Release form and composition
Dosage form - ointment for external use: almost white in color, with a soft homogeneous structure, without visible inclusions (30 g each in aluminum tubes, in a cardboard box 1 tube).
Content of active substances in 1 g of ointment:
- Betamethasone dipropionate - 0.64 mg, of which betamethasone - 0.50 mg;
- Salicylic acid - 30 mg
Auxiliary components: petroleum jelly, liquid paraffin.
Pharmacological properties
Pharmacodynamics
The effectiveness of the ointment for external use Diprosalik is provided by the pharmacological action of its active components:
- betamethasone dipropionate - synthetic fluorinated GCS (glucocorticosteroid), has antipruritic, anti-inflammatory and vasoconstrictor effect;
- salicylic acid (applied topically) is a chemical compound that provides a keratolytic effect by inhibiting the proliferation of keratinocytes, facilitating the softening and subsequent dissolution of the stratum corneum, as well as having bacteriostatic and some fungicidal efficacy.
Indications for use
The use of Diprosalik ointment is indicated for the removal of inflammatory manifestations in the treatment of hyperkeratic and dry dermatoses sensitive to glucocorticosteroids (GCS), including:
- Psoriasis;
- Lichen planus;
- Chronic atopic dermatitis;
- Dyshidrosis;
- Seborrheic dermatitis of the scalp;
- Neurodermatitis (chronic lichen simplex);
- Eczema, including hand eczema, eczematous dermatitis, coin eczema;
- Ichthyosis and other ichthyosis-like conditions.
Contraindications
- Breastfeeding period;
- Age up to 2 years;
- History of hypersensitivity to drug components.
According to the instructions, Diprosalik is prescribed with caution: during pregnancy (use is considered justified if the expected therapeutic effect for the mother outweighs the potential risk to the fetus); children over 2 years old.
Instructions for the use of Diprosalik: method and dosage
Diprosalik ointment is applied in a thin layer to the skin, completely covering the affected areas. The procedures are recommended to be carried out 2 times a day - in the morning and in the evening before going to bed.
For some clinical indications, it is recommended to carry out maintenance therapy using applications 1 time per day or less.
Side effects
The use of Diprosalik can cause the following side effects: itching, burning, dryness and / or irritation of the skin, perioral dermatitis, hypertrichosis, folliculitis, hypopigmentation, acne-like rashes, allergic contact dermatitis.
When using occlusive dressings, such undesirable phenomena as secondary infection, prickly heat, skin maceration, striae, and skin atrophy often occur.
The use of salicylic acid products can cause the development of dermatitis.

Overdose
Symptoms of an overdose of Diprosalik with prolonged use of high doses may be inhibition of the function of the pituitary-adrenal system with secondary insufficiency of adrenal function and hypercortisolism (including with Cushing's syndrome - as a consequence of the content of GCS in the preparation), as well as poisoning with salicylic acid due to it the presence in the composition of the ointment.
Appropriate symptomatic therapy is recommended. Usually acute symptoms of hypercortisolism are reversible; if necessary, the electrolyte imbalance is corrected, and in case of chronic toxicity - the gradual abolition of GCS. An overdose of salicylates also requires symptomatic treatment. In addition, it is necessary to take measures for their fastest excretion from the body, for example, forced diuresis and / or sodium bicarbonate ingestion to alkalize urine.

The appointment of Diprosalik ointment during pregnancy is made in the minimum effective dose, for a short period.
In case of development of hypersensitization or irritation during treatment with ointment, the use of the drug should be canceled.
The withdrawal of the drug after prolonged use should be carried out gradually.

Do not use Diprosalik in ophthalmology, it is necessary to avoid contact with mucous membranes, including the eyes.
Systemic absorption increases when the drug is applied to large areas and when using occlusive dressings.

It should be borne in mind that the use of the ointment, especially in pediatric patients, may be accompanied by side effects, including functional suppression of the hypothalamic-pituitary-adrenal system. Suppression of the function of the hypothalamic-pituitary-adrenal system is determined by a decrease in the level of cortisol in the blood plasma and the lack of response to stimulation of adrenocorticotropic hormone. Signs of the negative effect of the drug are also insufficient weight gain and stunting of the child, Cushing's syndrome, increased intracranial pressure.
Application during pregnancy and lactation
The safety of using local GCS during pregnancy has not been established, and therefore their appointment during gestation is justified only if the potential benefit to the mother outweighs the likely risk to the fetus; also, you should not use drugs of this class in high doses or for a long time.
Since there are no data from studies to study the level of systemic absorption of GCS when applied topically and their presence in detectable concentrations in human milk, breastfeeding should be discontinued if Diprosalik therapy is necessary.
Pediatric use
In pediatric practice, the use of Diprosalik in children under the age of 12 is allowed only under strict medical supervision. As a result of local application of GCS, the child's body is more at risk of inhibition of the function of the hypothalamic-pituitary-adrenal system, due to increased absorption of the drug, due to the greater value of the ratio of the child's body surface area to its weight.
