Terzhinan is a combined agent for topical use in gynecology with antibacterial, anti-inflammatory, antiprotozoal and antifungal effects.
Release form and composition
Dosage form - vaginal tablets: oblong, beveled, flat, the letter T is applied on both sides, light yellow in color, there may be light or darker inclusions (6 or 10 pcs. In a strip, in a cardboard box 1 strip).
1 tablet of Terzhinan contains:
- active ingredients: ternidazole - 200 mg, nystatin - 100,000 IU, neomycin sulfate - 100 mg or 65,000 international units (IU), prednisolone sodium metasulfobenzoate - 4.7 mg (3 mg prednisolone);
- auxiliary components: lactose monohydrate, wheat starch, sodium carboxymethyl starch, colloidal silicon dioxide, magnesium stearate.
Pharmacodynamics and pharmacokinetics
Ternidazole, which is part of Terzhinan, causes the death of Trichomonas and anaerobic microorganisms (including facultative anaerobes of the genus Gardnerella).
The mechanism of action of neomycin is due to its ability to irreversibly disrupt protein synthesis in pathogen cells (neomycin acts at the ribosomal level, disrupting the transcription of the genetic code). Listeria, corynebacterium, methicillin-resistant Staphylococcus aureus and other aerobes are sensitive to neomycin, as well as gram-negative bacteria (including Pseudomonas aeruginosa).
Nystatin belongs to the group of polyene antibiotics and is an antimycotic agent (acts only on eukaryotes). It binds to ergosterol contained in the cell membrane of the fungus, thereby provoking the death of the fungus. Fungi of the genus Candida (including Candida glabrata and Candida albicans) are sensitive to nystatin.
Prednisolone, also part of Terzhinan, is a glucocorticosteroid. When applied topically, it has antiallergic, antiexudative and anti-inflammatory effect.
The auxiliary components of the drug ensure the integrity of the vaginal epithelium in inflammatory and infectious diseases and maintain the pH level within normal limits.
The pharmacokinetics of Terzhinan have not been studied, since the systemic absorption of the drug is negligible.
Indications for use
The use of Terzhinan is indicated for the treatment of vaginitis caused by microorganisms sensitive to the drug, including:
- trichomoniasis of the vagina;
- bacterial vaginitis;
- vaginitis caused by fungi of the genus Candida albicans;
- vaginitis caused by a mixed infection (anaerobic infection, Trichomonas and yeast-like fungi).
In addition, the drug is prescribed for the prevention of urogenital infections or vaginitis before childbirth and gynecological operations, abortions, hysterography (intrauterine examination), before and after the installation of an intrauterine device and diathermocoagulation of the cervix.
Contraindications
According to the instructions, Terzhinan cannot be used by patients with individual hypersensitivity to its components.
During pregnancy, the use of the drug is possible from the second trimester, in the first trimester and during breastfeeding only if, in the doctor's opinion, the expected effect of therapy for the mother exceeds the potential threat to the fetus or child.
Instructions for the use of Terzhinan: method and dosage
The tablets are intended for vaginal administration by deep insertion into the vagina. The procedure is carried out at bedtime, in the supine position. Before administration, the tablet should be moistened with water for 0.5 minutes.
Recommended dosage: 1 tablet once a day. After the procedure for 10-15 minutes, you must maintain a horizontal position.
The duration of the course of treatment is 10 days; with confirmed mycosis - up to 20 days; prophylactic use - an average of 6 days.
Side effects
- local reactions: rarely - itching, burning sensation and irritation in the vagina (mainly at the beginning of use);
- others: in some cases - the development of allergic reactions.
