous system: headaches and dizziness, sleep disturbances, anxiety, convulsions, cases of aseptic meningitis have been recorded;
  • From the side of the cardiovascular system: tachycardia, arrhythmia, decreased blood pressure, progression of angina pectoris, cardialgia;
  • From the hematopoietic system and homeostasis: rarely - thrombocytopenia, pancytopenia, aplastic anemia, leukopenia, hypofibrinogenemia, bleeding (from the intestines, gastric vessels, mucous membranes and skin).   • On the part of the skin and subcutaneous fat: flushing of the skin of the face and upper chest, increased brittleness of the nails, hyperemia of the facial skin, edema;
  • From the side of the organs of vision: visual impairment, scotoma;
  • Allergic reactions: skin hyperemia, angioedema, Quincke's edema, pruritus, urticaria, anaphylactic shock.

Overdose

In case of an overdose of Vazonit, symptoms such as weakness, drowsiness, dizziness, flushing of the skin, a marked decrease in blood pressure, tachycardia, areflexia, increased body temperature (chills), fainting may occur. Sometimes an overdose of the drug is accompanied by vomiting like "coffee grounds", which speaks of gastrointestinal bleeding, and tonic-clonic seizures.

As a treatment, gastric lavage followed by ingestion of activated carbon is recommended. When vomiting with blood streaks appears, gastric lavage is strictly prohibited. In the future, symptomatic therapy is prescribed, aimed at maintaining normal blood pressure and respiratory function. For seizures, diazepam is recommended.

Pentoxifylline is able to enhance the effect of drugs that affect the blood coagulation system (thrombolytics, indirect and direct anticoagulants), valproic acid, antibiotics (including cephalosporins - cefotetan, cefoperazone, cefamandol). Increases the effectiveness of oral hypoglycemic agents, insulin and antihypertensive agents.

Cimetidine increases the level of pentoxifylline in the blood plasma (side effects may develop). The combined use of Vasonit with other xanthines can lead to nervous overexcitation. In some patients, the combination of theophylline and pentoxifylline can provoke an increase in theophylline concentration, which is accompanied by an increased risk of side effects associated with theophylline.
