Release form and composition

Dosage form - tablets, film-coated: oblong shape with rounded ends, biconvex, pink, with a line (5, 10, 12, 15, 20 or 30 pcs. In blisters, in a cardboard box 1, 2, 5, 10 or 14 packs and instructions for use of Analgin-Ultra).

Composition of 1 film-coated tablet:

  • active substance: metamizole sodium - 500 mg;
  • auxiliary components: hypromellose (hydroxypropyl methylcellulose), microcrystalline cellulose, potato starch, magnesium stearate, crospovidone, talc, colloidal silicon dioxide (aerosil);
  • shell composition: Opadray II [talc, macrogol, titanium dioxide, polyvinyl alcohol, pigments (aluminum oxides)].

Pharmacological properties

Pharmacodynamics

The active ingredient in the composition of Analgin-Ultra is metamizole sodium - a pyrazolone derivative, a non-narcotic analgesic agent. It has an analgesic, weak anti-inflammatory, antipyretic effect.

The mechanism of action of metamizole sodium is due to its ability to non-selectively block cyclooxygenase, reduce the formation of prostaglandins from arachidonic acid, and prevent painful extra- and proprioceptive impulses.

The drug has an antispasmodic effect on the smooth muscles of the biliary and urinary tract. It has a weak anti-inflammatory activity, which explains its small effect on the mucous membrane of the gastrointestinal tract, as well as on water-salt metabolism (manifested by water and sodium retention).

The action of metamizole sodium is manifested within 20-40 minutes after taking Analgin-Ultra.

Pharmacokinetics

Once in the gastrointestinal tract, metamizole sodium is rapidly and well absorbed, thereby ensuring the rapid development of the clinical effect.

The maximum plasma concentration reaches within 1-1.5 hours after taking the drug inside. The connection with plasma proteins is about 50-60%. It is biotransformed in the intestinal wall by hydrolysis, resulting in the formation of an active metabolite 4-methyl-amino-antipyrine, which is also metabolized to form 4-formyl-amino-antipyrine and other metabolites.

It is excreted in the form of metabolites through the kidneys.

Metamizole sodium and its metabolites pass into breast milk.

Indications for use

  • pain syndrome of mild and moderate severity: myositis, arthralgia, myalgia, radiculitis, neuralgia, trauma, headache of various etiologies, algodismenorrhea;
  • pain of visceral origin: intestinal, renal and hepatic colic (in combination with antispasmodics);
  • pain after diagnostic and surgical interventions;
  • fever due to infectious and inflammatory diseases.

Contraindications

  • anemia, including hereditary hemolytic anemia, associated with a deficiency of glucose-6-phosphate dehydrogenase;
  • hepatic and / or renal failure;
  • oppression of hematopoiesis (agranulocytosis, neutropenia);
  • leukopenia;
  • a history of asthma caused by the intake of acetylsalicylic acid, salicylates, or other nonsteroidal anti-inflammatory drugs;
  • age up to 15 years;
  • pregnancy (especially the first trimester and the last 6 weeks);
  • lactation period;
  • hypersensitivity to any component of Analgin-Ultra or other pyrazolone derivatives.

With caution, Analgin-Ultra should be used in patients with kidney disease (pyelonephritis, glomerulonephritis - currently or in history), moderately severe renal or liver dysfunction, bronchial asthma, a predisposition to the development of arterial hypotension, as well as patients who for a long time time to abuse alcohol.

Analgin-Ultra, instructions for use: method and dosage

Analgin-Ultra tablets should be taken orally, after meals.

The recommended dose for adults is 1 tablet 2 or 3 times a day.

The maximum allowable doses: single - 1000 mg (2 tablets), daily - 3000 mg (6 tablets).

Unless otherwise prescribed by the doctor, it is allowed to take 5 days as an anesthetic drug, and 3 days as an antipyretic drug.

Side effects

  • allergic reactions: urticaria (including on the mucous membranes of the nasopharynx and conjunctiva), angioedema; rarely - anaphylactic shock, bronchospastic syndrome, toxic epidermal necrolysis (Lyell's syndrome), malignant exudative erythema (Stevens-Johnson syndrome);
  • from the urinary system: urine staining red, proteinuria, anuria, oliguria, interstitial nephritis, renal dysfunction;
  • on the part of the hematopoietic organs: leukopenia, agranulocytosis, thrombocytopenia;
  • others: lowering blood pressure.

Overdose

Possible symptoms: tachycardia, decreased blood pressure, hypothermia, shortness of breath, oliguria, gastralgia, nausea, vomiting, drowsiness, tinnitus, impaired consciousness, delirium, hemorrhagic syndrome, respiratory muscle paralysis, convulsions, acute agranulocytosis, acute renal and / or hepatic failure.

In case of taking an excessive dose of Analgin-Ultra, you should make a gastric lavage, take activated charcoal and a saline laxative. If necessary, forced diuresis and hemodialysis are performed. With convulsive syndrome, intravenous administration of fast-acting barbiturates and diazepam is indicated.

Patients who receive concomitant therapy with cytostatics should be treated under strict medical supervision.

During long-term therapy, the peripheral blood picture should be monitored.

Analgin-Ultra can cause the development of agranulocytosis. Patients should be warned about the need to immediately discontinue the drug and seek medical attention if symptoms such as unmotivated fever, chills, difficulty swallowing, sore throat, stomatitis appear. Discontinuation of therapy is also required if signs of vaginitis or proctitis are detected.

Patients with pollinosis and atopic bronchial asthma have an increased risk of developing allergic reactions.

Analgin-Ultra is forbidden to take for acute abdominal pains until the exact cause of their occurrence is established.

Application during pregnancy and lactation

Analgin-Ultra is contraindicated for use during pregnancy (especially in the first trimester and in the last 6 weeks), as well as during breastfeeding.

Pediatric use

Analgin-Ultra is prohibited for children and adolescents under 15 years of age.

With impaired renal function

  • with caution: kidney disease such as pyelonephritis, glomerulonephritis (including a history), as well as moderate renal dysfunction;
  • contraindicated: renal failure.

For violations of liver function

  • with caution: moderately severe liver dysfunction;
  • contraindicated: liver failure.

Drug interactions

  • other non-narcotic analgesics, hormonal contraceptives, tricyclic antidepressants, allopurinol: possible increased toxicity;
  • anxiolytics and sedatives: the analgesic effect of metamizole sodium is enhanced;
  • cyclosporine: its concentration in the blood decreases;
  • inducers of microsomal liver oxidation (including barbiturates), phenylbutazone: the effect of metamizole sodium decreases;
  • histamine H 2 -blockers, codeine, propranolol: the effect of sodium metamizole is enhanced;
  • corticosteroids, indirect anticoagulants, oral hypoglycemic drugs, indomethacin: their activity increases;
  • phenothiazine, chlorpromazine: development of severe hyperthermia is possible;
  • ethanol: its effects are enhanced.

During the period of taking Analgin-Ultra, the use of penicillin, colloidal blood substitutes and X-ray contrast drugs is prohibited.
