Troxymethacin is a topical anti-inflammatory drug containing a combination of indomethacin with troxerutin.

Release form and composition

Dosage form - gel for external use, from yellow-brown to yellow, transparent, homogeneous (in a cardboard box 1 aluminum tube containing 40 g of gel, and instructions for the use of Troxymetacin).

Composition of 1 g of gel:

  • active substances: indomethacin - 30 mg; troxerutin - 20 mg;
  • Auxiliary components: purified water - 110 mg; dimethyl sulfoxide - 150 mg; isopropanol - 150 mg; macrogol-400 - 522 mg; sodium benzoate - 2.5 mg; disodium edetate - 0.5 mg; carbomer - 15 mg.

Pharmacological properties

Pharmacodynamics

Troxymethacin is a topical combination drug containing a non-steroidal anti-inflammatory agent, an indoleacetic acid derivative - indomethacin, and an angioprotector - troxerutin.

The pharmacological action of the drug is due to the following properties of its active ingredients:

  • indomethacin: has a pronounced anti-inflammatory, analgesic and decongestant effect, due to which the pain syndrome is relieved, swelling decreases and the regeneration time of the affected tissues is reduced. Through the reversible blockade of cyclooxygenase 1 and 2, its main mechanism of action is realized, associated with the suppression of prostaglandin synthesis;
  • troxerutin (trihydroxyethylrutinoside): bioflavonoid with angioprotective action. Reduces capillary permeability, has venotonic properties, blocks the venodilating effect of acetylcholine, bradykinin and histamine. In relation to the peri-venous tissue, it exhibits anti-inflammatory activity, reduces capillary fragility, and to some extent has an antiplatelet effect. Against the background of various pathological changes associated with venous insufficiency, it improves trophism and reduces edema.

When Troxymethacin gel is applied to the skin, the inflammatory edematous reaction is suppressed, pain and temperature decrease in the inflamed foci, both on the surface and in depth, since it penetrates deeply and reaches the blood vessels located there. The drug has a pronounced capillaroprotective and venotonic effect.

Pharmacokinetics

Due to the gel base, the release and solubility of the active components of Troxymetacin - troxerutin and indomethacin - are provided. This dosage form provides good absorption of substances from the skin surface and their therapeutic efficacy.

In the subcutaneous tissue and periarticular tissues, close to therapeutic concentrations are created when the gel is applied to the skin. The penetration of the active components of the drug into the systemic circulation has no clinical significance.

Indications for use

Gel Troxymetacin is prescribed for the symptomatic treatment of the following diseases / conditions:

  • varicose veins (chronic venous insufficiency of the lower extremities) - as part of complex therapy, to relieve pain, a feeling of heaviness and swelling of the legs;
  • periarthritis, fibrositis, bursitis, tendovaginitis (rheumatic lesions of soft tissues);
  • post-phlebitis conditions, superficial phlebitis, thrombophlebitis;
  • postoperative edema, sprains, dislocations, contusions.

Contraindications

Absolute:

  • violation of the integrity of the skin;
  • III trimester of pregnancy;
  • lactation period;
  • age up to 14 years (due to the lack of clinical experience in using the gel);
  • established hypersensitivity to drug components and non-steroidal anti-inflammatory drugs (NSAIDs).

Relative (the use of Troxymetacin requires special care and careful medical supervision):

  • the presence of polyps on the nasal mucosa;
  • allergic rhinitis;
  • bronchial asthma;
  • combination therapy with other NSAIDs.

Troxymetacin, instructions for use: method and dosage

Troxymetacin gel is intended for topical use.

A single dose should be applied in a thin layer and rubbed into the skin of painful areas of the body. For one procedure, 4–5 cm of gel squeezed out of the tube is enough. The frequency of application of the drug is 3-4 times a day.

The maximum daily amount of gel should not exceed 20 cm. The duration of the course of treatment is no more than 10 days.

Side effects

Troxymethacin is very well tolerated.

Local reactions are possible in the form of manifestations of hypersensitivity from the skin: rash, itching, redness, contact dermatitis, a feeling of warmth and burning at the site of application of the gel.

Overdose

With topical use of Troxymetacin, no cases of overdose have been reported.

Patients who accidentally swallow a large amount of the drug may develop a burning sensation in the mouth, nausea, vomiting, and profuse salivation.

It is recommended to flush the mouth and stomach, symptomatic treatment is prescribed if necessary.

It is important to apply Troxymetacin only to intact skin areas and avoid contact with mucous membranes and eyes. An occlusive dressing should not be applied over the gel.

There is no clinical experience of using Troxymetacin during pregnancy and during breastfeeding, sufficient to confirm its safety in these cases. The drug should be used only when its potential therapeutic benefit outweighs the expected risks to the fetus or newborn.

Pediatric use

The drug is not prescribed to children under the age of 14, since there is no clinical experience with its use in patients of this age group.
