TeraFlu LAR is an antimicrobial and anesthetic drug for local use in dentistry and otolaryngology.

Release form and composition

Dosage forms of TeraFlu LAR:

  • lozenge tablets: biconvex, round, on one side there is a Zyma logo in the form of two triangles arranged symmetrically with some offset, on the other - the “OR” code; large white tablets with a yellowish tinge with a pleasant orange flavor (8 pcs. in blisters, in cartons of 1, 2 or 3 blisters);
  • spray for topical application: a clear, colorless solution with a pleasant menthol aroma (30 ml in high-density polyethylene bottles, in cardboard packs, 1 bottle with a spray device included).

Composition for 1 tablet of TheraFlu LAR:

  • active ingredients: benzoxonium chloride - 1 mg; lidocaine hydrochloride - 1 mg;
  • auxiliary ingredients: microcrystalline cellulose, sodium chloride, sodium saccharinate, sorbitol, macrogol 6000, corn starch, magnesium stearate, citric acid, orange flavor.

The content of a sweetener (in the form of sorbitol) in one tablet is 1 g, which corresponds to an energy value of ~ 4 kcal, or 17 kJ.

Composition for 1 ml of TeraFlu LAR spray:

  • active ingredients: benzoxonium chloride - 2 mg; lidocaine hydrochloride - 1.5 mg;
  • auxiliary ingredients: glycerol, hydrochloric acid, menthol, peppermint oil, ethanol 96%, purified water.

Pharmacological properties

Pharmacodynamics

Active components of TeraFlu LAR:

  • benzoxonium chloride: N-benzyl N-dodecyl N, N-di (2-hydroxyethyl) ammonium chloride - salts of tetrasubstituted ammonium (quaternary ammonium bases), which have membranotropic activity due to the cationic structure and have a pronounced antibacterial effect against gram-positive and gram-negative (less degree) of microorganisms; the substance also has antimycotic and antiviral activity against membrane viruses, including herpes, influenza and parainfluenza viruses;
  • lidocaine hydrochloride: a local anesthetic that relieves inflammation of the throat during swallowing.

Pharmacokinetics

  • benzoxonium chloride: practically not absorbed into the systemic circulation and is not detected in the blood, its accumulation in the tissues of the body is also not detected; about 1% of the dose received is determined in urine;
  • lidocaine hydrochloride: absorbed by contact with the oral mucosa; the metabolism of a substance occurs by first passing through the liver; oral bioavailability is ~ 35%; metabolites of lidocaine are excreted by the kidneys, less than 10% of the active substance is excreted unchanged.

Indications for use

TeraFlu LAR is recommended for the local treatment of such infectious and inflammatory diseases of the oral cavity and pharynx as laryngitis, pharyngitis, catarrhal tonsillitis, ulcerative gingivitis, stomatitis. The drug is also included in the complex therapy of chronic tonsillitis as an adjunct.

Contraindications

  • pregnancy period (for tablets - I trimester);
  • breastfeeding (lactation);
  • children under 4 years of age;
  • individual hypersensitivity to ammonia compounds and any of the components of the drug.

Instructions for use of TheraFlu LAR: method and dosage

TeraFlu LAR is used topically, in the oral cavity. The tablets dissolve slowly until completely dissolved. Spray using a special device is sprayed into the oral cavity, holding the can upright.

Recommended dosage:

  • children aged 4 years and older: 1 tablet every 2-3 hours or 2-3 sprays 3 to 6 times a day; the maximum daily dose is 6 tablets;
  • adult patients: 1 tablet every 2-3 hours or 4 sprays (~ 0.5 ml) from 3 to 6 times a day; with severe symptoms of the disease, it is allowed to use 1 tablet every 1-2 hours; the maximum daily dose is 10 tablets.

The duration of the course of therapy is no more than 5 days.

If there is no relief of symptoms of the disease within 5 days of using the drug, the patient should definitely consult a doctor.

Side effects

  • reactions at the site of application: infrequently - irritation of a transient nature; continuous use of the drug for more than two weeks can cause reversible brown pigmentation of the tongue or teeth;
  • hypersensitivity reactions: in isolated episodes - facial edema, urticaria, laryngeal edema.

Overdose

Overdose symptoms in case of accidental swallowing of high doses of the drug inside (as is the case with other ammonium compounds) may be nausea and / or vomiting. Lidocaine in the drug contains a small amount, and therefore its overdose is unlikely.

In case of an overdose of TeraFlu LAR, it is recommended to drink milk or egg white whipped in water.

During therapy, it is required to avoid drinking alcoholic beverages.

Application during pregnancy and lactation

Since adequate and strictly controlled clinical studies of the use of the drug during pregnancy have not been conducted, there is no data on its safety and efficacy in this group of patients. In this regard, TeraFlu LAR tablets are contraindicated for use in the first trimester of pregnancy; in the second and third trimesters, their use should be avoided. Spray TeraFlu LAR is prohibited for use during the entire gestation period.

There are no clinical data on the excretion of the active substances of TheraFlu LAR in breast milk. Nevertheless, it is not recommended to use the drug during lactation.

Pediatric use

According to the instructions, TeraFlu LAR is contraindicated for use in children under 4 years of age. In children over 4 years of age, the drug is used according to indications according to the recommended dosage regimen.

Drug interactions

In the case of simultaneous use with agents with anionic activity, for example, with toothpaste, the effectiveness of benzoxonium chloride decreases.

Ethanol increases the absorption of benzoxonium chloride, so alcohol should be avoided during treatment.
