Release form and composition
Dosage form of release - tablets: white, biconvex, round, with a line on one side [10 pcs. in a blister made of aluminum foil and PVC film (polyvinyl chloride), in a cardboard box 5 blisters and instructions for use of Mendilex].
Composition for one tablet:
- active ingredient: biperiden hydrochloride - 2 mg;
- additional substances: MCC (microcrystalline cellulose) - 131 mg, lactose monohydrate - 65 mg, magnesium stearate - 2 mg.
Pharmacological properties
Pharmacodynamics
The active component of Mendilex is biperiden, a central anticholinergic antagonist that has a moderately pronounced peripheral effect. As a result of selective blockade of cholinergic receptors, it weakens the signs of parkinsonism, such as dyskinetic tremors, akathisia, akinesia, muscle rigidity, and oculogyric crisis.
Pharmacokinetics
Biperiden is rapidly absorbed from the gastrointestinal tract (gastrointestinal tract). Its bioavailability is 30%, which is evidence of extensive metabolism during the first passage through the liver.
Plasma protein binding - 93%, V d (volume of distribution) - 24 ± 4.1 l / kg. The metabolism of biperiden is not fully understood. It is known for certain that the substance is involved in the hydroxylation process.
T 1/2 (half-life) of biperiden from plasma is about 20 hours. Therefore, at a concentration of 0.1–0.2 ng / ml, it is determined in plasma even two days after taking Mendilex.
Biperiden is excreted in urine and feces.
Indications for use
- parkinsonism syndrome (as part of combination therapy);
- the initial stages of Parkinson's disease (monotherapy);
- extrapyramidal disorders (muscle rigidity, tremor, torsion dystonia and local forms of dystonia, including paroxysmal dyskinesias, various forms of tics, chorea hyperkinesis, myoclonus and hyperkinesis, dystonia of the muscles of the limbs, trunk, face, neck, akathisia, akinesia), drug-provoked funds (in particular antipsychotics).
Contraindications
Absolute:
- myasthenia gravis;
- angle-closure glaucoma;
- gastroesophageal reflux;
- nonspecific ulcerative colitis;
- intestinal obstruction, paralytic intestinal obstruction or intestinal atony;
- toxic megacolon;
- obstruction of the urinary tract;
- acute bleeding;
- breast-feeding;
- hypersensitivity to the components of the drug.
Relative (Mendilex tablets are taken, taking precautions):
- hypertrophy of the prostate;
- tachyarrhythmia;
- congestive heart failure;
- Ischemic heart disease (coronary heart disease);
- arterial hypertension;
- ileostomy / colostomy;
- epilepsy;
- age up to 18 years (there is no data on the effectiveness and safety of using the drug in children and adolescents);
- pregnancy;
- elderly age.
Mendilex, instructions for use: method and dosage
Mendilex tablets are intended for oral administration.
The recommended dosage regimen, according to the established diagnosis:
- Parkinson's disease and parkinsonism: the initial dose is 2 mg per day (divided into 2 doses). The dose should be gradually increased until it reaches the optimal dose, for most patients, from 3 to 12 mg per day (divided into 3-4 doses). The maximum daily dose should not exceed 16 mg;
- extrapyramidal disorders caused by taking medications (in particular antipsychotics): usually prescribed 2 mg 1-3 times a day.
You should not suddenly stop treatment with the drug due to the high likelihood of withdrawal.
Side effects
Adverse events, as a rule, developed as a result of the anticholinergic action of Mendilex.
More often than others, the appearance of mydriasis, paresis of accommodation, dry mouth, which were dose-dependent side reactions, was observed.
The development of dyspepsia, constipation, weakness, dizziness, increased fatigue, drowsiness, confusion, memory impairment, anxiety, hallucinations, disorientation in time and space, euphoria, catalepsy, motor agitation, urinary retention, lowering blood pressure (blood pressure), tachycardia, drug dependence, allergic reactions (eg, skin rash).
Overdose
An overdose of biperiden has symptoms similar to those arising from atropine poisoning, and is manifested by facial flushing, mydriasis, fever, dry mouth and upper respiratory tract, dry skin, cardiac arrhythmias, tachycardia, urinary retention, weakening of intestinal motility, delirium, anxiety, disorientation, confusion, hallucinations, ataxia, aggressiveness, and seizures. In case of severe poisoning, stupor, coma, respiratory and cardiac arrest, and death are possible.
In case of overdose, gastric lavage and symptomatic therapy are recommended. Physostigmine is prescribed as a specific antagonist at a dose of 1 mg intramuscularly (IM) or intravenously (IV) slowly. Injections at this dose can be repeated every 20 minutes, but the total amount of the drug should not exceed 4 mg per day. Since physostigmine is rapidly metabolized, in some cases additional administration is required every 1–2 hours. Children and elderly patients should be given ½ of the standard dose. The patient must be under the supervision of a physician for at least 8-12 hours after the last onset of symptoms.

With the use of biperiden, neuromuscular blockade may occur, leading to subsequent muscle weakness and paralysis.
Sudden cessation of treatment with Mendilex may lead to withdrawal symptoms.
During drug therapy, it is prohibited to consume alcohol.
In susceptible patients, when an anticholinergic drug is prescribed in recommended therapeutic doses, euphoria, motor agitation, and confusion are observed.
Influence on the ability to drive vehicles and complex mechanisms
Since Mendilex has a strong effect on psychomotor reactions, patients taking the drug should refrain from engaging in potentially hazardous activities that require quick mental and motor reactions.
Application during pregnancy and lactation
For patients carrying a child, Mendilex can be prescribed by the attending physician, provided that the expected benefit to the mother exceeds the possible risks to the fetus.
The drug is contraindicated during breastfeeding, since it is excreted in breast milk and can lead to the development of anticholinergic effects in a child. If it is necessary to prescribe a drug for breastfeeding women, it is necessary to decide on the transfer of the child to artificial feeding.
Pediatric use
Patients under 18 years of age should take Mendilex with caution, since there are no data on the effectiveness and safety of its use in children and adolescents.
With impaired renal function
There are no special dosing conditions for patients with impaired renal function.
For violations of liver function
There are no special dosing conditions for patients with impaired hepatic function.
Use in the elderly
Elderly patients should be careful when taking Mendilex.
Drug interactions
- metoclopramide: biperiden weakens its effect;
- M-anticholinergics, antiparkinsonian, antiepileptic and antihistamines: their effect is enhanced when used simultaneously with Mendilex;
- levodopa: increases the M-anticholinergic effect of biperiden;
- quinidine: increases the risk of developing dyskinesia;
- ethanol: incompatible with biperiden (combined use is not recommended).

In reviews of Mendilex, patients indicate both the strengths and weaknesses of the drug. Among the advantages, the effectiveness of the drug is noted, which helped with the first symptoms of parkinsonism, eliminated hand tremors after traumatic brain injury. Some patients have taken it successfully for the treatment of depression, despite the fact that such an indication is not included in the instructions for the drug.
