Doxal is a semi-synthetic antibacterial drug.
Release form and composition
Doxal is available in the form of capsules (10 pcs. In blisters, in a cardboard box one blister together with instructions for use).
1 capsule contains the active ingredient: doxycycline hyclate - 100 mg.
Pharmacological properties
Pharmacodynamics
Doxycycline is a semi-synthetic antibiotic with a broad spectrum of bacteriostatic action from the group of tetracyclines. Blocks protein synthesis at the stage of fixation of activated amino acids on the ribosomal membrane of microbes. It is active for most gram-negative, gram-positive bacteria, leptospira, spirochetes, pathogens of trachoma, rickettsia, mycoplasma, psittacosis, psittacosis, as well as some protozoa.
Pharmacokinetics
Doxal is almost completely and in a short time absorbed from the gastrointestinal tract (GIT). Food intake has no significant effect on its absorption. Plasma protein binding is 80–95%. The half-life (T 1 / 2) ranges from 12 to 22 hours. The main part of the drug is excreted unchanged in the feces (due to the secretion of bile), up to 40% of the dose can be excreted in the urine unchanged. The drug is widely distributed in body fluids and tissues.
Indications for use
Doxal is recommended for the treatment of infectious and inflammatory diseases caused by microorganisms sensitive to doxycycline: respiratory system, including ENT organs; Gastrointestinal tract; skin and soft tissues, including acne or acne; genitourinary system, including primary and secondary syphilis, gonorrhea; other infections such as brucellosis, typhus, rickettsioses, trachoma, osteomyelitis, chlamydia.
Contraindications
Contraindications for taking Doxal are:
- leukopenia;
- porphyria;
- severe liver failure;
- pregnancy and lactation;
- children under 8 years old;
- hypersensitivity to doxycycline, other tetracyclines and to any of the auxiliary components of the drug.
Instructions for the use of Doxal: method and dosage
The drug is taken orally during meals.
For children weighing 50 kg or less, Doxal is prescribed at the rate of 4 mg / kg on the first day in one dose, or the dose is divided into two equal parts and taken at intervals of 12 hours. On the following days, the drug is taken at the rate of 2 mg / kg of weight. In cases of severe disease, the daily dose is increased to 4 mg / kg body weight.
Adults and children weighing more than 50 kg, as well as elderly patients, are prescribed 2 capsules on the first day. On the following days, 1 capsule per day. In cases of severe disease, the dose of the drug can be increased to 2 capsules per day.
In primary and secondary syphilis, Doxal is taken in a course of 300 mg per day, for 10 days.
For gonococcal urethritis in women, the drug is prescribed at a dose of 100 mg 2 times a day. For men, 300 mg at a time, or 100 mg for 2 doses per day. The course of treatment is 2-4 days.
For the treatment of acne, the drug is prescribed at 50 mg per day, the duration of the course of treatment is 6-12 weeks.
The maximum allowable dose for adults is 300-600 mg / day, depending on the etiology of the pathogen.
Side effects
- hematopoietic system: hemolytic anemia, thrombocytopenia, neutropenia;
- digestive system: abdominal pain, diarrhea, constipation, nausea, vomiting, anorexia, dysphagia, esophagitis, glossitis, transient increase in blood levels of hepatic transaminases, bilirubin, alkaline phosphatase (ALP);
- allergic reactions: itching, skin rash, eosinophilia; rarely - Quincke's edema, photosensitivity;
- others: intestinal dysbiosis, candidiasis, an increase in residual nitrogen, in children - discoloration of the teeth.
special instructions
To reduce the risk of irritating effects of the drug on the gastrointestinal tract, it is recommended to take Doxal during the day and drink it with a large volume of water or milk. During treatment, as well as within 4–5 days after it, it is necessary to limit insolation, due to the possible development of photosensitization.
Overdose
There is no information on overdose.
Application during pregnancy and lactation
Doxal should not be taken by pregnant women.
If it is necessary to use the drug during lactation, breastfeeding is recommended to be interrupted.
Pediatric use
The drug is not recommended for children under 8 years of age due to the fact that it causes hypoplasia of tooth enamel, long-term discoloration of the teeth, as well as slowing down the longitudinal growth of the bones of the skeleton.
With impaired renal function
No dose adjustment is required in patients with impaired renal function.
For violations of liver function
It is recommended to prescribe Doxal with caution to patients with impaired liver function.
Use in the elderly
In elderly patients, the dose of Doxal on the first day is 2 capsules. On the next day, 1 capsule. In cases of severe disease, the dose of the drug can be increased to 2 capsules per day.
Drug interactions
It is recommended to avoid co-administration of Doxal with medicinal products containing metal ions, as they form inactive chelates in combination with doxycycline.
The simultaneous use of Doxal with penicillins and cephalosporins is not recommended, since they are antagonists of bacteriostatic antibiotics, including doxycillin.
Simultaneous administration of the drug with carbamazepine, barbiturates and phenytoin reduces the concentration of doxycycline in the blood plasma due to the induction of microsomal liver enzymes. As a result, the antibacterial effect of Doxal is reduced.
Joint reception with retinol provokes an increase in intracranial pressure.
The drug increases the risk of breakthrough bleeding when taking estrogen-containing oral contraceptives, and also reduces the reliability of contraception.
Doxycycline suppresses the intestinal microflora, which leads to a decrease in the prothrombin index and requires a dose adjustment of indirect coagulants.
An interval of at least 3 hours should be observed between taking Doxal with cholestyramine and colestipol to avoid a decrease in the absorption of doxycycline.
