Intralipid - means of parenteral nutrition; a fat emulsion that replenishes the deficiency of energy substrates, essential fatty acids in the body.

Release form and composition

The drug is produced in the form of an emulsion for infusion (10 or 20%): homogeneous, white or white with a cream shade (100, 250 or 500 ml each in a colorless glass bottle, sealed with a butyl rubber stopper, rolled in an aluminum cap with control of the first opening; c cardboard box of 10 or 12 bottles with or without plastic holders and instructions for the use of Intralipid).

1000 ml of emulsion for infusion contains:

  • active substance: purified soybean oil - 100 or 200 g;
  • additional components: glycerol (anhydrous), egg yolk phospholipids, water for injection, sodium hydroxide (for adjusting the pH to 8).

Indicators (per 1000 ml of 10/20% emulsion): osmolality - 300/350 mosmol / kg water; energy value - 1100 kcal / l (4600 kJ / l) / 2000 kcal / l (8400 kJ / l); pH - 7.5-8.5.

Pharmacological properties

Pharmacodynamics

Intralipid is a source of energy and makes up for the lack of essential fatty acids in the body. The composition of the fat emulsion includes purified soybean oil, emulsified with purified phospholipids isolated from egg yolk. Soybean oil is a mixture of triglycerides, mainly polyunsaturated fatty acids. The biological properties and size of the lipid globules of the drug are similar to those for chylomicrons, formed by intestinal cells of particles of neutral fat, with a diameter of about 1 μm.

Indications for use

  • parenteral nutrition: as a source of energy and essential fatty acids; especially necessary for polytrauma, sepsis, severe burns and other conditions associated with hypercatabolism;
  • deficiency of essential fatty acids: in the presence of a deficiency of essential fatty acids, in which it is impossible to independently replenish the normal balance of essential fatty acids through oral consumption.

Contraindications

Absolute:

  • severe lipid metabolism disorders, including with pathological hyperlipidemia;
  • acute stage of shock;
  • hypersensitivity to any of the constituents of Intralipid.

Relative contraindications for which it is recommended to use the agent, observing caution: allergy to soy protein (the emulsion can only be used after allergic tests), as well as diseases leading to lipid metabolism disorders: pancreatitis, decompensated diabetes mellitus, severe renal and / or liver failure, hypothyroidism (against the background of concomitant hypertriglyceridemia).

Intralipid, instructions for use: method and dosage

The Intralipid emulsion is intended for parenteral nutrition.

The dose and method of administration of the drug are determined according to the individual ability of the patient to remove it. The assessment of lipid elimination is carried out by setting the serum triglyceride concentration. Intralipid can be administered via the Y-shaped connector, or simultaneously into the same vein (central or peripheral) into which the amino acid and / or carbohydrate solution is injected.

The drug can also be added under aseptic conditions to a plastic (polymer) phthalate-free container, as one of the parts of the total All-in-One mixture, which includes amino acids, carbohydrates, vitamins, trace elements and Dipeptiven. In this case, it is required to control the physical stability of the mixture.

When using the drug in adults, the recommended maximum daily dose is 3 g / kg of triglycerides, which is equivalent to 30 ml / kg for 10% emulsion or 15 ml / kg for 20%. The drug can cover up to 70% of energy needs. At 10 or 20% Intralipid, the infusion rate in adults should not exceed 500 ml in 5 hours.

When prescribing the drug to newborns and young children, the recommended daily dose may be 0.5–4 g / kg of triglycerides, which is equivalent to 5–40 ml / kg of 10% or 2.5–20 ml / kg of 20% emulsion. The infusion rate of triglycerides should not be higher than 0.17 g / kg / hour (4 g / kg / 24 hours). For premature infants and children born with low birth weight, it is advisable to infusion Intralipid continuously throughout the day. The initial daily dose is 0.5–1 g / kg, if necessary, an increase of 0.5–1 g / kg per day up to a dose of 2 g / kg is allowed. A further increase in the daily dose to 4 g / kg is possible only with strict monitoring of the serum triglyceride concentration, hepatic function indicators and the degree of blood oxygen saturation. In case of a previously missed dose,in order to compensate, exceeding this maximum level is not allowed.

To prevent or correct the deficiency of essential fatty acids, it is recommended to use Intralipid in order to ensure that the body receives a sufficient amount of linolenic and linoleic acids and 4–8% of non-protein energy. Against the background of a stressful state in combination with a deficiency of unsaturated fatty acids, it is required to increase the dose of Intralipid administered.

Side effects

During the period of using Intralipid, rarely, with a frequency of less than 1%, side effects are noted in the form of nausea, vomiting, and fever.

Information about other disorders associated with Intralipid infusions is recorded extremely rarely, less than one in 1 million injections:

  • immediate or early unwanted effects: allergic reactions, including skin rash, urticaria, anaphylactic reactions; circulatory disorders in the form of a decrease / increase in blood pressure (BP); tachypnea, increased fatigue, abdominal pain, headache, reticulocytosis, hemolysis, priapism (with a frequency of less than 0.0001%);
  • delayed side effects: with prolonged use - thrombocytopenia (in newborns), a transient increase in liver function tests (with a frequency of less than 0.0001%); generally, all symptoms are reversible at the end of the infusion.

Overdose

In case of violation of the elimination of Intralipid, infection or the development of functional impairment of the kidneys may occur, due to the appearance of the syndrome of excessive fat load. This syndrome is characterized by fever, hyperlipidemia, fatty infiltration, dysfunction of various organs and coma. All of the above symptoms of overdose are relieved after discontinuation of drug infusions.

During the period of use of parenteral nutrition in the blood serum, it is necessary to control the level of triglycerides.

Neonates and premature infants with hyperbilirubinemia, as well as suspected pulmonary hypertension, require careful monitoring during infusion. In newborns, especially premature infants, against the background of prolonged parenteral nutrition, it is imperative to monitor liver function tests and platelet count.

It should be borne in mind that Intralipid can affect the results of some laboratory parameters, including lactate dehydrogenase, bilirubin, hemoglobin, blood oxygen saturation. For this reason, these studies are recommended to be carried out 5-6 hours after the completion of the drug infusion.

Any remaining fat emulsion from an open container must be discarded.

Influence on the ability to drive vehicles and complex mechanisms

  • vitamin solutions for parenteral nutrition (Vitalipid for children, Vitalipid for adults, Soluvit): these drugs can be administered together with Intralipid, provided that the solutions are mixed in compliance with aseptic requirements;
  • heparin: an increase in plasma lipolysis is recorded, which causes a temporary decrease in the clearance of triglycerides as a result of suppression of lipoprotein lipase;
  • insulin: There is an effect on lipase activity, but this effect is of limited clinical significance.
