Diprospan
Diprospan

Diprospan is a glucocorticoid (adrenal cortex) drug intended for the treatment of inflammatory diseases.

pharmachologic effect

Diprospan has anti-inflammatory, immunosuppressive (suppression of immunity), antiallergic, anti-shock and desensitizing (decreased sensitivity of the body) properties.

The active ingredient, betamethasone, is presented in Diprospan in two forms: betamethasone disodium phosphate, which ensures the rapid onset of the therapeutic effect, and betamethasone dipropionate, which serves to prolong the action of the drug.

The widespread use of Diprospan in various diseases is due to its ability to inhibit the synthesis of substances that cause an inflammatory reaction, blocking the development of the inflammatory process at the cellular level.

Diprospan has a weakly pronounced ability to retain sodium in the body and enhance the release of hydrogen, potassium and ammonium ions, however, when the drug is used in recommended doses, these properties have no clinical significance.

Diprospan, the price had no effect on it, is one of the most popular glucocorticoid drugs.

Indications for the use of Diprospan

The instruction recommends using Diprospan for:

  • diseases of the soft tissues and the musculoskeletal system: bursitis (inflammation of the periarticular bursa), osteoarthritis (damage to the cartilage tissue of the surface of the joints), rheumatoid arthritis (inflammation of the connective tissue and damage to the joints), epicondylitis (degenerative process at the site of muscle attachment to the humerus), sciatica (damage to the nerve fibers that extend from the spinal cord), lumbago (acute lower back pain), torticollis, fasciitis (pain in the heel with stress), foot diseases;
  • allergic diseases: hay fever, bronchial asthma, rhinitis, allergic bronchitis, drug allergy, serum sickness (allergy to the administration of drugs with a large amount of protein), allergy to insect bites;
  • dermatological diseases: neurodermatitis (the appearance of itchy areas of redness on the skin), coin-like eczema (inflammatory skin rashes), contact dermatitis, atopic dermatitis, urticaria, severe photodermatitis, lichen planus, psoriasis, pemphigus vulgaris, cystic acne, herpetic dermatitis;
  • systemic connective tissue diseases: periarteritis nodosa (lesion, inflammation of small arteries with their subsequent necrosis), dermatomyositis (systemic damage to the muscles and skin), scleroderma (deposition of scar tissue), systemic lupus erythematosus (damage to the skin and various internal organs);
  • hemoblastosis (tumor diseases of the lymphatic and hematopoietic tissue);
  • insufficiency of the adrenal cortex;
  • other pathological conditions and diseases: nephritis, nephrotic syndrome, ulcerative colitis, adrenogenital syndrome, malabsorption syndrome (loss of nutrients as a result of insufficient absorption in the small intestine), regional ileitis (disease of the small intestine).

Instructions for the use of Diprospan

Diprospan injections
Diprospan injections

Diprospan is available in the form of a solution or suspension for injection.

Diprospan injections can be administered intramuscularly, intradermally, perarticular (around the joint) or intraarticular way. Diprospan injections should not be administered intravenously.

According to the instructions for Diprospan, the drug should be used intramuscularly every 2-4 weeks, 1-2 ml. Diprospan injections are intra-articular and perarticularly injected in the following doses: in the hip joint - 1-2 ml, shoulder, ankle, knee - 1 ml, carpal, elbow - 0.5-1 ml, interphalangeal, metacarpophalangeal, sternoclavicular - 0.25 -5 ml.

When using Diprospan directly into the lesion by intradermal injection, a weekly dose of 1 ml should not be exceeded. Depending on the nature of the disease, local infiltration involves the introduction of 0.25 to 2 ml of Diprospan.

As a rule, Diprospan injections are painless, but in some cases it is necessary to administer the drug together with an anesthetic in order to eliminate painful sensations.

Side effects

A single use of Diprospan or a short course of treatment does not cause significant negative reactions.

The possible side effects of the drug and the price of Diprospan are commensurate with its effectiveness.

According to the instructions, Diprospan with prolonged use can provoke:

  • hypernatremia, increased excretion of potassium and calcium from the body, fluid retention in tissues, increased body weight;
  • increased blood pressure, chronic heart failure;
  • steroid myopathy (metabolic disorders in muscle tissue), muscle weakness, osteoporosis (thinning of bone tissue), tendon ruptures, fractures of tubular bones, aseptic necrosis of the humerus and femur head;
  • hiccups, flatulence, pancreatitis, erosive and ulcerative lesions of the digestive tract;
  • increased sweating, steroid acne, dermatitis, atrophy and thinning of the skin, impaired wound healing;
  • increased intracranial pressure, convulsions, headache, dizziness, mood changes, increased irritability, depression, insomnia;
  • secondary adrenal insufficiency, menstrual irregularities, steroid diabetes mellitus, growth retardation and sexual development in children;
  • glaucoma, increased intraocular pressure, cataracts;
  • arterial hypotension, angioedema, shock, anaphylactic reactions;
  • aseptic abscesses, subcutaneous and cutaneous atrophy.

Contraindications

In the treatment of many diseases, Diprospan, the price does not matter, is quite effective, however, doctors do not prescribe this drug for systemic mycoses and hypersensitivity to betamethasone and other glucocorticoid drugs.

The use of Diprospan is prohibited in the intervertebral space in the infected surfaces, and intra-articular administration is contraindicated in infectious arthritis and an unstable joint.

The instruction recommends to use Diprospan with caution in cirrhosis of the liver, peptic ulcer, recent intestinal anastomoses (connection of intersected areas), renal failure, osteoporosis, hypertension, chickenpox, diabetes, thrombophlebitis.

Since there are no reliable data on the safety of Diprospan for the mother and the fetus, the doctor should make a decision on the advisability of using the drug during pregnancy, comparing all the risks and benefits.
