Itopra is a drug that stimulates the release of acetylcholine, increasing motility and tone of the gastrointestinal tract (GIT).

Release form and composition

The drug is available in the form of film-coated tablets: almost white or white, biconvex, round, engraved with "ID 50" on one of the sides, the core is white or almost white (10 pcs. In blisters, in cardboard pack of 1, 2, 3, 4, 5 or 10 packs and instructions for use of Itopra).

1 tablet contains:

  • active substance: itoprid hydrochloride - 50 mg;
  • auxiliary components: lactose monohydrate, sodium carmellose, corn starch, magnesium stearate, hyprolose:
  • film shell: opadry OY-S-7223 (macrogol-400, titanium dioxide, hypromellose, hyprolosis).

Pharmacological properties

Pharmacodynamics

Itopra is a drug designed to stimulate gastrointestinal motility. Its active ingredient - itopride, has antiemetic, cholinomimetic, prokinetic and anti-dopamine activity. Antagonism of itopride in relation to dopamine D 2 -receptors and inhibition of acetylcholinesterase by it cause an increase in the release and suppression of the destruction of acetylcholine.

The interaction of itopride with D 2 receptors located in the trigger zone provides the drug with an antiemetic effect. Itopride suppresses vomiting caused by high doses of apomorphine.

The specific effect of the drug on the upper gastrointestinal tract helps to activate the propulsive motility of the stomach, improve its emptying, and accelerate transit.

Serum gastrin concentrations are not affected by itoprid.

Pharmacokinetics

After oral administration, itopride hydrochloride is rapidly absorbed in the gastrointestinal tract. The relative bioavailability is 60%. The maximum concentration (C max) in the blood plasma after taking 50 mg of itopride is achieved in 0.5–0.75 hours and is 0.28 μg / ml. The pharmacokinetics of the drug when taken for 7 days at a dose of 50 to 200 mg 3 times a day remains linear, its cumulation is minimal.

Plasma protein binding (mainly albumin) - 96%, with alpha 1- acid glycoprotein - up to 15%.

The volume of distribution (V d) - 6.1 l / kg, distributed in the stomach, small intestine, kidneys, adrenal glands, liver. When taking therapeutic doses of the drug, a small amount of itopride hydrochloride penetrates into the brain and spinal cord, breast milk.

The drug is metabolized in the liver under the action of flavin-dependent monooxygenase to form three identified metabolites, one of which exhibits insignificant activity.

Excretion of itopride hydrochloride and its metabolites mainly occurs through the kidneys. The half-life (T 1/2) is 6 hours, with trimethylaminuria, an increase in T 1/2 is observed.

Indications for use

The use of Itopra is indicated for the symptomatic treatment of functional non-ulcer dyspepsia (chronic gastritis), including the relief of conditions such as flatulence (bloating), nausea, vomiting, discomfort or pain in the epigastric region (gastralgia), heartburn, feeling of fast satiety, anorexia.

Contraindications

  • gastrointestinal bleeding;
  • perforation and mechanical obstruction of the digestive tract;
  • lactase deficiency, glucose-galactose malabsorption syndrome, lactose intolerance (due to the presence of lactose in the preparation);
  • age under 16;
  • period of pregnancy;
  • breast-feeding;
  • hypersensitivity to the components of the drug.

Care should be taken when prescribing Itopra tablets to patients in whom an increase in the action of acetylcholine can cause the appearance of unwanted cholinergic reactions that aggravate the course of concomitant diseases, as well as to elderly patients with impaired renal and / or liver function.

Itopra, instructions for use: method and dosage

Itopra tablets are taken orally, before meals.

Recommended dosage: 1 pc. 3 times a day.

Taking into account the patient's age, the daily dose of 150 mg may be reduced.

Side effects

  • from the nervous system: headache, tremor, dizziness;
  • from the endocrine system: an increase in the concentration of prolactin, gynecomastia;
  • from the immune system: the development of allergic reactions in the form of itching, rash, hyperemia, anaphylactic shock;
  • from the digestive system: nausea, increased salivation, abdominal pain, diarrhea, constipation, jaundice;
  • on the part of the hematopoietic organs: thrombocytopenia, leukopenia;
  • laboratory parameters: an increase in the level of bilirubin, the activity of alanine aminotransferase (ALT), aspartate aminotransferase (ACT), alkaline phosphatase (ALP), gamma-glutamyl transpeptidase (GGT).

  • M-anticholinergics: reduce the therapeutic efficacy of itopride;
  • cholinesterase inhibitors, M-anticholinergics: may increase the cholinergic effect of itopride.

Special care should be taken with concomitant therapy with oral dosage forms. An increase in gastric motility due to the action of itopride hydrochloride can negatively affect the absorption of agents with a low therapeutic index, a slow release of the active substance or an enteric coating. In this regard, when prescribing Itopra, the patient must inform the doctor about the use of other drugs.
