Release form and composition
Diabeton is produced in the form of tablets (30 pcs. In blisters, 2 blisters in a cardboard box).
The active ingredient is a part of 1 tablet: gliclazide - 80 mg.
Pharmacological properties
Pharmacodynamics
The active ingredient of Diabeton is gliclazide - a derivative of II generation sulfonylurea, hypoglycemic action. It is effective in metabolic, latent diabetes mellitus, including in patients with exogenous constitutional obesity.
Gliclazide improves glucose utilization and increases the secretion of insulin by the beta cells of the pancreas. Stimulates the activity of muscle glycogen synthetase.
The drug normalizes the glycemic profile within a few days of treatment. It shortens the time from meal intake to onset of insulin secretion, restoring the early peak in insulin secretion. Reduces food-induced hyperglycemia. Improves rheological properties of blood, hematological parameters, microcirculation and hemostasis. Prevents the development of microvasculitis, including lesions of the retina of the eye. Provides continuous blood flow in the capillaries, improves vascularization of the conjunctiva, eliminates the signs of microconstriction. Shows antioxidant properties. Significantly increases the index of relative disaggregation, inhibits platelet aggregation, increases fibrinolytic and heparin activity, and increases heparin tolerance.
It has been reliably established that gliclazide slows down the development of diabetic retinopathy. In the case of long-term use in this disease, the drug does not affect renal function, while reducing proteinuria, improves control of blood glucose and blood pressure. Does not contribute to an increase in body weight, on the contrary, in obese patients it reduces it (subject to an appropriate diet). Does not cause hyperinsulinemia.
According to the results of experiments on animals (rabbits and rats), during which chronic and specific types of toxicity of gliclazide were studied, no signs of carcinogenicity, teratogenicity and mutagenicity, as well as a negative effect on fertility, were revealed.
Pharmacokinetics
After taking Diabeton inside, gliclazide is rapidly and completely absorbed from the gastrointestinal tract. The maximum concentration reaches within 2-6 hours. Equilibrium plasma concentration is observed after 2 days. It binds to plasma proteins by 85–99%. The volume of distribution is 13-24 liters.
With a single dose, the duration of action is maintained for 24 hours.
It is metabolized in the liver by oxidation, hydroxylation and glucuronidation to form 8 inactive metabolites, one of which has a pronounced effect on microcirculation.
The half-life is 8-12 hours. The drug is excreted mainly in the form of metabolites (unchanged less than 1%): by the kidneys with urine - 65%; through the intestines - 12%.
Indications for use
According to the instructions, Diabeton is prescribed for the treatment of non-insulin dependent diabetes mellitus.
Contraindications
- Juvenile diabetes;
- Comatose and precomatose states;
- Ketoacidosis;
- Severe renal and hepatic impairment;
- Pregnancy and lactation (breastfeeding);
- Hypersensitivity to the components of the drug, as well as to other sulfamides.
Instructions for the use of Diabeton: method and dosage
The tablets should be taken orally with meals.
The doctor determines the dosage regimen individually, depending on the severity of the disease. The dose can range from 1 to 4 tablets per day.
Usually, the drug is prescribed 2 tablets per day (1 tablet in the morning and evening).
Side effects
During therapy, the following side effects may develop:
- Reversible hematological disorders;
- Hypoglycemia;
- Dyspeptic symptoms (very rare);
- Allergic reactions including erythema, pruritus, and hives (rare, usually mild).
Overdose
In case of an overdose, hypoglycemic conditions of varying severity develop. Symptoms of moderate hypoglycemia are malaise, pallor, excessive sweating, and tachycardia. Adjustment of the Diabeton dose is indicated. In severe cases, disturbances of consciousness, coma, and cerebral edema are also possible. Intravenous administration of a glucose solution is shown, with cerebral edema - an osmotic diuretic mannitol. Monitoring of glucose, urea nitrogen and serum electrolytes is required.

Do not take Diabeton and miconazole at the same time.
During therapy, you need to regularly monitor the blood glucose level after meals, on an empty stomach and the "daily curve".
To prevent the development of dyspeptic symptoms, it is recommended to take Diabeton with meals.
Therapy does not eliminate the need for a low-calorie, low-carbohydrate diet.
Influence on the ability to drive vehicles and complex mechanisms
Patients receiving Diabeton should be careful while driving vehicles and performing potentially hazardous types of work that require increased attention and speed of reactions.
Application during pregnancy and lactation
Diabeton is contraindicated in pregnant women and breastfeeding women. If it is necessary to take the drug during lactation, breastfeeding should be discontinued.
Pediatric use
Due to the lack of data on the efficacy and safety of gliclazide in pediatrics, Diabeton is not used to treat children and adolescents under the age of 18 years.
With impaired renal function
Diabetone is prohibited for patients with severe renal failure.
For violations of liver function
Diabetone is prohibited for patients with hepatic impairment.
Drug interactions
With the simultaneous use of Diabeton with some drugs, a change in its effectiveness may be observed:
- Barbiturates and diuretics: decreased effectiveness;
- Sulfonamides, non-steroidal anti-inflammatory drugs, beta-blockers: increased effectiveness.
