Dexon is a combined agent for topical use in otolaryngology and ophthalmology, anti-inflammatory and antibacterial action.

Release form and composition

Dosage form: solution-eye and ear drops (5 ml each in dark glass bottles, complete with a dropper, 1 set in a cardboard box).

Content of active ingredients in 1 ml:

  • Neomycin sulfate - 5 mg;
  • Dexamethasone sodium phosphate - 1 mg.

Excipients: hydrogen phosphate disodium, benzalkonium chloride (preservative), ethylenediaminetetraacetic acid disodium salt, creatinine, sodium chloride, propylene glycol, sodium metabisulfite, water for injection.

Pharmacological properties

Pharmacodynamics

Neomycin is an aminoglycoside antibiotic. Produced by Streptococcus fradiae. Acts bactericidal, disrupting protein synthesis in microbial cells. Active against gram-negative and gram-positive microorganisms, including Streptococcus pneumoniae, Staphylococcus aureus, Shigella, E. coli and Proteus. Has low efficacy against streptococci and P. aeruginosa. Neomycin has no effect on anaerobic microorganisms, viruses and pathogenic fungi. Antibiotic resistance develops slowly and to a small extent.

Dexamethasone is a glucocorticosteroid. It has a desensitizing, anti-allergic and anti-inflammatory effect. Actively suppresses the process of inflammation, inhibiting the migration of mast cells, the release of inflammatory mediators by eosinophils and reducing capillary permeability. In combination with neomycin, the risk of an infectious process is reduced.

Pharmacokinetics

After instillation of the drug, neomycin is almost not absorbed into the bloodstream. It is found in the vitreous body, corneal stroma and moisture of the anterior chamber within 6 hours.

Aminoglycoside antibiotics are not metabolized and are excreted by the kidneys unchanged, so their concentration in the urine is quite high. The rate of excretion depends on renal function, patient age and comorbidities. The half-life of aminoglycosides is 2–4 hours in adult patients with normal renal function, 5–8 hours in neonates, and 2.5–4 hours in older children. With impaired renal function, the half-life of aminoglycosides can increase to 70 hours or more.

After instillation of the drug into the conjunctival sac, dexamethasone penetrates into the epithelium of the conjunctiva and cornea. The highest therapeutic concentrations are found in the anterior chamber of the eye, vitreous body and cornea within 1.5–2 hours after the instillation of Dexon drops and persist for 4–8 hours. The absorption of dexamethasone into the systemic circulation in the local treatment of ear and eye diseases is insignificant.

Indications for use

  • Chronic and acute form of bacterial blepharitis, conjunctivitis, iridocyclitis, keratitis (in the absence of damage to the epithelium);
  • Prevention of inflammation of the anterior part of the eye after surgery;
  • Acute and chronic otitis externa;
  • Allergic and infectious pathologies of the outer ear.

Contraindications

  • Chicken pox;
  • Keratitis caused by Herpes simplex or Varicella zoster virus (treelike keratitis);
  • Mycobacterial eye infections;
  • Diseases of the conjunctiva and cornea of viral etiology;
  • Fungal eye pathologies;
  • Application after removal of a foreign object from the cornea without complications;
  • Breastfeeding period;
  • Childhood;
  • Hypersensitivity to drug components.

Instructions for use of Dexona: method and dosage

Dosing of the drug for the treatment of moderate or mild infectious eye disease - 1-2 drops in the conjunctival sac, instill every 4-6 hours. In the case of a severe form of the infectious process, instillation is recommended to be carried out every hour, the interval between instillations is increased as the inflammatory phenomenon decreases.

Dosing in the treatment of ear diseases - 3-4 drops are instilled 2-4 times a day.

Side effects

The drug can cause side effects due to an antibiotic or glucocorticosteroid (GCS), as well as their combination.

Development of allergic reactions: often in the form of swelling of the eyelids, itching, redness of the conjunctiva (due to the action of neomycin).

From the side of the organ of vision: it is possible - a slowdown in the wound healing process, an increase in intraocular pressure with a possible risk of developing glaucoma, against the background of which there is a characteristic lesion of the optic nerve and visual field, the formation of posterior subcapsular cataract (due to the action of dexamethasone).

The use of drops in the treatment of pathologies that contribute to the thinning of the sclera or cornea can cause perforation of the fibrous membrane of the eye.

The combined agent (antibiotic with GCS) can cause the development of a secondary infection, as a consequence of the antibiotic suppression of the protective function of the patient's body. In the treatment of acute purulent infections of the eye, the presence of GCS in the composition can hide or intensify the inflammatory process.

Prolonged use of drops containing corticosteroids can cause damage to the cornea by a fungus. The appearance of non-healing ulcers indicates the development of fungal invasion.

Overdose

In case of drug overdose, local adverse reactions are possible.

There is no specific antidote. It is necessary to cancel the drug and prescribe symptomatic treatment.
