Vivorax is a highly selective antiviral drug. Experimentally (in vitro) proved the effectiveness of the drug against herpes simplex viruses of the 1st and 2nd types, varicella zoster virus (Varicella zoster); in higher doses, acyclovir inhibits type 4 human herpesvirus (Human gammaherpesvirus 4, the so-called Epstein-Barr virus). In clinical trials (in vivo), therapeutic and prophylactic efficacy has been proven in the treatment of viral infections caused by Herpes simplex.

Release form and composition

Dosage forms of Vivorax:

  • tablets: blue, round, flat-cylindrical, with beveled edges, on one side - risk (10 pcs. in aluminum strips, in a cardboard box 1 strip);
  • cream for external use 5%: homogeneous, from almost white to white (5 g each in aluminum tubes with a screw cap, placed in a transparent polystyrene container, in a cardboard box 1 container).

Composition of 1 tablet:

  • active substance: acyclovir - 0.2 g;
  • auxiliary components: lactose monohydrate, corn starch, magnesium stearate, indigo carmine dye (E 132).

Composition of 100 g cream:

  • active substance: acyclovir - 5 g;
  • auxiliary components: liquid paraffin and white soft paraffin, sorbitol, sodium lauryl sulfate solution 70%, methyl parahydroxybenzoate and sodium propyl parahydroxybenzoate, stearic alcohol, purified water.

Indications for use

Vivorax tablets (systemic use):

  • infections of the skin and mucous membranes (primary and secondary) caused by herpes simplex viruses of the 1st and 2nd types, including genital herpes - for therapy;
  • recurrent infections caused by herpes simplex viruses of the 1st and 2nd types, in patients with normal immune status - to prevent exacerbations;
  • infections caused by herpes simplex viruses of the 1st and 2nd types in patients with immunodeficiency - for the prevention of primary infection and relapse;
  • infections of the skin and mucous membranes caused by herpes simplex viruses of the 1st and 2nd types with severe immunodeficiency after bone marrow transplantation and due to HIV infection (stages of AIDS from early clinical manifestations to a detailed clinical picture) - as part of complex therapy;
  • infections caused by the varicella zoster virus (Varicella zoster) and shingles (Herpes zoster) - for the treatment of primary infection and relapse.

Vivorax cream (topical application) is recommended for the treatment of skin lesions caused by herpes simplex viruses type 1 and 2, including herpes of the lips.

Contraindications

All forms of Vivorax are contraindicated for use in case of revealed increased individual sensitivity to acyclovir, famciclovir, valacyclovir, ganciclovir or excipients of the drug.

The tablets are also contraindicated during breastfeeding (lactation) and in children under 3 years of age.

The tablets are taken with caution during pregnancy, in old age and in large doses, especially with dehydration, impaired renal function, neurological disorders or neurological reactions to the intake of cytotoxic substances (including in history).

Vivorax cream for pregnant and lactating women is allowed to be used only if the intended benefit to the mother exceeds the potential risk to the fetus / child.

Method of administration and dosage

To get the maximum therapeutic effect from Vivorax, treatment is started as soon as possible, after the first symptoms of the disease appear, such as itching, tingling, burning, feeling of tension, redness at the sites of infection.

The tablets are taken orally during meals or immediately after a meal with plenty of water.

According to indications, the following dosing regimen is recommended for adult patients:

  • treatment of lesions of the skin and mucous membranes caused by herpes simplex viruses of the 1st and 2nd types: 5 times a day, 200 mg at 4 hour intervals during the day and 8 hour intervals at night. The duration of the course is 5 days, in more severe cases it is possible to extend the course as prescribed by the doctor up to 10 days;
  • treatment of lesions of the skin and mucous membranes caused by herpes simplex viruses of the 1st and 2nd types with severe immunodeficiency after bone marrow implantation and an expanded clinical picture of HIV infection (from early clinical manifestations to the stage of AIDS): 5 times per 400 mg daily (as part of complex therapy);
  • prevention of recurrence of infections caused by herpes simplex viruses of the 1st and 2nd types, and therapy of recurrent diseases in patients with normal immune status: 4 times a day, 200 mg with an interval between doses of 6 hours. The duration of the course is 6–12 months;
  • prevention of infections caused by herpes simplex viruses of the 1st and 2nd types in adults with immunodeficiency: 4 times a day, 200 mg with an interval between doses of 6 hours. The maximum single dose is 400 mg of acyclovir, the frequency of administration is no more than 5 times a day (depending on the severity of the disease);
  • treatment of infections caused by the varicella zoster virus (Varicella zoster): 5 times a day, 800 mg at 4 hour intervals during the day and 8 hour intervals at night. Duration of the course - 7-10 days;
  • treatment of infections caused by the herpes zoster virus: 800 mg 4 times a day with an interval of 6 hours between doses. The duration of the course is 5 days.

Recommended dosage regimen for children:

  • treatment of infections caused by herpes simplex viruses of the 1st and 2nd types, and the prevention of this infection with weakened immune protection: children over 3 years old receive an adult dose;
  • treatment of infections caused by the varicella zoster virus (Varicella zoster): up to 3 years - 4 times a day, 20 mg / kg of the child's weight (maximum single dose - 800 mg), from 3 to 6 years - 4 times a day, 400 mg, over 6 years old - 4 times a day, 800 mg. The duration of the course is 5 days.

Recommended dosing regimen for patients with impaired renal function:

  • treatment and prevention of infections caused by herpes simplex viruses in patients with creatinine clearance (CC) ≤ 10 ml / min: 2 times a day, 200 mg at 12-hour intervals;
  • treatment of infections caused by the varicella zoster virus (Varicella zoster) in patients with CC ≤ 10 ml / min: 2 times a day, 800 mg at 12-hour intervals;
  • treatment of infections caused by the varicella zoster virus (Varicella zoster) in patients with CC up to 25 ml / min: 800 mg 3 times a day at 8-hour intervals.

Cream

It is recommended to apply Vivorax cream in a thin layer to infected and adjacent skin areas. For application, use a clean cotton swab, or apply the cream directly with disinfected hands to avoid additional infection of the affected areas.

Recommended dosage: 5 applications per day at 4 hour intervals.

The duration of the course is at least 5 days. In the absence of complete healing, therapy continues for up to 10 days, and if symptoms persist for more than 10 days, you should consult a doctor.

Side effects

With systemic use of the drug Vivorax (for tablets), the following adverse reactions from organs and systems may develop:

  • gastrointestinal tract (GIT): in isolated cases - nausea / vomiting, abdominal pain, diarrhea;
  • central nervous system (CNS): rarely - weakness, headache; in isolated cases - increased fatigue, dizziness, drowsiness / insomnia, confusion, exhaustion, tremors, decreased concentration, hallucinations;
  • laboratory indicators: a slight transient increase in the activity of liver enzymes; rarely - a slight increase in creatinine and urea levels, hyperbilirubinemia, erythropenia, leukopenia;
  • allergic reactions: itching, skin rash, Lyell's syndrome, exudative erythema multiforme (including Stevens-Johnson syndrome), urticaria, fever;
  • other reactions: rarely - visual impairment, peripheral edema, alopecia, lymphadenopathy.

With local application of Vivorax cream, short-term skin reactions are possible in the form of redness, itching, peeling, burning or tingling in the areas of application. In extremely rare cases, allergic dermatitis may develop, often associated with a reaction to the auxiliary components of the cream. There are also isolated data on cases of Quincke's edema.

An overdose of the drug is unlikely, since even with oral administration of 20 g of acyclovir, specific symptoms were not observed. It is theoretically possible to develop side effects from the central nervous system - tremors, convulsions, lethargy. In this case, it is recommended to induce vomiting (preferably no later than half an hour after the overdose) and take absorbents. It is important for patients with acute renal failure and anuria to ensure the maintenance of vital functions and to undergo hemodialysis. Peritoneal dialysis is less effective in this situation.

Overdose with topical application of the drug is unlikely.

special instructions

Long-term or repeated therapy with acyclovir in patients with reduced immunity may cause the appearance of viral strains insensitive to its action. Therefore, Vivorax should be used systemically only as directed by a doctor, strictly following all his recommendations in order to avoid complications.

In the process of treatment, it is necessary to ensure that a sufficient amount of fluid is supplied to the patient's body.

When taking tablets, such indicators of renal function as the concentration of urea and creatinine in the blood plasma should be monitored.

Medical personnel should warn patients that taking acyclovir does not interfere with the transmission of herpes through sexual contact, therefore, during treatment, it is necessary to refrain from unprotected sexual intercourse even in the absence of clinical manifestations of the disease.

Before starting local treatment with Vivorax cream of pronounced manifestations of herpes of the lips, you should consult your doctor.

The cream should not be applied to the mucous membranes of the eyes and oral cavity, as this can cause local inflammation.

Drug interactions

Vivorax tablets (systemic use):

  • probenecid: increases the mean half-life and decreases the clearance of acyclovir;
  • nephrotoxic drugs: the likelihood of impaired renal function increases.

Interactions with other medicinal substances / preparations in the case of topical application of Vivorax cream have not been identified.
