Pharmacological properties

Pharmacodynamics

Mexicor improves the contractile function of the heart muscle and the functional state of the ischemic myocardium during myocardial infarction, and also minimizes the manifestations of diastolic and systolic dysfunction of the left ventricle. The drug has antioxidant activity and the ability to inhibit processes involving free radicals (the latter become more intense in ischemic heart disease and myocardial necrosis, especially during reperfusion). It also minimizes the damaging effect of free radical particles on cardiomyocytes.

With a critical decrease in coronary blood flow, Mexicor ensures the preservation of a stable structural and functional organization of the membranes of cardiomyocytes, increases the activity of membrane enzymes - acetylcholinesterase, adenylate cyclase, phosphodiesterase. The drug enhances the activation of aerobic glycolysis, which develops during acute ischemia, promotes the restoration of mitochondrial redox processes under hypoxic conditions, and increases the production of creatine phosphate and ATP. All this ensures the integrity of the morphological structures and the normalization of the physiological functions of the ischemic myocardium.

Ethylmethylhydroxypyridine succinate facilitates the clinical course of myocardial infarction, makes the therapy more effective, promotes the early restoration of the functional activity of the left ventricular myocardium, and reduces the risk of developing intracardiac conduction disorders and arrhythmic complications. The drug normalizes metabolic processes in the ischemic myocardium, reduces the area of necrosis, improves and / or restores myocardial contractility and electrical activity, and also increases coronary blood flow in the ischemic zone, improves the rheological characteristics of blood, increases the antianginal effect of nitro drugs, minimizes the consequences reperfusion syndrome, characteristic of acute heart failure.

Mexicor is characterized by a neuroprotective effect, stabilizes cerebral blood circulation in conditions of hypoperfusion, prevents a decrease in cerebral blood flow in the postischemic reperfusion period. The drug allows you to adapt to the damaging effect of ischemic disease by slowing down the consumption of carbohydrate reserves, blocking the post-ischemic decrease in the rate of utilization of oxygen and glucose by the brain and inhibition of the progressive accumulation of lactate.

Mexicor improves and normalizes the blood supply to the brain and the metabolic processes occurring in it. Its reception has a positive effect on the rheological properties of blood and microcirculation. The drug is able to maintain the functional activity of the brain both during ischemic heart disease and in the postischemic period. The drug is characterized by a selective anxiolytic effect, not accompanied by muscle relaxation and sedation. It also eliminates anxiety, anxiety, fear, and the feeling of constant tension.

Ethylmethylhydroxypyridine succinate has nootropic properties and allows you to prevent or reduce the severity of learning and memory impairments resulting from cerebral vascular diseases (discirculatory encephalopathy, ischemic stroke) and mild to moderate cognitive disorders. The drug has an antihypoxic effect, improves performance and concentration.

Mexicor is included in the complex therapy of patients with acute circulatory disorders of the brain, which reduces the severity of clinical symptoms of stroke and facilitates the rehabilitation period.

Pharmacokinetics

When administered intravenously, the drug quickly (within 0.5-1.5 hours) passes from the bloodstream to tissues and organs, which causes a rapid decrease in its concentration in an unchanged form. When administered intramuscularly in recommended doses, the maximum level of the active substance Mexicor in the blood is reached after 30-40 minutes and is equal to 2.5-3 μg / ml. Moreover, its metabolites are found in blood plasma within 7-9 hours.

The drug is excreted from the body in the urine in glucuronoconjugated form, as well as in small concentrations unchanged.

Indications for use

  • Ischemic stroke;
  • Mild to moderate cognitive impairment of various etiologies;
  • Encephalopathy.

In addition, the use of Mexicor is shown as part of complex therapy:

  • Capsules: ischemic heart disease;
  • Solution: from the first day of acute myocardial infarction.

Contraindications

  • Severe renal and / or liver dysfunction;
  • The period of pregnancy and breastfeeding;
  • Age under 18;
  • Hypersensitivity to drug components.

Instructions for the use of Mexicor: method and dosage

Capsules:

Capsules are taken orally. The recommended dosage at the beginning of therapy is 1 capsule 3 times a day. Depending on the patient's tolerance and clinical condition, the dose of the drug should be gradually increased, while a single dose should not exceed 2 capsules, and the daily dose should not exceed 8 capsules.

The duration of use in the complex therapy of circulatory disorders of the brain and coronary heart disease should be 1.5-2 months or more. The drug is canceled gradually, by daily reducing the dose of Mexicor by 1 capsule. It is advisable to plan the repeated courses recommended by the doctor for the spring-autumn period.

As part of the complex therapy of mild or moderate cognitive disorders and discirculatory encephalopathy, the drug is prescribed 1 capsule 3-4 times a day, without limiting the period of use.

Solution for intravenous and intramuscular administration: The

drug can be administered intravenously slowly in a stream (5 minutes or more) or by drip. For infusion administration, the drug is mixed with 100-150 ml of saline or 5% dextrose (glucose) solution. In order to prevent the development of side effects, the duration of the infusion should be 0.5-1.5 hours. The recommended daily dosage is determined at the rate of 6-9 mg per 1 kg of patient weight, but not more than 800 mg per day. The resulting dose is divided into 3 (intravenous or intramuscular) administrations per day, every 8 hours, while a single dose of Mexicor should not exceed 250 mg.

From the first days of acute myocardial infarction, the use of the drug is indicated for 14 days as part of traditional therapy, consisting of the simultaneous administration of beta-blockers, nitrates, antiplatelet agents and anticoagulants, angiotensin-converting enzyme (ACE) inhibitors, thrombolytics and corresponding symptomatic agents. To achieve a clinical effect, the first 5 days are recommended for intravenous administration, after which you can switch to intramuscular administration of the solution.

Recommended dosage for ischemic stroke (as part of complex treatment): 200-300 mg 2-3 times a day as an intravenous drip during the first 2-4 days of therapy, then 100 mg 3 times a day intravenously m. The course of treatment is 10-14 days, followed by the transfer of the patient to oral administration of the drug, 1 capsule 2 times a day for the first 14 days and 1 capsule 3 times a day for the next 7 days. The duration of repeated courses and the frequency of their appointment is determined by the doctor based on clinical indications.

For the treatment of discirculatory encephalopathy in the decompensation phase, an intravenous jet or drip introduction of Mexicor is indicated, 100 mg 2-3 times a day for 14 days, then 100 mg once a day IM for the next 14 days. It is recommended to continue treatment by taking the drug orally at a dose of 1 capsule 2-4 times a day. The duration and frequency of repeated courses is determined by the attending physician. For the prevention of discirculatory encephalopathy, the patient is prescribed an intramuscular injection of a solution of 100 mg 2 times a day with a course duration of 10-14 days.

The recommended dose for mild and moderate cognitive impairment is 100-300 mg per day IM for 14 days. If necessary, therapy can be continued by taking the drug orally, 1 capsule 2-4 times a day. The frequency of admission is recommended by the doctor, while the course of treatment has no time limits.

Side effects

Long-term use of Mexicor can cause side effects:

  • Nervous system - sleep disturbances in the form of drowsiness or sleep disturbance;
  • Digestive system - flatulence; when using a solution for i / v and i / m injections - nausea.

Taking capsules is usually well tolerated, rarely their action can cause the following conditions:

  • Digestive system: dry mouth, dyspepsia, nausea, diarrhea (side effects are transient);
  • Others: allergic reactions.

The high rate of intravenous administration of the drug, especially the jet, can cause the following transient side effects:

  • Respiratory system: feeling of lack of air and discomfort in the chest, sore throat;
  • Digestive system: Metallic taste and dry mouth;
  • Others: feeling of spreading warmth throughout the body.

Overdose

According to the instructions, in case of an overdose, Mexicor causes the following symptoms: sleep disorders (insomnia), sometimes drowsiness; occasionally insignificant and short-lived (up to 1.5-2 hours) increase in blood pressure with intravenous administration.

This condition usually does not require the use of curatives, the above signs of sleep disturbance disappear on their own throughout the day. When Mexicor is taken in especially high doses, one of the following anxiolytic and hypnotic drugs should be taken orally: diazepam at a dose of 5 mg, oxazepam at a dose of 10 mg, nitrazepam at a dose of 10 mg. An excessive increase in blood pressure requires the appointment of antihypertensive drugs.

special instructions

Parenteral use of Mexicor in the treatment of acute myocardial infarction and ischemic stroke, a chronic form of cerebrovascular accident (discirculatory encephalopathy) should always be continued by taking the drug orally (1 capsule 3 times a day), this allows maintaining the achieved clinical effect. The duration of the capsule intake is determined by the doctor.

During the period of taking the capsules, the patient must be careful when driving and engaging in potentially hazardous activities, the implementation of which requires an increased speed of psychomotor reactions and concentration of attention.

Drug interactions
