Release form and composition

Dosage form - cream for external use 0.1%: uniform consistency, white (30 g in aluminum tubes, each tube is placed in a cardboard box).

Composition of 1 g of cream:

  • active substance: hydrocortisone 17-butyrate - 1 mg;
  • additional components: propyl parahydroxybenzoate, cetostearyl alcohol, soft white paraffin, benzyl alcohol, sodium citrate anhydrous, liquid paraffin, citric acid anhydrous, cetomacrogol, purified water.

Pharmacological properties

Pharmacodynamics

Lokoid Lipokrem is a synthetic non-halogenated glucocorticosteroid for external use.

Compared with native hydrocortisone, the activity of the drug is radically increased due to the esterification of the hydrocortisone molecule with butyrate (butyric acid residue).

Lokoid Lipokrem has a fast-onset anti-inflammatory, antipruritic and anti-edema effect.

When used in recommended doses, the drug does not suppress the hypothalamic-pituitary-adrenal system. However, with long-term use in high doses, especially under occlusive dressings, the cream can increase the level of cortisol in the blood plasma, although this phenomenon usually does not reduce the reactivity of the pituitary-adrenal system, and after the drug is discontinued, cortisol production quickly normalizes.

Pharmacokinetics

When used externally, hydrocortisone 17-butyrate accumulates in the epidermis (mainly in the granular layer). Only a small amount of the active substance is absorbed into the systemic circulation unchanged, therefore the systemic absorption of Lokoid Lipocrem is negligible.

Most of the drug is metabolized directly in the epidermis, and then in the liver with the formation of hydrocortisone and other metabolites.

The drug is excreted by the kidneys and through the intestines, mainly in the form of metabolites, a small part in unchanged form.

Indications for use

According to the instructions, Lokoid Lipokrem is used to treat superficial and uninfected skin diseases that are sensitive to the action of local glucocorticosteroids. In particular, the drug is used in the following cases:

  • eczema;
  • dermatitis, including contact, atopic and seborrheic;
  • psoriasis.

Lokoid Lipokrem is prescribed for subacute and chronic processes, especially in the presence of dry skin, infiltration and lichenification.

Contraindications

Absolute:

  • violation of the integrity of the skin (wounds, ulcers);
  • rosacea, acne, perioral dermatitis;
  • viral skin infections (such as chickenpox, herpes simplex, or shingles);
  • bacterial skin infections (eg, gram-negative folliculitis, streptoderma, or staphyloderma);
  • fungal infections of the skin;
  • parasitic skin infections;
  • neoplastic skin changes (benign and malignant formations);
  • syphilitic and tuberculous skin lesions;
  • post-vaccination period;
  • children's age up to 6 months;
  • hypersensitivity to any component of the drug.

Lokoid Lipokrem should be used with caution during pregnancy and lactation.

Instructions for the use of Lokoid Lipocrem: method and dosage

The drug is intended for external use.

The cream should be applied in a thin layer to the affected skin 1 to 3 times a day. After improving the condition, the frequency of applications can be reduced to 2–3 times a week.

The weekly dose of Lokoid Lipokrem should not exceed 30-60 g, depending on the area to be treated.

To improve the penetration of the cream into the epidermis, it is recommended to apply it with massaging movements.

The drug can be applied under occlusive dressings in cases of resistant course of the disease, for example, with localization of dense psoriatic plaques in the knees and elbows.

The duration of therapy is individual.

Side effects

Possible local reactions: skin irritation; in rare cases - other phenomena characteristic of external glucocorticosteroid drugs.

With hyperkeratosis and very severe infiltration, it is preferable to use hydrocortisone-based ointments.
