Cream for external use 0.1% Locoid Lipocrem
Cream for external use 0.1% Locoid Lipocrem

Lokoid Lipokrem is a glucocorticosteroid for topical use.

Release form and composition

Dosage form - cream for external use 0.1%: uniform consistency, white (30 g in aluminum tubes, each tube is placed in a cardboard box).

Composition of 1 g of cream:

  • active substance: hydrocortisone 17-butyrate - 1 mg;
  • additional components: propyl parahydroxybenzoate, cetostearyl alcohol, soft white paraffin, benzyl alcohol, sodium citrate anhydrous, liquid paraffin, citric acid anhydrous, cetomacrogol, purified water.

Pharmacological properties

Lokoid Lipokrem is a synthetic non-halogenated glucocorticosteroid for external use.

Compared with native hydrocortisone, the activity of the drug is radically increased due to the esterification of the hydrocortisone molecule with butyrate (butyric acid residue).

Lokoid Lipokrem has a fast-onset anti-inflammatory, antipruritic and anti-edema effect.

When used in recommended doses, the drug does not suppress the hypothalamic-pituitary-adrenal system. However, with long-term use in high doses, especially under occlusive dressings, the cream can increase the level of cortisol in the blood plasma, although this phenomenon usually does not reduce the reactivity of the pituitary-adrenal system, and after the drug is discontinued, cortisol production quickly normalizes.

When used externally, hydrocortisone 17-butyrate accumulates in the epidermis (mainly in the granular layer). Only a small amount of the active substance is absorbed into the systemic circulation unchanged, therefore the systemic absorption of Lokoid Lipocrem is negligible.

Most of the drug is metabolized directly in the epidermis, and then in the liver with the formation of hydrocortisone and other metabolites.

The drug is excreted by the kidneys and through the intestines, mainly in the form of metabolites, a small part in unchanged form.

Indications for use

According to the instructions, Lokoid Lipokrem is used to treat superficial and uninfected skin diseases that are sensitive to the action of local glucocorticosteroids. In particular, the drug is used in the following cases:

  • eczema;
  • dermatitis, including contact, atopic and seborrheic;
  • psoriasis.

Lokoid Lipokrem is prescribed for subacute and chronic processes, especially in the presence of dry skin, infiltration and lichenification.

  • violation of the integrity of the skin (wounds, ulcers);
  • rosacea, acne, perioral dermatitis;
  • viral skin infections (such as chickenpox, herpes simplex, or shingles);
  • bacterial skin infections (eg, gram-negative folliculitis, streptoderma, or staphyloderma);
  • fungal infections of the skin;
  • parasitic skin infections;
  • neoplastic skin changes (benign and malignant formations);
  • syphilitic and tuberculous skin lesions;
  • post-vaccination period;
  • children's age up to 6 months;
  • hypersensitivity to any component of the drug.

Lokoid Lipokrem should be used with caution during pregnancy and lactation.

Instructions for the use of Lokoid Lipocrem: method and dosage

The drug is intended for external use.

The cream should be applied in a thin layer to the affected skin 1 to 3 times a day. After improving the condition, the frequency of applications can be reduced to 2–3 times a week.

The weekly dose of Lokoid Lipokrem should not exceed 30-60 g, depending on the area to be treated.

To improve the penetration of the cream into the epidermis, it is recommended to apply it with massaging movements.

The drug can be applied under occlusive dressings in cases of resistant course of the disease, for example, with localization of dense psoriatic plaques in the knees and elbows.

The duration of therapy is individual.

Side effects

Possible local reactions: skin irritation; in rare cases - other phenomena characteristic of external glucocorticosteroid drugs.

With prolonged therapy, applying the cream on large surfaces and / or under occlusive dressings, side effects typical of systemic glucocorticosteroids may develop.

When treating with Lokoid Lipokrem, the risk of developing local and systemic effects is lower than when using fluorinated steroids.

There have been no reports of overdose cases of Lokoid Lipocrem.

With prolonged use of the drug and / or in high doses, the severity of the above side effects may increase.

The development of symptoms of hypercortisolism is the basis for the cancellation of the therapy.

special instructions

Due to the risk of developing glaucoma, the cream should not be applied to the periorbital area. With the systematic ingestion of the drug on the conjunctiva, the likelihood of an increase in intraocular pressure increases.

It is undesirable to use Lokoid Lipokrem in the presence of atrophic changes in the skin and acute inflammation (especially exudative), and also apply to the scalp.

For the treatment of steroid-sensitive skin lesions localized on areas of the skin with abundant hair, in the face, folds or genitals, it is preferable to use other dosage forms of the drug [cream (Locoid) or emulsion (Locoid Crelo)].

With hyperkeratosis and very severe infiltration, it is preferable to use hydrocortisone-based ointments.

As with the treatment of any other glucocorticosteroid drug, when prescribing Lokoid Lipocrem, it is recommended to limit the duration of therapy and apply the minimum effective dose that allows to stop skin inflammation.

If there is no effect within 2 weeks of regular use of the cream, you should consult a doctor to clarify the diagnosis.

When treating children, with prolonged use of the drug, its application under occlusive dressings or on large areas of the body, the risk of developing local and systemic side effects increases.

Influence on the ability to drive vehicles and complex mechanisms

There is no information about the negative effect of the components of the drug Lokoid Lipokrem on the patient's ability to concentrate, the speed of his mental and physical reactions.

Application during pregnancy and lactation

Hydrocortisone crosses the placenta. The effect of the drug can be especially pronounced if it is used to treat large areas of the skin.

Whether hydrocortisone is excreted in breast milk is unknown.

Given these data, during pregnancy and lactation Lokoid Lipocrem can be used only as directed by a doctor, strictly adhering to his prescriptions.

Pediatric use

The drug can be administered to children at least 6 months of age.

The surface area of the skin on which the preparation will be applied should not exceed 20% of the total area.

The course of treatment should be short-lived if the cream is applied on the face or under an occlusive dressing.

It should be borne in mind that in childhood, a decrease in the excretion of growth hormone is possible, as well as a more rapid suppression of the function of the adrenal cortex. Long-term treatment requires careful monitoring of plasma cortisone levels, height and body weight of the child.
